Book excerpt: The purpose of this pilot study was to assess the effects of music therapy on comfort in acute mechanically ventilated patients in the Intensive Care Unit. Mechanical ventilation leads to decreased comfort. Pain and anxiety may increase during this treatment modality, and the literature suggests this may directly affect patient comfort levels. Music therapy as a nursing intervention within the context of comfort, pain, and anxiety of mechanically ventilated patients was investigated. This quasi-experiemental study used a pre-test and post-test design with subjects serving as their own control. The convenience sample consisted of 2 men and 3 women who were mechanically ventilated and fit the selection criteria. Dependent variables measured included comfort, pain, and anxiety. Physiologic dependent measures included heart rate, respiratory rate, and systolic and diastolic blood pressure collected at timed intervals. Comfort was measured using a modified version of the Hospice Comfort Questionnaire. Pain was measured using the Numerical Graphic Rating Pain Scale. Anxiety was measured using the Faces Anxiety Scale. Important preliminary quantitative results are provided by this pilot study. The mean, standard deviation, significance, and paired t-tests were compared for each tool to determine changes in scoring before and after the intervention and control. This was also completed for physiological data including systolic and diastolic blood pressure, respiratory rate, and heart rate. Paired t-tests showed no significant mean differences between two points of measurement on systolic, diastolic, heart rate, and respiratory rate in both the intervention and control groups. Comfort, anxiety, and pain scores before and after the intervention and control also did not demonstrate significance. The small sample size makes generalization of these findings impossible to the entire population of acute mechanically ventilated patients in Billings, Montana.

Book excerpt: This randomized controlled trial examined the effects of Music Therapy (MT), Music Medicine (MM), or Attention Control (AC) on physiological and psychological parameters of stress for adult and older adult patients receiving care in the Intensive Care Unit of a community general hospital. Previous studies have indicated effectiveness of music therapy or music medicine for these medical patients, but few data are available for music therapy interventions. This study was an attempt to add to available information about the effects of music therapy compared to the effects of music medicine or attention control for this patient population. Participants (twenty-eight adults, ranging in age from 37-83 years; not mechanically ventilated at the time of session) were randomly assigned to music therapy, music medicine or the attention control group. Repeated measures of heart rate, blood pressure, respiratory rate, oxygen saturation, and anxiety and pain levels were collected before the session, immediately after the session and at 60 minutes post-session. Anxiety was measured using the Faces Anxiety Scale, and pain was self-reported via a Visual Analog Scale. Post-session length of stay was collected from the participants' medical records. Overall, there were no significant interactions among study groups and outcome measures. There was a statistically significant difference between length of stay for music therapy participants and attention control. Over time from pre-session to post-session, statistically significant decreases in anxiety scores were measured for both music medicine and music therapy groups. Pain scores decreased for both music medicine and music therapy groups, however not significantly. Some medically beneficial effects of music therapy or music medicine were evident in the data.

Book excerpt: Patients in the intensive care unit (ICU) are exposed to numerous stressors including environmental disturbance and extensive medical treatments. The critical state of patients’ medical conditions requires close monitoring for possible rapid changes. One of the standard procedures to maintain adequate oxygen level is mechanical ventilation. This invasive treatment tremendously augments the level of state anxiety among patients. Music therapy has a long history of use in hospital settings as a complementary treatment to improve or maintain patients’ well-being. Studies have shown observable benefits of music therapy in the reduction of state anxiety in patients undergoing medical treatments. However, the administration of musical experience for mechanically ventilated patients in ICU are generally done by non-music therapy staff using pre-recorded music. Publications on music therapy intervention models in the ICU remains scarce. Given the broad range of music therapy methods that can be used for this population, the purpose of this thesis is to design a single session music therapy intervention model for mechanically ventilated patients in the ICU to reduce state anxiety. The structure of this intervention model was based on the intervention research methodology outlined by Fraser and Galinsky [Research on Social Work Practice, 2010]. A systematic literature survey served as the main source of data collection and was then analysed with qualitative content analysis method through a data coding process. This research provides a framework for future music therapy clinical practices and identifies areas for further research with this population.

Book excerpt: The purpose of this study was to determine the effects of preferred music, nonpreferred music, and silence on measures of state anxiety, relaxation, and muscle tension. Nine subjects were selected from a pool of 90 college students after passing criteria for trait anxiety and musical experience. Each of the nine subjects was tested individually for a total of three testing sessions, using one condition (preferred music, nonpreferred music, or silence) per session. Pretests-posttests of state anxiety and relaxation were administered during each condition, and muscle tension was measured using an electromyogram. Results were calculated with a repeated measures analysis of variance and Pearson product-moment correlations. A main effect was discovered between subjects in the measure of state anxiety, as well as a difference between conditions on the measure of relaxation. It was found also that state anxiety and relaxation were correlated significantly under silence. State anxiety and EMG muscle tension were related inversely under preferred music, while other EMG correlations were associated with low correlation coefficients. Lastly, a trend appeared for preferred music to induce lesser anxiety and greater relaxation than nonpreferred music.

Book excerpt: The purpose of this study was to explore the effectiveness of music therapy relaxation techniques on stress and anxiety levels of music and music therapy students and music and music therapy professionals. Subjects (N=60) were divided into an experimental group and a control group. Half of the subjects participated in a music-assisted relaxation (MAR) session, and the other half of the subjects participated in a relaxation session with no music. Each session consisted of approximately 30 minutes of a relaxation session, with 15 minutes before the session and after the session to fill out the pre- and post-test questionnaires. The relaxation script used, entitled Total Relaxation, comes from 30 Scripts for Relaxation, Imagery, & Inner Healing edited by Julie T. Lusk. The pre- and post-test questionnaires used were the State-Trait Anxiety Inventory (STAI) (Spielberger, 1983) and the Derogatis Stress Profile (DSP) (Derogatis, 1997). An additional post-test questionnaire determining the level of enjoyment and relaxation using a Likert Scale of 10 degrees was also used. All subjects gave written consent prior to participation in this study. Data results were graphically and statistically analyzed after all subjects participated in one 30-minute relaxation session. Results indicated an overall significant decrease of stress levels in all groups but no significant difference between music and non-music groups.
