This percentage is alarming as excess body weight is not only a major risk factor for chronic diseases but it is also linked to maternal-child outcomes including breastfeeding initiation. This paper-based thesis is comprised of three sections. The first section provides a review of previous research examining the association between mothers’ pre-pregnancy weight and breastfeeding initiation. The second section is a manuscript for publication describing an analysis of administrative data for 1,079 mother and newborn dyads. Regression analysis was used to predict breastfeeding initiation based on women’s pre-pregnancy weight after adjusting for 10 variables reflecting attributes of the mothers, birth processes, and newborns. A statistically significant association was detected indicating obese mothers are less likely to initiate breastfeeding compared to normal weight mothers. The final section is a paper outlining the benefits and challenges of conducting secondary analysis of administrative data for research purposes."--Page ii.

Book excerpt: Influence of Pregnancy Weight on Maternal and Child Health: Workshop Report summarizes a one and a half day workshop convened in May 2006 that reviewed U.S. trends in maternal weight (prior to, during, and after pregnancy) among different populations of women; examined the emerging research findings related to the complex relationship of the biological, behavioral, psychological, and social interactions that affect maternal and pregnancy weight on maternal and child health outcomes; and discussed interventions that use this complex relationship to promote appropriate weight during pregnancy and postpartum. Given the unprecedented environment in the United States in which two-thirds of the adult population meets the criteria for being overweight or obese, the implications for women in the reproductive age period are unique in the history of the country. The concerns for maternal and infant health are real. The questions and answers tackled by committee members and workshop participants were not easy. Nevertheless, having an opportunity to explore what is known, examine the gaps in knowledge, and explore what to do now and in the future build a pathway for further inquiry and action. This report summarizes the workshop proceedings and highlights key themes that deserve further attention. The participants in this workshop describe what is known about recent trends in maternal weight gain and the impact of maternal weight during pregnancy on the health of mothers and their children. The workshop provided a valuable opportunity to assess trends that have occurred since the publication of an earlier study by the Institute of Medicine (IOM), which included guidelines for recommended weight gain during pregnancy.

Book excerpt: Introduction; Breastfeeding is well-established as optimal for both mother and baby, with a number of benefits including a quicker return to pre-pregnancy weight for the mother and the reduced risk of non-communicable diseases, including obesity, for the infant. The current global recommendation is to breastfeed exclusively for 6 months postpartum, yet in Ireland initiation and duration of breastfeeding is extremely low. Low education level is considered a barrier to breastfeeding, however, there is currently a paucity of research on the influence of maternal well-being on breastfeeding habits in Ireland. Aim: To explore the relationship between education levels and well-being in early pregnancy on breastfeeding habits in Irish women.Methods: This was a secondary analysis of the randomised control trial, the ROLO study in the National Maternity Hospital, Dublin, Ireland. 610 healthy, pregnant women on their 2nd pregnancy were included. Well-being was measured using the WHO-5 item well-being index at the end of the 1st trimester. Education level and breastfeeding initiation information were both obtained from hospital records. Women were stratified into two groups of education level; those that had achieved 3rd level education (n=341), and those with less than 3rd level (n=269). Within these two groups, the relationship between well-being scores and breastfeeding habits was explored using Pearson correlations, chi-square tests, and multiple regression modelling for confounders.Results: In Women with high education, those with high well-being (n=269) were significantly more likely to initiate breastfeeding than those low well-being (n=72; 69.5% vs 56.9%, p

Book excerpt: Weight and weight change during pregnancy influence fetal growth and development and may have long-lasting effects on offspring health. Additionally, nearly half of women now begin pregnancy overweight or obese and a wide range of weight is gained and lost during and after pregnancy. In response to these concerns, the Institute of Medicine (IOM) revised its gestational weight gain recommendations in 2009 to include a specific, relatively narrow range of recommended gain for women with prepregnancy obesity. Unfortunately, limited scientific evidence and quantitative methods guided these recommendations. The IOM made several research recommendations, including that studies on the impact of gestational weight gain on a range of child outcomes be conducted in large, diverse populations, and that such studies address the extent to which optimal weight gain differs by maternal prepregnancy body mass index (BMI) and other maternal factors, including maternal race/ethnicity. This dissertation aims to address gaps in our understanding of how pregnancy weight and weight change are associated with child health. The first paper demonstrates two recently developed, quantitative approaches to studying gestational weight gain and determining optimal ranges of weight gain for minimal risk of preterm birth. It also informs whether weight gain recommendations should be tailored to maternal race/ethnicity, which has previously been suggested by the IOM. The second paper identifies longitudinal trajectories of maternal weight from prepregnancy through the postpartum period and assesses the relationship between maternal weight trajectories and offspring obesity in childhood. The third paper determines if maternal history of physical abuse in childhood is related to the risk of offspring overweight in childhood, and whether prepregnancy BMI and gestational weight gain play mediating roles in such an association. These dissertation papers together provide valuable information to help determine ranges of weight gain during pregnancy that minimize risk of adverse infant and child health outcomes. They also intend to stimulate further research to establish a scientific evidence base for creating effective interventions and clinical gestational weight gain guidelines. Promoting healthy weight and weight gain in pregnancy presents a potentially feasible and effective opportunity to improve infant and child health.

Prominent reasons for not initiating breastfeeding include lactation problems and perceived insufficient milk supply. Animal studies have shown that a high fat diet is associated with negative changes in the lactation process, while omega-3 supplementation appears to have positive effects. Hypothesis: 1. High maternal fat intake during pregnancy is associated with breastfeeding non-initiation. 2. Long-chain omega-3 fatty acid intake (EPA+DHA) is associated with breastfeeding initiation. 3. Maternal EPA+DHA intake modifies the relationship between total fat intake during pregnancy and breastfeeding initiation. Materials and Methods: The data for this study were taken from the Prenatal Health Project, a prospective cohort study in London, Ontario. Women 10-22 week's gestation were recruited from ultrasound clinics and followed-up at the perinatal, infant (yr) and toddler (2-5 yrs) stage. Multivariable logistic regression analyses were conducted with blockwise data entry based on a directed acyclic graph (DAG) created to conceptualize the hypothesized relationships and potential confounders. Results: Contrary to our hypothesis, higher maternal fat (>35% fat), compared to ≤35% fat, was associated with a higher odds of breastfeeding initiation (OR=2.014 (95% CI: 1.002, 4.045)). Mothers who met the dietary EPA+DHA recommendations may have been more likely to breastfeed, but this did not achieve statistical significance. There was no significant interaction between percent fat intake and EPA+DHA intake. Discussion and Summary: Mothers whose diets were composed of a higher fat percentage were more likely to initiate breastfeeding compared to mothers with a lower percentage.
