Book excerpt: Hospitals are challenged to understand factors related to staff nurse retention in the context of the nursing shortage slowdown (Huselid, 1995; Mancino, 2008; Mancino, 2009). Leadership is a major factor in organizational success (Gandossy & Guarnieri, 2008; Gettler, 2003; Wooten & Crane, 2003) and nurse managers are the direct link between the executive nurse leaders and the registered nurses (RN) in the staff position (Taunton, Boyle, Woods, Hansen, & Bott, 1997). Studies show that nurse manager leader behaviors influence staff nurse job satisfaction (Blegen, 1993; Irvine & Evans, 1995; Lashinger & Finegan, 2005). A primary role of the nurse leader is to plan strategies for recruiting nursing personnel and to prevent rapid and frequent turnover of nursing staff (American Nurses Association [ANA] Nursing Administration Scope and Standards of Practice, 2009). Nurse managers are in a position to influence staff nurses' job satisfaction and retention through their leader behaviors. McGuire and Kennerly (2006) and Brown and Reilly (2009) conducted studies that show managers perceive their leadership behaviors different than those who reported to them. Bass and Avolio (2004) pointed out that leadership is in the 'eye of the beholder' and the leadership behavior ratings represented the people's perceptions of leaders (p. 9). Understanding the staff nurse perception of their nurse manager leadership behaviors that are related to staff nurse job satisfaction will contribute to improving staff nurse retention strategies. The Magnet Recognition program is one approach to improving staff nurse job satisfaction and retention. The Magnet Recognition program (Magnet) considers that the nurse leaders are the key to nursing excellence (American Nurses Credentialing Center [ANCC], n.d.). The sample population for this study was staff nurses in an acute care hospital applying for Magnet Recognition status. The purpose of this correlational research study was to describe the relationship between the staff nurse perception of their nurse manager leadership behaviors associated with both the facets of staff nurse job satisfaction (job satisfaction with: work on present job, pay, promotion, supervision, and co-workers) and staff nurse overall job satisfaction and the relationship between the facets of staff nurse job satisfaction and staff nurse overall job satisfaction in an acute care hospital applying for Magnet Recognition status (Magnet status). The intent to depart is conceptually and empirically shown to be a direct antecedent to actual turnover. Overall job satisfaction and turnover are inversely related. Brodke et al. (2009) reported overall job satisfaction was the best predictor of intent to quit. Many studies examined facets of staff nurse job satisfaction but the researcher identified that few if any studies examined nurse manager leadership behaviors related to facets of staff nurse job satisfaction and staff nurse overall job satisfaction in a hospital in the process of applying for Magnet status.

Book excerpt: The incidence and significance of harm that occurs to patients while in the hospital setting continues to be alarming despite decades of efforts to produce safer healthcare systems. Leaders have a role in ensuring staff provide safe care to patients. The literature supports that certain leadership styles promote interactions with healthcare staff that produce work climates conducive to positive patient outcomes. It is not clear what types of activities conducted by nurse leaders have the most impact on nursing unit patient safety. The purpose of this study was to explore the relationships between nurse leader characteristics (nurse leader behavior, educational level, and experience level) and patient safety (perceived patient safety culture, Patient safety grade, Number of patient safety events reported, and patient safety event rates) in the acute care hospital. The objective of this study was to more clearly describe the types of observable, actionable, leadership behaviors that are perceived as most beneficial to patient safety. Several existing data sources were combined to explore the relationships between nurse leader characteristics, leader patient safety behaviors, patient safety culture and patient safety events across a large health system in Eastern North Carolina. Nurse leader patient safety behavior was significantly associated with staff perceptions of patient safety culture. Nurse leader patient safety rounding was a safety behavior found to be associated with higher rating of patient safety culture including an improved culture of non-punitive response to error. This study has implications for nurse leaders, educators, and researchers. The study adds valuable information about safety behaviors that are associated with nurse leadership practice. Specifically, this study will move the current literature forward by exploring observable, repeatable, and replicable behaviors of nurse leaders in acute care nursing units with lower levels of patient safety events.

Design: Integrative literature review Methods: CINAHL and MEDLINE databases were searched 2004-2014 using "leadership behaviors of nurse managers", and "nurse manager performance outcomes" terms for research in English, peer-reviewed journals meeting this review's purpose. Findings: Ten studies support the use of transformational leadership behaviors by nurse managers to positively influence their performance outcomes. Conclusions: Strengthening nurse manager leadership skills will be a positive step in addressing the challenges of an emerging shortage of registered nurses. Relevance to Nursing Education: Findings are relevant in determining the needed content of nursing education to support nurse managers in becoming effective, transformational leaders.

