Book excerpt: Thermal ablation of benign prostatic hyperplasia (BPH) and prostate cancer is a minimally-invasive alternative to standard surgical prostate treatments. Although minimally invasive, many of the current prostate thermal ablation technologies lack the ability to precisely treat large prostate regions while avoiding sensitive surrounding structures. This Ph.D. dissertation describes the development and evaluation of directional transurethral ultrasound applicators capable of precise image-guided prostate ablation. MR compatible, transurethral ultrasound applicators were designed and fabricated using either planar, sectored tubular, or slightly focused curvilinear transducers. The applicator designs consisted of a linear array of separately powered transducers integrated onto a custom multi-lumen catheter, which could be rotated within an inflatable urethral cooling balloon. A computer-controlled, MR compatible motor system was developed to rotate the applicators during thermal procedures. Controlled image-guided ablation of prostate tissue was demonstrated in multiple in vivo canine experiments. An open bore interventional MR system allowed for direct access during the thermal procedures with concurrent MR temperature imaging. Single stationary sonications of the directive transurethral applicators ablated selective prostate regions (20-70°) out to prostate capsule. Larger ablation zones were then created by rotating the directive applicators in discrete angular steps. The depth of ablation at each angle was controlled by continuously regulating the applied power level and sonication duration with MR temperature feedback as guidance. An acoustic and biothermal model of transurethral ultrasound thermal therapies was also developed and verified in this work. Studies with the biothermal model demonstrated that all three applicator types could conformably ablate prostate tissue out to the prostate capsule boundary while avoiding thermal damage to rectal tissues. The curvilinear applicator proved the most selective applicator and the tubular applicator required the shortest treatment times and was the easiest to control. In summary, this work represents a major advance in image-guided prostate thermal therapy. There is proven potential for directive transurethral ultrasound applicators and the technologies developed herein can be easily modified to treat other intraluminal locations such as esophageal and rectal pathologies. With added development and verification, the biothermal model may provide a base for future thermal treatment planning systems.

A further compounding factor of concern to clinicians and scientists has been the use of higher and higher intensities by the manufacturers of ultrasound equipment, particularly higher peak pulse intensities.
