Book excerpt: Nurse education literature reflects that educators face a problem of how to provide meaningful clinical learning experiences that help undergraduate nursing students develop clinical judgment and increase self-efficacy. Many schools of nursing are incorporating high-fidelity simulation as an alternative clinical teaching strategy, yet there is little quantitative evidence to support the effectiveness of this teaching method in undergraduate nursing education, particularly regarding the development of clinical judgment. The purpose of this quasi-experimental time series design study was to explore the effectiveness of using multiple high-fidelity simulation experiences to increase perceptions of clinical judgment and self-efficacy in baccalaureate nursing students. Self-report data was collected following three different high-fidelity simulation experiences at three points in time. Data analysis revealed a statistically significant increase (p = .041) in students' perceptions of clinical judgment occurred between Time 1 and Time 3. Students' perceptions of self-efficacy also increased significantly from Time 1 to Time 3 (p = .003) and Time 2 to Time 3(p = .001). Regression analysis revealed a slight positive correlation (sig. = .003) between students' perceptions of self-efficacy and clinical judgment.

Book excerpt: Due to an increasingly complex patient population, new graduate nurses are expected to enter the workforce and provide competent safe care in order to promote positive patient outcomes. Pre-licensure nursing education holds the responsibility for providing this population with the critical thinking and clinical self-efficacy necessary for the successful transition from student nurse to practicing Registered Nurse. Unsuccessful transition into this new role has been attributed to a gap between what is learned in school and the reality of nursing practice, the so called "theory-practice gap" leading to poor self-efficacy. In addition, it has been suggested that poor self-efficacy leads to high nurse turnover, medical errors, and nurses leaving the profession entirely. With a looming nursing shortage expected to reach 500,000 Registered Nurses by 2025, ensuring the nursing profession is hiring competent nurses with high self-efficacy is crucial. The purpose of this pilot cohort study was to evaluate the perceived self-efficacy of final semester pre-licensure nursing students before and after completing the required clinical precepted experience. Using a quantitative approach, senior baccalaureate nursing students were surveyed during their final semester, prior to beginning their required clinical precepted experience and again after completing all required clinical hours. Descriptive statistics were used to summarize survey responses while paired samples t-tests were used to evaluate pre and post-test scores. While no statistical significance was found, the variability in student responses to the survey increased, with some students' reporting improved self-efficacy while others remained the same or declined. The interepretation of findings needs caution due to the small sample size. The influence of the theory-practice gap on students' perceived self-efficacy and preparedness to enter the nursing workforce needs to be evaluated further.

Book excerpt: Nursing faculty members are utilizing interactive teaching tools to improve nursing student's critical decision-making skills; one method that has been found to be potentially effective is human patient simulation (HPS). The purpose of this time series design study was to determine whether undergraduate nursing students were able to transfer knowledge and skills learned from classroom lecture and a HPS clinical to the traditional clinical setting. Over the course of a semester, students were observed prior to a respiratory lecture, following the respiratory lecture, and following a simulation clinical. The researcher collected quantitative data to assess participants during specific points of their instruction to determine if transfer of learning had taken place. Participants included were entry level nursing students. The findings indicated that students were able to transfer knowledge and skills learned from the HPS setting to the traditional clinical setting. An assessment rubric was also used to determine clinical competence in the traditional clinical setting. The results provided evidence that students' perceived clinical competence and actual clinical competence was enhanced following HPS clinical. Results of the study also demonstrated that there was a positive correlation between students' self-perception of clinical competence and their actual competence following HPS clinical.
