A rebirth of urban family medicine has accompanied these pioneering efforts. To date, the accomplishments are substantial and the prospects are bright. Although diseases are generally the same wherever you are, their effects as illness on the individual and the family are strongly influenced by the environment and social milieu. Urban families have distinctive and diverse problems-cultural, economic, and ethnic. Training pro grams situated in the large cities must recognize these issues and include special emphasis on the situations that the family physician is likely to encounter during and after his training. There is very little research literature on the background and nature of special urban problems and these areas are the subject of several chapters of this long overdue volume devoted specifically to urban family medicine. Dr. Birrer has persuaded true experts to share their knowledge with the reader.

Book excerpt: As cities in developing countries grow and become more prosperous, energy use shifts from fuelwood to fuels like charcoal, kerosene, and coal, and, ultimately, to fuels such as liquid petroleum gas, and electricity. Energy use is not usually considered as a social issue. Yet, as this book demonstrates, the movement away from traditional fuels has a strong socio-economic dimension, as poor people are the last to attain the benefits of using modern energy. The result is that health risks from the continued use of wood fuel fall most heavily on the poor, and indoor pollution from wood stoves has its greatest effect on women and children who cook and spend much more of their time indoors. Barnes, Krutilla, and Hyde provide the first worldwide assessment of the energy transition as it occurs in urban households, drawing upon data collected by the World Bank Energy Sector Management Assistance Programme (ESMAP). From 1984-2000, the program conducted over 25,000 household energy surveys in 45 cities spanning 12 countries and 3 continents. Additionally, GIS mapping software was used to compile a biomass database of vegetation patterns surrounding 34 cities. Using this rich set of geographic, biological, and socioeconomic data, the authors describe problems and policy options associated with each stage in the energy transition. The authors show how the poorest are most vulnerable to changes in energy markets and demonstrate how the collection of biomass fuel contributes to deforestation. Their book serves as an important contribution to development studies, and as a guide for policymakers hoping to encourage sustainable energy markets and an improved quality of life for growing urban populations.

Far from being a compilation of undigested statistics, however, The Mexican Urban Household uses its rich data to vividly reveal how Mexican families use their every resource to defend themselves against a political and economic system that overwhelms and exploits them. It describes how families band together, sometimes with three generations in one small house, to minimize expenses and pool resources. It explores the limited range of available jobs, from secure but scarce bureaucratic positions to more common and less reliable jobs in blue-collar industries and the informal economy. And, most important, it traces the high cost to families, particularly to women, of the endless struggle to make ends meet. These important findings outline the dimensions of the economic crisis for ordinary Mexicans. It will be crucial reading not only for everyone interested in the future of Mexico but also for students of development throughout the Third World.

One of these is a Home-Based Economic Activity (HBEA), e.g. sales of home-made snacks or car maintenance. This study examines the prevalence, organisation and relevance of HBEAs in four neighbourhoods in the Caribbean cities Paramaribo (Suriname) and Port of Spain (Trinidad and Tobago). Recent economic developments in these countries diverge; Suriname recovers slowly from a crisis while Trinidad and Tobago's economy is buoyant. These economic features together with local political developments have produced distinct institutional contexts. This gives ground for a comparison between the two cities. In addition, the study discusses the relevance of currently popular policies on entrepreneurship and micro-finance. The above issues have been assessed through use of multiple quantitative and qualitative methods. The study shows that forty percent of households in the examined neighbourhoods earn money through operating HBEAs. These are mainly operated by women and assist households in improving their livelihoods from a level of survival to a level of security. Most HBEA-operators aim at earning additional incomes and reducing vulnerability. Only a small group meets the image of the classic operator who innovates, takes risk and aims at growth and profit. The two groups organize their HBEA in very distinct ways. Differences between Suriname and Trinidad and Tobago are small. First of all economic growth has limited impact on assets and vulnerability of low-income groups. Moreover, policies aiming at stimulation of entrepreneurship such as micro-credit are relevant to classic entrepreneurs and not to the largegroup of security-seeking HBEA-operators.