Book excerpt: Work environments for health care providers in acute care hospitals have become increasingly demanding due to the impact of economic constraints, the rapid advancement of treatment modalities, and value systems changes among clients, as well as among heath care providers. In Japan, health care industries also face severe economic constraints. Because Japan has socialized medicine, the government controls reimbursements. Due to the dramatic growth in health care expenditures, the Japanese government has imposed regulations that reward shorter lengths of hospital stays with higher reimbursement. As a result, only patients whose conditions are critical and require complicated nursing care are now hospitalized. Consequently, the acuity levels of patients have increased every year. Under such conditions, administrators are charged with keeping the organization financially solvent so that they can remain in business, while continuing to improve the quality of their services. Although systems research in health care settings has received considerable attention in North American countries, there has been little research in this area in Japan, where systematic leadership training for nurse managers is also still in a developmental stage. Research on organizational effectiveness has shown positive correlations between managers' leadership styles and employees' psychological well-being or self-efficacy. The purposes of this study were: 1) to test Laschinger's Work Empowerment Theory with incorporation of leadership behaviors in acute care hospitals in Japan, and 2) to investigate how leadership behaviors might mitigate burnout and empower staff nurses working in acute care hospitals in Japan. It was expected that employees who perceived a high level of Structural Empowerment would demonstrate high Psychological Empowerment and low burnout level. If employees perceived high leadership behaviors in their immediate supervisors, their Psychological Empowerment was expected to be higher and their burnout level was expected to be lower. The following four instruments were used: 1) Conditions of Work Effectiveness Questionnaire-II (CWEQ-II); 2) Psychological Empowerment Scale; 3) Nurse Manager's Action Scale; and 4) Maslach Burnout Inventory. The questionnaire was distributed to 1,377 staff nurses working on 50 inpatient care units in two acute care hospitals in Japan. Participant response rates for all units were equal or greater than 50%. Psychometric evaluation of the instruments was performed. Construct validity and reliability were established for all instruments at the individual level. At the group level, construct validity and reliability for two instruments (Structural Empowerment and Nurse Manger's Action Scale) were confirmed, but not for two others (Psychological Empowerment and Maslach Burnout Inventory). Results suggested that the Work Empowerment Theory also fits Japanese nurses, but there was little effect of leadership behaviors on staff nurses' perceived empowerment. A group level analysis indicated that leadership behaviors did not influence Psychological Empowerment or Burnout, but influenced Structural Empowerment.

A synthesis of nursing research suggests that nurse managers may role model the leader behaviors of their nurse executives. Using a descriptive comparative design, 22 acute care nurse executive/nurse manager sets from three Pacific northwest states completed a demographic questionnaire and the Leader Effectiveness Adaptability Description - Self (LEAD-S) (Hersey & Blanchard, 1988). Data analysis showed that the dominant leadership style of the nurse executives was Participating. The dominant leadership style of the nurse managers was Selling. Both of these styles emphasize high relationship behaviors. These were my first efforts as an author, which were published in 2013: Promoting a Culture of Safety: Preventing Central Line Infections in Weill Cornell Medical Center, which used a performance improvement process to lower the rate at which critically ill patients in cardiac care developed central line infections, and Factors Influencing Critical Nurses' Perception of their Overall Job Satisfaction: An Empirical Study, which used a correctional approach and was statistically analyzed to determine the perception of critical-care nurses of their manager's leadership style and its effect on their job satisfaction. Having been on the receiving end of leadership behaviors gave me a firsthand opportunity to observe these diverse nurse leaders at both extremes of the spectrumfrom laissez-faire leadership style to dictatorial leadership style and everything in between. Each encounter has enriched my life immeasurably. My personal and professional experiences, as well as the knowledge I gained from completing my dissertation, all compelled me to write this bookto share with novice managers and those aspiring for a leadership role an awareness and provide them with some valuable information needed as they forge their career paths into a leadership role, knowing that one of the keys to effective leadership is the ability to stay intellectually curious and committed to learning with the understanding that new knowledge can come from variety of sources and to make it a point of duty to be always on a lookout for new knowledge.

Although there has been extensive nursing research on job satisfaction and retention, the first-line manager's influence on this has not been adequately studied. The purpose of this study was to determine if nurse managers' leadership behaviors and the task structure of the subordinates' jobs were related to subordinates' job satisfaction and intent to stay in home health care agencies. House's (1971) path-goal theory of leadership provided the conceptual framework for this investigation. This research study revealed there was a difference between registered nurses' (RNs) and home health aides' (HHAs) perceptions of their nurse managers' leader behaviors and perceptions of their task structure. There was a positive relationship between supportive leader behavior and job satisfaction for the RNs. There was a positive relationship between job satisfaction and instrumental leader behavior for the HHAs. Task structure was positively related to job satisfaction and instrumental leader behavior for the HHAs"--Document.
