Book excerpt: Millions of Americans are infected each year with Human Papillomavirus (HPV), which causes genital warts and cervical cancer in women. Vaccinations are available for the prevention of HPV infection. However, studies have shown that minorities and individuals with low socioeconomic status or no insurance are less likely to be vaccinated for HPV. No studies assessing the relationship between parental perceived racial discrimination and HPV vaccination initiation in children have been published. This cross-sectional study used 2010 Behavioral Risk Factor Surveillance System (BRFSS) data from Kentucky to assess this possible association. Analysis focused on respondents' self-reports of experiencing racial discrimination in a healthcare setting, and of their child having received the HPV vaccination. Logistic regression was used to obtain odds ratios (ORs) and 95% confidence intervals (CIs). Respondents reporting being treated better than other races had increased odds of vaccination initiation in children (OR=2.95; 95% CI: 0.60-14.49), and those reporting worse treatment had decreased odds of vaccination initiation (OR=0.95; 95% CI: 0.29-3.13). When adjusted for child's age, income, and insurance, the odds of vaccination initiation increased for those reporting better treatment (OR=4.71; 95% CI: 0.77-28.69), and decreased for those reporting worse treatment (OR=0.59; 95% CI: 0.18-1.92). Future results confirming the association may help to inform health policy to reduce the occurrence of perceived discrimination in the healthcare setting.

Book excerpt: Human Papillomavirus (HPV) is a common sexually transmitted infection that may lead to medical conditions, such as genital warts and cancers of the reproductive and oropharyngeal tracts in both men and women. The HPV vaccine offers protection against this infection, and has been available since 2006; it is recommended that adolescents receive the vaccine to provide protection before their sexual debut. Using the health belief model (HBM) and testing through hierarchical binary logistic regression, this quantitative nonexperimental correlational project was an investigation into the role that proactive provider education and recommendation has on the acceptance of the HPV vaccine among parents of adolescents when compared to provision of vaccine information statements (VIS). This project was conducted within four school-based health centers (SBHCs) in Las Vegas, NV. The participants were from families with a lower than average socioeconomic status. The sample consisted of 67 participants, 42 of whom received standard vaccine education and 25 of whom received targeted vaccine education. Findings indicated that when providers had direct conversations with participants about the vaccine, parents were 8.54 times more likely to accept vaccination for their child than when they were provided the VIS. This information about best practices may help protect children prior to exposure to HPV. In turn, this change may reduce the incidence of HPV-related infections and cancers, thus protecting the health of future generations.

Three Shots at Prevention explores the contentious disputes surrounding the controversial vaccine intended to protect against HPV, the most common sexually transmitted infection. STIs are often asymptomatic (especially in women) and are therefore often undiagnosed and unreported. Untreated STIs can have severe health consequences, including chronic pelvic pain, infertility, miscarriage or newborn death, and increased risk of HIV infection, genital and oral cancers, neurological and rheumatological effects. In light of this, the Centers for Disease Control and Prevention, through the National Association of County and City Health Officials, commissioned the National Academies of Sciences, Engineering, and Medicine to convene a committee to examine the prevention and control of sexually transmitted infections in the United States and provide recommendations for action. In 1997, the Institute of Medicine released a report, The Hidden Epidemic: Confronting Sexually Transmitted Diseases. Although significant scientific advances have been made since that time, many of the problems and barriers described in that report persist today; STIs remain an underfunded and comparatively neglected field of public health practice and research. The committee reviewed the current state of STIs in the United States, and the resulting report, Sexually Transmitted Infections: Advancing a Sexual Health Paradigm, provides advice on future public health programs, policy, and research.
