Book excerpt: Developmental Coordination Disorder (DCD) is detected in approximately 5% of children and describes a condition in which motor coordination is below the level expected given a child’s age or opportunity for learning. Children with DCD display fine and/or gross motor difficulties which persist into adulthood and cannot be better explained by a medical or neurological condition. The difficulties that individuals with DCD experience have a significant impact on activities of daily living, scholastic achievement, inter-personal relationships, and employment. Motor difficulties such as these have been poorly defined and are poorly understood by healthcare and education professionals, rendering treatments and care more difficult. European guidelines from 2012 and further revisions in 2019, have helped to clarify the diagnostic issues and there has been a significant growth in research in this field over the last four decades. A search for the topic ‘Developmental Coordination Disorder’ using Web of Science yielded 4153 publications with 69% of these published in the last 10 years alone. Despite this growth there still are pending questions in research regarding our understanding of the etiology, the co-occurrence with other developmental disorders, and the lived experience.

A lack of activity increases the risk of heart disease, colon and breast cancer, diabetes mellitus, hypertension, osteoporosis, anxiety and depression and others diseases. Emerging literature has suggested that in terms of mortality, the global population health burden of physical inactivity approaches that of cigarette smoking. The prevalence and substantial disease risk associated with physical inactivity has been described as a pandemic. The prevalence, health impact, and evidence of changeability all have resulted in calls for action to increase physical activity across the lifespan. In response to the need to find ways to make physical activity a health priority for youth, the Institute of Medicine's Committee on Physical Activity and Physical Education in the School Environment was formed. Its purpose was to review the current status of physical activity and physical education in the school environment, including before, during, and after school, and examine the influences of physical activity and physical education on the short and long term physical, cognitive and brain, and psychosocial health and development of children and adolescents. Educating the Student Body makes recommendations about approaches for strengthening and improving programs and policies for physical activity and physical education in the school environment. This report lays out a set of guiding principles to guide its work on these tasks. These included: recognizing the benefits of instilling life-long physical activity habits in children; the value of using systems thinking in improving physical activity and physical education in the school environment; the recognition of current disparities in opportunities and the need to achieve equity in physical activity and physical education; the importance of considering all types of school environments; the need to take into consideration the diversity of students as recommendations are developed. This report will be of interest to local and national policymakers, school officials, teachers, and the education community, researchers, professional organizations, and parents interested in physical activity, physical education, and health for school-aged children and adolescents.

Although there is a significant body of literature addressing developmental and cognitive issues in children with DCD, few studies have actually examined the associations between DCD, physical activity and physical fitness. Therefore, the aim of the present research work was to assess these associations in a series of four successive studies which were set: a) (study 1) to estimate DCD prevalence rates in Greek children and investigate whether these children exhibit different obesity and cardiorespiratory fitness levels than an overseas sample, b) (study 2) to provide evidence on the association between DCD and physical fitness levels, c) (study 3) examine whether a motor skills and exercise training intervention programme affects motor proficiency in a cohort of elementary school children with and without DCD, and d) (study 4) to test the hypothesis that DCD is associated with CVD risk, identify modes of physical activity that mediate such an association and to evaluate the CSAPPA scale as a potential tool for identifying Greek children for DCD. Methods: The total of 574 Greek (Age: 11.46? 1.54 years; BMI: 19.96? 3.53) children were assessed for anthropometry, physical fitness (flexibility, hand strength, leg explosive power, speed and cardiorespiratory fitness), motor competence (i.e., short form of the Bruininks-Oseretsky Test of Motor Profiency- BOTMP-sf) and subjected to two self assessments for: i) perception of adequacy for physical activity (CSAPPA scale), and ii) children?s participation in physical activity (Participation Questionnaire - PQ). Book excerpt: Cardiorespiratory fitness is an outcome of physical activity, enabling the transport of oxygen from the atmosphere to the mitochondria to perform physical work and therefore reflects the overall capacity of the cardiovascular and respiratory systems to perform the prolonged exercise. In recent decades, it has declined in the paediatric population. Cardiovascular fitness measurement has yet to be standardised in children but is a powerful marker of health in youth and is strongly associated with inflammation and inversely associated with cardiovascular risk factors, especially obesity. Notably, youth with low cardiorespiratory fitness levels have a higher risk of developing cardiovascular diseases during adulthood. Lowered cardiorespiratory fitness has been demonstrated most often in children with obesity and associated cardiovascular comorbidities, however, these can be associated with cardiorespiratory fitness independently to body mass index. The benefits of physical activity on health have been well demonstrated during growth and it should be encouraged in children with cardiovascular risk to prevent further reduction of cardiorespiratory fitness and the development of other comorbidities. Along with appropriate physical exercise and diet in childhood, breastfeeding in the first year of life is recommended.

