During natural or transportation disasters, industrial accidents, shootings, suicides or dozens of other instances, police officers, firefighters, and paramedics are called upon to assist both injured and uninjured people. Although often romanticized in television series and in films, the real-life tasks of public safety professionals are usually unpleasant--restraining violent individuals and removing accident, homicide, and suicide victims from death scenes--and always highly stressful. They are frequently subjected to additional stress when their efforts are criticized by family members of the injured or deceased. Although stress can be harmful, even fatal, police officers, firefighters, and paramedics can have more productive and satisfying lives when they learn to positively control stress, rather than be controlled by it. This English language bibliography consisting of more than 700 references, covering the time period 1945 to early 1989, can help these and other professionals manage stress more effectively. Those disparities are caused not only by fundamental differences in health status across segments of the population, but also because of inequities in factors that impact health status, so-called determinants of health. Only part of an individual's health status depends on his or her behavior and choice; community-wide problems like poverty, unemployment, poor education, inadequate housing, poor public transportation, interpersonal violence, and decaying neighborhoods also contribute to health inequities, as well as the historic and ongoing interplay of structures, policies, and norms that shape lives. When these factors are not optimal in a community, it does not mean they are intractable: such inequities can be mitigated by social policies that can shape health in powerful ways. Communities in Action: Pathways to Health Equity seeks to delineate the causes of and the solutions to health inequities in the United States. This report focuses on what communities can do to promote health equity, what actions are needed by the many and varied stakeholders that are part of communities or support them, as well as the root causes and structural barriers that need to be overcome.

The tradi tional roles of the police had been those of law enforcement and the maintenance of public order. As a consequence police officers were typically large-bodied males, selected for their physical abilities and trained to accept orders and enforce the law. Over the past two decades, however, the industrialized nations have placed a variety of new demands on police officers. To traditional law enforcement and public order tasks have been added social work, mental health duties, and cORllluni ty relations work. For example, domestic disputes, violence between husbands and wives, lovers, relatives, etc. , have increased in frequency and severity (or at least there has been a dramatic increase in reporting the occurence of domestic violence). Our societies have no formal system to deal with domestic disputes and the responsibility to do so, in most countries, has fallen to the police. In fact, in some areas as many as 607. of calls for service to the police are related to domestic disputes (see the chapter in this text by Dutton). As a result the police officer has had to become a skilled social worker, able to intervene with sensi ti vi ty in domestic situations. Alternatively, in the case of West Germany, the officer has had to learn to work co-operatively with social workers (see the chapter by Steinhilper).

Many of the myriad of structured psychological tests that can be used in FFD evaluations are discussed. Medication issues in FFD evaluations are explored as well as the potential impact that medications may have on officers' fitness. Another section of the book explores police officers' expectations of mental health professionals and outlines stereotypes and expectations that law enforcement personnel have regarding mental health personnel. For evaluators, an understanding of the lack of information and misconceptions that officers have may assist them with interacting and educating officers and referring departments. Other sections consider gender and ethnic issues in hiring, stress, and FFD evaluations; duty death and the nature of the stress experienced by law enforcement personnel; police suicide and FFD; how to manage misfit officers; and legal considerations involved in the conduct of FFD evaluations. It is hoped that by reading this book, mental health professionals will be better able to understand and treat the difficulties that law enforcement professionals encounter in the performance of their hazardous and stressful public service. Book excerpt: This study seeks participation from patrol-level officers with at least two years’ experience in the New Castle County Police Department in Delaware (n=134) and requests that they respond to two questionnaires measuring resilience and job burnout: the 25-item Resilience Scale and the Maslach Burnout Inventory. Additionally, participants are asked to self-report their pre-hire fitness scores. Police officers are repeatedly exposed to trauma, which has negative mental and physical consequences, often leading to extreme stress, fatigue, and emotional imbalance. Repeated trauma also impacts physical fitness, which is critical for not only physical and mental health but also work performance, especially for police officers. Two related concepts are used to investigate the relationship between physical and mental health: resilience, which describes an individual’s capacity to recover quickly from difficulties, and burnout, a state of emotional, physical, and mental exhaustion caused by excessive and prolonged stress. The data generated by the aforementioned scales are compared to pre-hire physical fitness scores to observe the relationship between pre-hire fitness and current levels of burnout and resilience. This study aims to improve the hiring practices of police organizations by identifying characteristics that enable candidates to manage repeated exposure to trauma. Failing to hire resilient police officers can produce physically and mentally exhausted police populations that cannot provide the public with safe and effective policing. The study hypothesizes that pre-hire physical fitness scores influence measures of resilience and burnout in patrol-level police officer populations.

Book excerpt: The author hypothesized that reasonable investment of effort and funds into carefully constructed and well-monitored fitness programs will improve the performance of law enforcement personnel. The report, which is divided into three main sections, evaluates contemporary literature, examines existing programs, and emerging technology. It does not evaluate current, longitudinal studies that relate to possible improvement of a police officer's ability to perform specific tasks, but with physical fitness as a means of increasing the probability of preserving law enforcement operating personnel. Studies in the private sector report significant return for time and funds spent on fitness programs. Benefits, costs, and tradeoffs in fitness programs are examined. Studies indicating program effects on such factors as use of sick time, medical claims, medical retirements, individual competence, ability to recruit new personnel, and vulnerability to vicarious liability are discussed. Decision making about developing and implementing a physical fitness program are discussed, and details of management theory implemented in the Downey Police Department (California) are examined. Trends that may impact on the future of fitness programs in law enforcement are also discussed.
