Book excerpt: This dissertation outlines a program of research that examined the role of affective associations in the decision-making process surrounding condom use. The decision whether or not to use a condom is impacted by both cognitive and affective factors, but research and interventions aimed at increasing condom use rarely address these affective processes directly. A more explicit integration of affective associations into decision-making theory and interventions may help to increase rates of condom use. To this end, the overarching goal of this research was to advance our theoretical understanding of the role of affect in condom use decision making. In the first of three studies, we established preliminary support for the behavioral affective associations model as a useful framework for studying condom use and sexual behavior. Affective associations with condoms had a direct effect on condom use and also mediated the relation of cognitive beliefs to behavior. These findings were replicated in Study 2, but in this study, the behavioral affective associations model did not extend to an analysis of the interplay between perceived risk and worry as predictors of condom use. Building on these cross-sectional findings, the third study involved an experimental study designed to provide stronger evidence for a causal relation between affective associations and condom use behavior. Findings from Study 3 suggest that an experimental manipulation changed affective associations with condoms, and also influenced behavior for participants who used condoms regularly at baseline. Furthermore, the condition's effect on condom selection behavior was mediated by participants' affective associations. Together, these findings replicate prior support for the role of affect in condom use decision making, and underscore the need to better integrate these factors into decision making theory and public health interventions. They also point to a number of unanswered questions that remain as directions for future research.

Book excerpt: The current project involved three studies designed to examine which characteristics of persuasive communications change condom use attitudes. Study 1 investigated the effect of message type (affective versus cognitive) and source (male versus female) on attitude change among a sample of African American women attending college. In Study 1 (N = 146), the affective message and female source produced the most positive change in condom use attitudes. Study 2 examined whether message type and source were associated with favorable condom attitudes in a community sample and whether these findings differed from a college sample. Twenty-seven women participated in group discussions and completed measures of attitudes and intentions. Similar to Study 1, the pattern of means in Study 2 showed the affective message delivered by the female source resulted in the most positive condom use attitude change. These results were not significant likely due to the small sample. Focus groups were also conducted to better understand the types of condom use messages African American women find effective. Several themes emerged: condom used primarily for pregnancy prevention; negative condom use attitudes; communicating messages through internet, TV, and radio; creating fun, catchy, and informative messages; using celebrities and peers to deliver messages; and increasing the frequency of messages to equal importance of topic. The third study assessed the effectiveness of an attitude change pilot intervention that was based on the results of Studies 1 and 2. The study used a 2 (intervention vs. control) X 3 (pretest, posttest, follow-up) mixed factorial design to examine whether women0́9s attitudes changed after a pilot session and whether the change persisted over time. African American women (N = 115) were recruited through Craigslist and community agencies. There were no significant differences across time for the intervention and the control group. Reasons for the lack of an effect in Study 3 are discussed. Taken together, these studies provide evidence that, under certain conditions an affective message delivered by a female source can change attitudes toward using condoms. The results of these studies could potentially enhance existing interventions by renewing a focus on changing attitudes as well as behavior.

Book excerpt: The two goals in this study were to utilize vignettes to determine the role of the "known partner is a safe partner" heuristic and to examine the effect of vignette perspective on college students' decisions to engage in risky sexual behavior. The hypotheses were: (1) Participants would endorse a greater likelihood of engaging in sexual intercourse with a friend than an acquaintance, (2) participants would be more likely to indicate condom use with an acquaintance than a friend, (3) participants would rate the likelihood of sexual intercourse as more likely in the 3 rd person vignette than in the 2 nd person vignette, and (4) participants would rate the likelihood of condom use as higher for vignettes written in the 2nd person than in the 3rd person. ANCOVAs were used to test hypotheses controlling for virginity. Partner familiarity did not affect likelihood of engaging in sexual intercourse or condom use. This suggests that friend and acquaintance may not be considered different sex relationship types among college students. In terms of vignette perspective, men and women, indicated less intention to engage in sexual intercourse if the vignette was written in the second person perspective (M = 2.99, SD = 1.44), than in the third person (M = 3.97, SD = 0.85). A similar significant effect was found for intent to use condoms, (2nd person, M = 4.69, SD = 0.79; 3rd person, M = 3.43, SD = 0.75). The results support the use of downward social comparison in that students chose the socially appropriate behavior when the vignette referenced themselves versus an unknown other (i.e., Stephen and Laura). One way interventionists may approach college students' use of downward social comparison biases (i.e., the better than average effect) is to use it to their advantage when presenting sexual health information. This might include positively framing intervention messages to show the average college student that by using condoms every time or being selective about his or her sex partners, he or she is engaging in "above average" safer sex behavior.

In light of this, the Centers for Disease Control and Prevention, through the National Association of County and City Health Officials, commissioned the National Academies of Sciences, Engineering, and Medicine to convene a committee to examine the prevention and control of sexually transmitted infections in the United States and provide recommendations for action. In 1997, the Institute of Medicine released a report, The Hidden Epidemic: Confronting Sexually Transmitted Diseases. Although significant scientific advances have been made since that time, many of the problems and barriers described in that report persist today; STIs remain an underfunded and comparatively neglected field of public health practice and research. The committee reviewed the current state of STIs in the United States, and the resulting report, Sexually Transmitted Infections: Advancing a Sexual Health Paradigm, provides advice on future public health programs, policy, and research.
