Book excerpt: Gastric banding as a weight loss surgery has increased in popularity in the United States since its approval by the Food and Drug Administration in 2001. Successful weight loss after weight loss surgery is most frequently defined as greater than 50 percent of excess weight loss (EWL). Systematic reviews show that the band is widely effective in achieving successful weight loss in most patients, however individual studies show more inconsistent outcomes. Although previous research supports gastric bypass surgery as having a positive effect on quality of life after gastric bypass, there is very little data to support this relationship in gastric banding patients. Weight loss outcomes after gastric banding surgery at Virginia Commonwealth Health System were less than 50 per cent EWL at one and two years. A retrospective, descriptive study sought to answer the following questions: 1) What is the relationship between physical activity, eating behaviors and hunger control on weight loss and quality of life after gastric banding surgery and 2) Does successful weight loss at one year predict successful weight loss at two years. The International Physical Activity Questionnaire Sort Form (IPAQ) was used to assess participation in physical activity. The Medical Outcomes Study Short Form (SF-36) was used to determine quality of life. Weight loss outcomes were 34.6 per cent at one year and 39.7 per cent at two years, which was less than the established success rate of greater than 50 per cent EWL, However, most patients lost some weight and demonstrated a significant improvement in most domains of health related quality of life. Participation in physical activity increased fro preoperatively to year one but decreased in year two. Book excerpt: Bariatric surgery is rapidly gaining acceptance among the morbidly obese population, yet studies assessing variables associated with post-surgical outcomes have yielded inconsistent findings. The purpose of this investigation was to assess the association between the relative reinforcing value of food (RRV-F), delay discounting for food (DD-F), and other eating-related and non eating-related variables to post-bariatric surgery percent total weight loss (%TWL) and substance-related outcomes. Participants were a convenience sample of 147 adults with a history of Roux-en-Y gastric bypass surgery. Using online survey methodology, variables that may contribute to post-surgical %TWL and substance use outcomes were assessed: the RRV-F, DD-F, quality of life, and other eating-related variables (food addiction, binge eating, dietary restraint, disinhibition, craving, grazing, nocturnal eating, and emotional eating). The RRV-F was significantly associated with post-bariatric surgery %TWL, while the DD-F was not found to be a valid measure of food-related impulsivity among post-bariatric surgery patients. Weight loss and substance-related outcomes were inversely related, as those who achieved less weight loss were more likely to endorse symptoms of a probable post-surgical substance use disorder (SUD). Dietary restraint and disinhibition were found to influence %TWL, while the eating-related variables found to contribute most significantly to a probable post-surgical SUD were nocturnal eating, food addiction, subjective feelings of hunger, and environmental sensitivity to food cues. Family history of substance abuse was also strongly associated with endorsing symptoms of a probable post-surgical SUD. Regarding %TWL outcomes, results suggest strongly advising post-bariatric surgery patients to avoid substance use. With respect to endorsing symptoms of a probable SUD, results have practical implications in that family history of SUD can be easily assessed, and at-risk patients can be advised accordingly. In addition, those who develop post-bariatric surgery SUD appear to have stronger cognitive and behavioral responses to food, providing some support for the concept of "addiction transfer."

Additionally, the differential impact of each type of bariatric surgery on improving eating behaviors and quality of life has received comparatively little attention.This study aims to investigate the impact of RYGB and SG on dietary intake, quality of life, appetite/satiety, perception of meal size energy homeostasis and body composition. These types of surgeries were chosen because of their district involvement in parts of the gastrointestinal (GI) tract (gastric vs intestinal). Thus, understanding the physiologic mechanism of different parts of the GI tract in humans.The study will consist of three different groups, including prospective group (pre-surgery to 12 months post-surgery), cross-sectional long-term group (3-5 years post-surgery), and a normal-weight control group. Patients will be asked to write a 3-day food diary, complete Eating-Disorder-Examination-Questionnaire, 36-Item-Short-Form-Health-Survey, and Impact-of-Weight-on-Quality-of-Life-Questionnaire and have their body measures. Blood samples will be collected to measure Ghrelin, PYY3-36, and GLP-1. Resting energy expenditure will be estimated by indirect calorimetry. Appetite/satiety will be measured by visual analogue scales and perception of meal size will be assessed by an invitation to an open buffet.Knowledge gained from this study will offer further understanding of the mechanisms that lead to post-surgical long-term weight maintenance. This could have impacts on the improvement of nutritional health in the clinical management of surgical patients. Studies on the impact of bariatric surgeries on dietary intake and metabolism are important in improving the understanding of the GI mechanism.

Book excerpt: While obesity rates have plateaued within the last decade, two-thirds of the United States population is currently classified as overweight (defined as a body mass index [BMI] of 25-29.9 kg/m2) or obese (a BMI greater than 30 kg/m2). Bariatric surgical interventions are not only more effective than behavioral treatments in the short term but are the only form of obesity intervention with evidence of consistent long-term effectiveness. However, even among bariatric surgery patients, weight loss often stabilizes and it is estimated that more than 20% of bariatric surgery patients will regain a significant amount of weight that was initially lost long-term. Little research to date has been conducted on physical activity in post bariatric surgery patients. More specifically, there have been no studies to date examining the effects of Meditative Movement (MM) programs on body composition in bariatric patients. A study using an 8-week Tai Chi Easy program was conducted in female gastric bypass patients to explore feasibility of MM in the bariatric population as well as pre- and post-intervention changes in weight, mindfulness, eating behaviors, body awareness, physical activity patterns, dietary quality and mood. Data analysis revealed that there were no significant changes in weight or physical activity patterns; however, significant changes were observed in anxiety, overall body awareness and cognitive restraint in eating. Additionally, a significant decrease in processed meat consumption and a weak trend towards increased consumption of fruits may suggest an overall improvement in dietary quality.
