Each tracing represents the combined interpretation of the plate made by skilled observers who were in every instance familiar with the clinical aspects of the case. The writings of many who have contributed their experience to the literature of fractures have been consulted. Those to whom I feel indebted for suggestions are mentioned in the section on Bibliography. References to literature are not made in the text. I take this opportunity to extend my thanks to the members of the Surgical Staff of the Massachusetts General Hospital for their courtesy in permitting me to study cases of fracture of the lower extremity in the wards of the hospital, and to Professor Thomas Dwight for the use of valuable anatomical material. I also thank Dr. F. J. Cotton for an untiring interest in the pro duction of most of the drawings, and in the search for fracture literature. The special interest and importance, in a surgical sense, of the great European War lies not so much in the fact that examples of every form of gross lesion of organs and limbs have been seen, for if we read the older writers we find little in the moderns that is new in this respect, but is to be found in the enormous mass of clinical material which has been presented to us and in the production of evidence sufficient to eliminate sources of error in determining important conclusions. For the first time also in any campaign the labours of the surgeon and the physician have had the aid of the bacteriologist, the pathologist, the physiologist, and indeed of every form of scientific assistance, in the solution of their respective problems. The clinician entered upon the great war armed with all the resources which the advances of fifty years had made available. If the surgical problems of modern war can be said not to differ sensibly from the campaigns of the past, the form in which they have been presented is certainly as different as are the methods of their solution. The achievements in the field of discovery of the chemist, the physicist, and the biologist have given the military surgeon an advantage in diagnosis and treatment which was denied to his predecessors, and we are able to measure the effects of these advantages when we come to appraise the results which have been attained. And here I would enquire how many young practitioners have the means to purchase mechanical appliances for the treatment of fractures indiscriminately, in order that they may be able to compete with old practitioners, or be prepared for any emergency? It has been said that accident makes the man. Would accident make the man if he had not the knowledge to take advantage or cir'cumstances? I can conceive how accident might give us wealth; but accident, developing wealth or social position, and accident developing one's mental and scientific resources, are two things quite separate and distinct. I would ask of what service are splints? What office do they perform in the treatment of fractures? Authors say they sup port the bone while union is being effected. I would ask, how many fractures of the long bones can be placed in apposition without the use of extension, particularly in oblique fractures, or where there has been great injury to soft parts, and overlapping of the bones? I know of few exceptions. The true use of splints should be to keep the fractured ends of the bone in apposition, by placing the muscles on the stretch, and thereby making them (the muscles) the true splints. The experiments of Reid and others show that muscles are not susceptible of being stretched beyond their normal capacity; that when so stretched, they are capable of bearing great lateral pressure without much deflection; and any attempt at undue lateral pressue [results in rupture of the muscular substance.
