Book excerpt: The blowout of the Macondo well on April 20, 2010, led to enormous consequences for the individuals involved in the drilling operations, and for their families. Eleven workers on the Deepwater Horizon drilling rig lost their lives and 16 others were seriously injured. There were also enormous consequences for the companies involved in the drilling operations, to the Gulf of Mexico environment, and to the economy of the region and beyond. The flow continued for nearly 3 months before the well could be completely killed, during which time, nearly 5 million barrels of oil spilled into the gulf. Macondo Well-Deepwater Horizon Blowout examines the causes of the blowout and provides a series of recommendations, for both the oil and gas industry and government regulators, intended to reduce the likelihood and impact of any future losses of well control during offshore drilling. According to this report, companies involved in offshore drilling should take a "system safety" approach to anticipating and managing possible dangers at every level of operation-from ensuring the integrity of wells to designing blowout preventers that function under all foreseeable conditions-in order to reduce the risk of another accident as catastrophic as the Deepwater Horizon explosion and oil spill. In addition, an enhanced regulatory approach should combine strong industry safety goals with mandatory oversight at critical points during drilling operations. Macondo Well-Deepwater Horizon Blowout discusses ultimate responsibility and accountability for well integrity and safety of offshore equipment, formal system safety education and training of personnel engaged in offshore drilling, and guidelines that should be established so that well designs incorporate protection against the various credible risks associated with the drilling and abandonment process. The Gulf of Mexico is likewise vitally important to the entire nation as a bountiful source of food, energy and recreation. The Gulf Coast's unique culture and natural beauty are world-renowned. There is no place like it anywhere else on Earth. On April 20, 2010 the eyes of the world focused on an oil platform in the Gulf, approximately 50 miles off the Louisiana coast. The mobile drilling unit Deepwater Horizon, which was being used to drill an exploratory well for BP Exploration and Production, Inc. (BP), violently exploded, caught fire and eventually sank, tragically killing 11 workers. But that was only the beginning of the disaster. Oil and other substances from the rig and the well head immediately began flowing unabated approximately one mile below the surface. Initial efforts to cap the well were unsuccessful, and for 87 days oil spewed unabated into the Gulf. Oil eventually covered a vast area of thousands of square miles, and carried by the tides and currents reached the coast, polluting beaches, bays, estuaries and marshes from the Florida panhandle to west of the Mississippi River delta. At the height of the spill, approximately 37% of the open water in the Gulf was closed to fishing. Before the well was finally capped, an estimated 5 million barrels (210 million gallons) escaped from the well over a period of approximately 3 months. In addition, approximately 771,000 gallons of dispersants were applied to the waters of the spill area, both on the surface and at the well head one mile below. It was an environmental disaster of unprecedented proportions. It also was a devastating blow to the resource-dependent economy of the region. While the extent of natural resources impacted by the Deepwater Horizon oil spill and response (collectively, “the Spill”) is not yet fully evaluated, impacts were widespread and extensive. The full spectrum of the impacts from this spill, given its magnitude, duration, depth and complexity, will be difficult to determine. The trustees for the Spill, however, are working to assess every aspect of the injury, both to individual resources and lost recreational use of them, as well as the cumulative impacts of the Spill. Affected natural resources include ecologically, recreationally, and commercially important species and their habitats across a wide swath of the coastal areas of Alabama, Florida, Louisiana, Mississippi, and Texas, and a huge area of open water in the Gulf of Mexico. When injuries to migratory species such as birds, whales, tuna and turtles are considered, the impacts of the Spill could be felt across the United States and around the globe. This ERP/EA serves as the Trustees' final selection of Phase I early restoration projects, taking into account the suite of potential projects proposed, the NRDA and Framework Agreement process, and public comment on the Draft Phase I ERP/EA. Per the Framework Agreement, the Trustees will move forward with agreements with BP to fund projects and commence implementation, as described in more detail throughout this document.

Floating booms were placed across inlets, within estuaries, and along sandy beaches creating a barrier to people and to important wildlife habitats. Heavy equipment and lines of workers moved large amounts of sand to form additional berms and barriers. Some response activities to the spill negatively impacted sandy beaches and marshes as thousands of workers descended on the beaches and sensitive wetland areas preparing for the oil to come ashore, searching for oil and removing product by hand and with machines. It was an environmental disaster of unprecedented proportions. It also was a devastating blow to the resource-dependent economy of the region. Book excerpt: On the night of April 20, 2010, the Deepwater Horizon rig, located 45 miles off the coast of Venice, Louisiana, exploded and caught fire, resulting in the deaths of eleven workers. The rig sank on the morning of April 22, and on April 23 crews discovered oil leaking from the well's riser and drill pipe. Oil began reaching shore in late May. During the peak of the operations, more than 47,000 men and women were involved in responding to and cleaning up the oil spill each day. This included more than 42,000 response and cleanup workers employed by BP and its contractors, 1,600 members of the National Guard, and more than 2,400 federal employees. The area of operations spanned the coastline from Louisiana to Florida, as well as offshore operations from the shoreline to the site of the release; 6,400 vessels were involved in the operations. Many workers faced potential exposure to weathered oil, oil byproducts, dispersants, cleaning products, and other chemicals used in the cleanup process. Depending on their assignments, these workers also faced potential hazards from extreme heat, slips, falls, material handling, drowning, confined spaces, struck-by, fatigue, loud noises, sharp objects, and electrical hazards, as well as bites from insects, snakes, and other species native to the Gulf Coast region. The Occupational Safety and Health Administration (OSHA) was part of the coordinated federal response to ensure that workers were protected from these hazards. The U.S. Coast Guard, the National Institute for Occupational Safety and Health (NIOSH), the National Institute of Environmental Health Sciences (NIEHS), the U.S. Environmental Protection Agency (EPA), other government agencies, and BP worked together to protect workers involved in the response. As a member of the National Response Team (NRT), the Department of Labor (DOL), through OSHA, provided guidance and safety and health expertise to the Coast Guard at the National Incident Command, the Unified Area Command (UAC), and the local incident command posts (ICPs)/unified commands. (See Appendix A for a description of the federal government's National Response System and Incident Command System/Unified Command.) OSHA's role in the Unified Command (UC) was to monitor the health and safety hazards facing workers involved in the oil spill response. OSHA continually monitored and evaluated BP's efforts to ensure that BP and its contractors were protecting workers from the hazards associated with their response and cleanup work.
