Book excerpt: The Future of Nursing explores how nurses' roles, responsibilities, and education should change significantly to meet the increased demand for care that will be created by health care reform and to advance improvements in America's increasingly complex health system. At more than 3 million in number, nurses make up the single largest segment of the health care work force. They also spend the greatest amount of time in delivering patient care as a profession. Nurses therefore have valuable insights and unique abilities to contribute as partners with other health care professionals in improving the quality and safety of care as envisioned in the Affordable Care Act (ACA) enacted this year. Nurses should be fully engaged with other health professionals and assume leadership roles in redesigning care in the United States. To ensure its members are well-prepared, the profession should institute residency training for nurses, increase the percentage of nurses who attain a bachelor's degree to 80 percent by 2020, and double the number who pursue doctorates. Furthermore, regulatory and institutional obstacles-including limits on nurses' scope of practice-should be removed so that the health system can reap the full benefit of nurses' training, skills, and knowledge in patient care. In this book, the Institute of Medicine makes recommendations for an action-oriented blueprint for the future of nursing.

Self-reported perceptions of associate degree and baccalaureate degree graduate nurses in central Kentucky of the effectiveness of their program of nursing education in preparing them for entry into to professional nursing practice are compared. Eleven specific skills are identified as essential in nursing practice (Li & Kenward, 2006) and are the basis of the survey. No significant difference was noted either nationally (Li & Kenward, 2006) or in central Kentucky between associate degree and baccalaureate graduate nurses. The sample consists of voluntary respondents to a mailed survey. Survey participants had been licensed as registered nurses for the first time between January and June 2009 in central Kentucky. Demographic characteristics of the central Kentucky respondents and the national respondents were also compared.

Book excerpt: Today's complex healthcare system requires nursing graduates to be prepared to deliver high quality, patient-centered care to a diverse population. Nurse educators must be highly proficient nurses as well as excellent educators who are able to facilitate student learning. There is a paucity of published literature regarding the optimal preparation for nurses entering the faculty role. This retrospective survey study examined the relationship between the educational preparation of nursing faculty and the student outcomes of program pass rates and performance on key components on the National Council Licensure Examination for Registered Nurses (NCLEX-RN) in 40 associate degree nursing (ADN) programs in Florida, both public and private/proprietary. The specific components of educational preparation that were included in this study were (a) the highest degree attained; (b) the type of doctoral degree attained (Doctor of Education, Doctor of Philosophy, Doctor of Nursing Practice, or other); and (c) completion of a minimum of nine credits of graduate level education coursework covering topics such as curriculum development, instructional design, assessment and evaluation, and adult learning theories. Other faculty characteristics analyzed were teaching experience, student-to-faculty ratio, and percentage of full-time faculty. All faculty data were grouped by program. Faculty characteristics shown to predict program NCLEX-RN pass rates were faculty completion of graduate level education coursework and years of teaching experience (averaged teaching experience by the full-time faculty group within a program), although faculty completion of education coursework was related to NCLEX-RN pass rates at a significantly higher rate than was found with years of teaching experience. Based on the findings from this study, ADN programs in Florida that employ full-time faculty members who have completed graduate-level education coursework may see positive effects on NCLEX-RN pass rates. Likewise, ADN programs in Florida that increase the average teaching experience of the full-time faculty may see positive effects on NCLEX-RN pass rates.

Book excerpt: The purpose of this study was to examine associate degree nursing faculty perceptions of factors that influence their career advisement of nursing students to matriculate in nursing baccalaureate degree completion programs. The study was guided by the Theory of Planned Behavior (Ajzen, 1988) as expanded by Godin and Kok (1996). Associate degree nursing faculty perceptions of factors that influence their advisement were viewed through the lens of attitude, subjective norm, perceived behavioral control, personal norm and moral norm. An exploratory mixed method case study was conducted. A case state that had mandated articulation agreements between nursing educational levels was purposely chosen. An online survey, developed by the researcher, was sent to all full time nursing faculty from six of the seven associate degree nursing programs in the case state. Thirty-eight (67%) faculty provided quantitative data by completing the survey. Eight of these respondents provided further qualitative data by participating in telephone interviews. Five conclusions were drawn and recommendations were made for practice and continued research. The study concluded that advisement regarding educational continuation by associate degree nursing faculty is influenced by their belief that, given adequate support, and a period of recuperation, graduates are well prepared to educationally advance. In addition, the degree to which linkages had been established with baccalaureate completion programs, influenced advisement strategies by associate degree nursing faculty. Furthermore, faculty described a moral obligation to advise students based on a perceived relationship that was uniquely collegial. Recommendations for practice include development of intentional advisement strategies within associate degree programs that nurture professional excitement and ensure individualized plans for continuation. Stronger collaborative relationships with upper division programs are needed. Innovative advisement initiatives within the clinical arena merit further exploration. Research that focuses on student and baccalaureate faculty perceptions is needed to add to the discourse. Until a reliable method of tracking educational advancement in the case state can be established, evaluation of advisement effectiveness will remain anecdotal.

Community colleges in the United States make quality and affordable higher education accessible to individuals who may not otherwise pursue it. While these institutions attract diverse and talented learners, community colleges make rewarding and life-altering careers a reality for many. For associate degree nurses, this educational opportunity serves as a strong foundation upon which to build a meaningful career. Increased emphasis on the educational advancement of associate degree nurses began with the 2010 Institute of Medicine report on the future of nursing. In order to reach the national goal recommended in the report that 80% of registered nurses be prepared at the minimum of the baccalaureate by 2020, a paradigm shift representing the acceptance of multiple pathways to the baccalaureate needs to occur. Community college faculty and leadership have consistently encouraged the educational advancement of graduates; however, pathways for associate degree nurses to advance educationally have not always been clear and achievable. This dissertation begins with a description of the experiences of associate degree nurses who have attained a baccalaureate degree, continues with an initial analysis of a database designed to track the rate and frequency of the educational advancement of associate degree nurses, and concludes with an analysis of related demographic and academic variables. Adoption of strategies presented here can assist associate degree programs to become more aware of the educational advancement of graduates. In addition, the collection and analysis of these data by associate degree programs can demonstrate their commitment to the goal of the profession to reach 80% baccalaureate preparation by 2020, while supporting the sustainability of the associate degree as an entry level to practice as a registered nurse. Increased awareness of the patterns of educational advancement by associate degree nurses can assist related programmatic planning and policy development.