Current public health recommendations call for at least 60 minutes a day of moderate to vigorous physical activity for children. Many children do not meet these recommendations. Objective/hypothesis . To examine the relationship between coordination and physical activity in 8-10 year-old children living in Erie County, New York. The relative reinforcing value of physical and sedentary activity will also be assessed. Previous findings . Previous research has suggested the amount of physical activity children engage in may be influenced by their degree of coordination/motor proficiency. Further, reduced coordination may contribute to childhood obesity, particularly in children where there is increasing evidence of a link between obesity and a sedentary lifestyle. Research conducted in our lab previously found that coordination as measured by the Bruininks-Oseretsky Test of Motor Proficiency (BOTMP) was a predictor of self-reported physical activity in children. Research design/material methods . Seventy-two children (39 girls and 33 boys) met the eligibility for this study and completed all components. Children's physical activity was assessed by activity monitored data (accelerometry), and their coordination was determined by the Bruininks-Oseretsky Test of Motor Proficiency (BOTMP). Additionally, a computerized behavioral choice task program was utilized to determine the relative reinforcing value of physical and sedentary activity. The Children's Self-Perceptions of Adequacy in and Predilection for Physical Activity Scale (CSAPPA) was used to measure children's self-perceptions of their adequacy in performing, and their desire to participate in, physical activities. This study was approved by the Child and Youth Institutional Review Board at the Women and Children's Hospital of Buffalo. The Child and Youth Institutional Review Board reviews research proposals received from University at Buffalo researchers involving the health and well being of minors. Results . Positive associations were found between motor proficiency and physical activity in children, while percent time in sedentary activity was negatively associated with motor proficiency. Motor proficiency was independently associated with physical activity after controlling for the variables in step one, explaining an additional 7.5% of the variance. Children who scored higher on the CSAPPA scale were more likely to have significantly greater motor proficiency compared to children who scored lower on the CSAPPA. The relationship between the relative reinforcing value for activity and physical activity was significant for girls but not boys. Potential Significance . This research has the potential benefit of helping understand the role of coordination and the relative reinforcing value of physical activity in physical activity participation. Abstract: Abstract of the thesis entitled The Pattern of Physical Activity in Children Submitted by Chu Yuet Wah For the Degree of Doctor of Philosophy at The University of Hong Kong in July 2007 Complexity in the assessment of the dimensional pattern of physical activity has limited the understanding of short duration activity in children. Although many acknowledge that children's movement is sporadic and of short duration, this contention has been based on one observational study. Accelerometers provide multi- axial movement in both the laboratory and field settings. This thesis aims to characterize children's physical activity patterns using traxial accelerometry (the Research Track 3 or RT3). Six studies were conducted to develop a technique for assessing dimensions of the tempo of physical activity in children, and to uncover the physiological importance of activity of this nature. Following extensive calibration of the accelerometer device, a novel analytical program was written which provided detailed characteristics of the tempo of physical -1 activity. Calibration of the RT3 provided thresholds for sedentary (=5.2 RT3 .s to 60.0 RT3 .s ) and vigorous activity (> 60.0 vmag vmag -1 RT3 .s ). The data processing programme utilized cluster recognition algorithms, vmag showing children experience many clusters per hour, which are of short duration and iare on average of low-to-moderate frequency. These tempo characteristics were found to differ markedly between the weekday and weekend. We found weekday physical activity pattern characteristics was related to cardiorespiratory fitness and dynamic exercise responses, whilst biomechanical efficiency was weakly related physical activity bouts. Markers of cardiovascular risk and obesity, namely high sensitive R- reactive protein (hsCRP), adiponectin and insulin resistance (IR) were investigated in relation to physical activity tempo characteristics. Dimension of short duration physical activity in children were inversely associated with hsCRP and IR (both p 0.05). In conclusion, the majority of an average day for a Hong Kong Chinese child is spent sedentary. The activity they engage in is of short duration and largely low-to- moderate intensity. Occasional spurts of high intensity are experienced. The variation of these short-duration bouts appeared to be important for fatness, cardiovascular and metabolic health.
