Book excerpt: In a profession where the nursing shortage is well known and inevitable despite of vacancy reprieves, understanding determinants of job satisfaction may now be an essential component of retention strategies. Many factors are causing the health care industry to see the potential significant implications of the nursing shortage. Collaboratively creating an environment of empowerment, autonomy, and lower stress, nurses are content and satisfied. In turn, satisfied nurses can continue to deliver safe and quality patient care. The purpose of this study was to understand the relationships among the determinants of job satisfaction. Nurse-manager collaboration, empowerment, autonomy, and subjected job stress were the identified independent variables and job satisfaction was the dependent variable. The following instruments were used to measure the perspective study variables: Collaborative Behavior Scale-B (CBS-B); Conditions of Work Effectiveness Questionnaire-II (CWEQ-II); a subscale from the Index of Work Satisfaction; Subjective Job Stress; and Michigan Organizational Assessment Scale. The hypotheses proposed for the study included: there will be positive relationship between nurse-manager collaboration and job satisfaction; there will be a positive relationship between empowerment and job satisfaction; there will be positive relationship between autonomy and job satisfaction; there will be negative relationships between subjective job stress and job satisfaction; there will be significant relationships among nurse-manager collaboration, empowerment, autonomy, and subjective job stress. The research study was a descriptive correlation study design. A convenience, snowball sample methodology was used. Once correlation was identified, the study variables were further analyzed using multiple regression and dominance analysis. There were significant correlations among the study variables. Empowerment had the strongest correlation to job satisfaction (r = 0.71, p [less than or equal to] 0.05). It explained 50% of the variance in job satisfaction ([lower case beta] = 0.76, p [less than] 0.001). Dominance analysis revealed empowerment is the most importance predicator of job satisfaction (F = 46.57, df = 1, 70; R2 = 0.213). The data supported all hypotheses. A post hoc analysis was conducted to determine if any of the study variables were correlated with nurses' perceptions of unit commitment to patient safety, quality of work, and nurse safety. These three variables were measured with one question on the demographic requesting the respondent to rate each of these variables on a 10-point scale. Several statistically significant correlations were identified. Perception of NMC was highly and significantly correlated to perception of unit's commitment to nurse safety. This validates the desire that safety in nursing practice is affected by how effective the collaboration is between the nurse and the nurse manager. The perception of autonomy was significantly correlated to unit's commitment to patient safety. This may imply that the more the nurse believes that the higher their autonomy (scopespecific decision making power) patient safety increases. These results indicate the relationship between nurses' perceptions of NMC, empowerment, autonomy, SJS, and JS with the nurses' perceptions of the units' commitment to patient safety, quality of work, and nurse safety. This is the first time that such findings have been reported.

Book excerpt: Purpose: This research evaluated relationships among hospital types (Magnet versus non-Magnet), nurse manger leadership attributes, and staff registered nurse autonomy. Hypothesis: Magnet hospital status would be related to positive nurse manager leadership attributes which would be related to greater clinical nurse autonomy and nurse manager leadership would mediate relationships between hospital status and clinical nurse autonomy. Conceptual Basis and Background: Structural Contingency Theory modified in the Nursing Systems Outcomes Research model was the research model for conceptualizing the structure-environment-effectiveness relationship. Critical Social Theory (CST) provided the conceptual/motivational basis for this study, a lens through which to frame the question. Clinical nurse autonomy is characteristic of hospitals noted for good patient outcomes and excellent nursing care. Research demonstrates that leadership attributes of nurse managers are related to increased nurse autonomy and positive patient outcomes. Magnet hospitals have been found to have superior nurse executive attributes, greater nurse autonomy, and high-quality nursing care and to generate better patient care outcomes than non-Magnet hospitals. Design and Methods: This study employed a pre-experimental, cross-sectional correlational design. Two groups (nursing managers and staff registered nurses) represented Magnet and non-Magnet hospitals. These two types of hospitals were matched on 12 criteria for a total of 388 hospitals. Statistical power analyses demonstrated sufficient power for detecting down to between medium effects and large effects for the 104 units included. Chief Nursing Officers, nursing managers and staff registered nurses were contacted via email addresses or phone numbers. Measurements were demographic questions, and assessments for leadership actions (Nurse Managers' Actions Scale, Mrayyan, 2004) and clinical autonomy (Autonomy Scale developed by Blegen, Goode, Johnson, Maas, Chen & Moorhead, 1993). Results: There were no mean differences between hospital types. There was a relationship between manager leadership attributes and nurse clinical autonomy and this relationship was dependent on Magnet status. Implication: This type of research will help identify leadership traits and attributes that empower nurse autonomy, which is related to better nurse recruitment, nurse job satisfaction, nurse retention, and patient outcomes, and it will evaluate the role of Magnet hospital status in these relationships. It may also enable developing alternative praxis-based approaches.

Book excerpt: Background: Research suggests that medical/surgical nurses have lower job satisfaction than nurses in other areas. Research also reports that 80% of nurses will experience bullying in their careers and that leadership style has a significant impact on the organizational work environment. Authentic leadership is a relatively new concept, which has been linked to increased job satisfaction and decreased bullying. Although job satisfaction has been widely explored, the relationship between job satisfaction, bullying, and authentic leadership in medical-surgical nurses has not been studied. Therefore, purpose of this thesis study was to use the Organizational Framework for Predicting Nurse Retention to explore the relationship between workplace bullying, job satisfaction, and authentic leadership among medical-surgical nurses. Methods: As part of a larger study, a cross-sectional survey was utilized (N=317). Invitations to participate were sent to all medical-surgical nurses in Manitoba, via the College of Registered Nurses of Manitoba. Results: The findings revealed that an alarming 43% of nurses had been reportedly bullied (occasionally/ severely) at work. However, 65% of the participants reported overall job satisfaction (i.e., satisfied/ very satisfied) and rated their managers as relatively authentic. While bivariate and multivariate regression analysis revealed significant relationships among the three main study concepts, control/autonomy emerged as a central and common influencing factor. Discussion: Based on these findings, control/autonomy is key factor in the medical-surgical nursing environment. Therefore, strategies to decrease bullying and increase job satisfaction should focus on developing authentic leadership in nurse managers and increasing perceived control and autonomy for nurses working in medical-surgical areas. Further studies with more diverse nursing populations are needed to support this novel research evidence.
