Bauhofer, Geneva In disaster situations, the particular concern of WHO is not so much to offer immediate relief and assistance for affected communities, but to have ready prepared plans for the provision of primary medical care as well as for resuscitation and casualty services. Disast ers are characterized by a need for rapid assistance and by the inability of affected communities to cope with the large scale mortality, morbidity, and damage to essential installations and homes. In some highly elaborate and centralized societies even small scale events may assume the proportion of a disaster, if they result in the serious break down of vital services. The Executive Board of the World Health Organization has defmed disasters or "emergencies", as situations where there are unforeseen, serious, and immediate threats to public health. Particularly severe disasters may be classified as catastrophes; such -occurrences, whether natural or man-made, disturb or overthrow the existing order. For planning purposes, it is important to distinguish between different types of catastrophes since they require special relief measures. In the past, medical assistance was primarily needed in epidemics of, for example, plague, cholera, and smallpox. Today, health authorities face emergency problems brought about by major accidents and outbreaks of chemical pollution and poisoning, which may have long-term effects. The role of health services may differ quite extenSively in different types of cata strophes, and an attempt must be made to draw up specific plans to deal with them.

In parallel, various levels of the community have been mobilizing fire prevention and fire-fighting mechanisms that protect society and the environment from the ever-increasing hazards of fire disasters. It is therefore surprising that, while aiming at the same objective, the two sectors have rarely come together; doctor has rarely worked with fire chief. Yet both disciplines have so much to learn from and contribute to each other's efforts. The Mediterranean Burns Club is a professional organization that brings together persons concerned with burn therapy and fire safety in all forms, especially in the countries of the Mediterranean basin. It is honoured to have been identified by the United Nations as a premier scientific body in its field within the programme of the International Decade for Natural Disaster Reduction. It is therefore natural that it should have initiated an international gathering of specialists engaged in burns as a surgical, clinical problem, and of counterparts dealing with fires as a societal, disaster management problem.

In this perspective, the major clinical excellence and testing laboratories for disaster response are the scenes where disasters happen. Yet, most of the research and reports in the field are produced in high-income countries, while most of the disasters happen in middle- or low-income countries. There is thus a need to bring these two environments together in order to translate theory into practice and among people who are highly exposed and involved in the multidisciplinary management of a disaster or major incident.

Such programs bring some degree of standardization and offer a thorough evaluation of participants and the program by using evidence-based scientific methods. One important part of the program is to offer a standardized handbook in which all necessary information is gathered. This is not intended as a substitute for reference books, but it is intended to make the information accessible and easy to learn so that the information can be combined with the practical parts of the education
