Book excerpt: To address the high prevalence and costs associated with low back pain, U.S. Navy physical therapists currently provide back injury prevention education to requesting commands in the form of the Healthy Back Program. Although the content of the Healthy Back Program has face validity, no explicit evaluation of its effectiveness has been conducted. Because of the need to develop effective health interventions, a formal study is needed. The primary purposes of the present study are to: (a) present findings examining the immediate effect of the Healthy Back Program in changing knowledge about back injury prevention, and (b) provide a baseline assessment of back problems among incoming recruits. Correlates of back pain also are examined to suggest factors that may be amenable to intervention. Intervention recruits (n=1,772) received the Healthy Back Program brief, which is a slide presentation that focuses largely on information about back injury and prevention techniques. Immediately following the presentation, intervention subjects completed a short questionnaire assessing their history of back problems, demographic and health information, back knowledge, and nutrition knowledge. Control subjects (n=1,658) completed identical questionnaires but received no presentation. Intervention recruits had a significantly higher back knowledge score (67% correct) than controls (50% correct); nutrition knowledge differences were minimal (52% vs. 53% correct). About 41% of all recruits had ever experienced a back problem, 27% reported at least one back problem within the past year, and 11% reported back pain during recruit training. Liftlng, sports participation, and bending were reported as the leading causes of previous back problems. Although better self-reported health and fitness measures were associated with fewer low back problems, other correlates of back pain reported elsewhere (e.g., smoking, overweight) were not found.

The smoking experience of women entering the Navy was described, and their smoking prevalence was compared to civilians. The effect of an involuntary, 8-week smoking ban during recruit training on recruits' smoking perceptions and intentions was examined. Three months after leaving recruit training, baseline smokers were reassessed to determine the impact of the eight weeks of enforced cessation on smoking relapse. Finally, nonresponse to the 3-month assessment and its potential effect on validity of findings was investigated. About 5,500 women were surveyed over a 1-year period at entry to recruit training. Standardized comparisons showed the smoking rate among recruits was higher than civilians' (38.7% versus 28.8%, respectively) controlling for differences in age, race, and education. Age, education, and race/ethnicity were related to smoking prevalence at entry. As a result of the 8-week smoking ban, there was a 39% reduction in the percent of all women recruits who perceived themselves as smokers from entry to graduation from recruit training. Age, education, race/ethnicity, level of baseline smoking and addiction, and intentions to smoke were related to changes in perceptions of being a smoker, or the maintenance of consistent positive perceptions of being a nonsmoker. Similarly, changes in intentions to smoke were associated with sociodemographic and baseline smoking variables. Those with any smoking experience at entry were surveyed 3-months after leaving recruit training to determine subsequent short-term effects of the involuntary smoking ban. Response rate to the follow-up survey was 39%. Reluctant respondents were not different from those responding promptly to the original survey, but nonrespondents as a group were less educated, had a higher prevalence of baseline smoking, and were more frequent smokers that respondents. Sixty-eight percent had relapsed at the follow-up. Relapse rates varied by sociodemographic characteristics, entry and graduation smoking variables, entry-to-graduation changes in perceptions and intentions to smoke, and Navy factors. The type of smoker one was at entry to the Navy was an important predictor of relapse, with 89% of daily smokers smoking at the 3-month follow-up. Results suggest that the 8-week involuntary ban on smoking was useful in bringing about changes in self-reported smoking status and in helping some individuals stay off cigarettes once they leave recruit training, although policy alone cannot be expected to be effective for all smokers. High relapse rates particularly among heavier smokers underscore the need for educational/motivational efforts during recruit training to help maintain the quit status achieved during those eight weeks.

Book excerpt: In order to provide a baseline for the evaluation of programs to prevent lower back problems in the Navy, the present study investigated the incidence of inpatient back problems for enlisted active duty Navy personnel during 1974-1983. There were 13,109 individuals with a first hospitalization for a back problem during the decade, and the most frequent diagnosis was Vertebrogenic Pain Syndrome (36.7%). Individuals with Navy service of less than one year, as well as those with over 20 years, were most likely to be hospitalized. The 17-19 age group was the most likely to be hospitalized. Those occupations most likely to be associated with back problems were health care, weapons control, seaman-striker, general seamanship, and master-at-arms. There were 3,377 individuals with back problems who appeared before a medical board and 2,313 who appeared before a physical evaluation board. The two diagnoses that most often led to a medical board or a physical evaluation board were Displacement of Intervertebral Disk and Vertebrogenic Pain Syndrome. The most common disposition of a physical evaluation board was an award of 10% disability (35.9%). The results are discussed in terms of greatest-risk groups and back problem prevention. (SDW).
