Book excerpt: This study has explored how quality issues in delivering family planning services (supply side), and attitudinal and behavioural issues of the potential users (demand side) merge together to influence adoption of family planning methods. The subject was explored by blending Bruce's (1990) quality of care of family planning services framework with an access framework with five determinants of decisions to use and sustain family planning services (availability, accessibility, affordability, adequacy, and acceptability of the services). Two districts, Misungwi and Ukerewe in Mwanza region, one of the regions with the lowest contraceptive prevalence rates, were used as case studies. Data were collected from providers of health services, clients of family planning services, and men and women non-users in catchment areas of the sampled health facilities. The study finds that although family planning services are generally available, the range of options available to users at the primary healthcare level tends to be limited, which limits the ability of users to switch methods. Proper management of clients, including protection of clients' privacy and ensuring that necessary tests and counselling are done before any method is prescribed, was also found to be weak. On the demand side, inadequate information on modern contraceptives (benefits, efficacy, side effects, and how to deal with them), misunderstandings on possible side effects, distrust among spouses, and problems of male inclusion stand out as important constraints to adoption. Delivery of comprehensive and appropriately packaged information to communities through properly designed social marketing campaigns, as well as deliberate efforts to ensure inclusive integrated services at primary healthcare level, can considerably help boost adoption. Improving the technical competence, in terms of infrastructure to allow for privacy in service delivery and skills to manage clients appropriately, is essential. The potential of community health workers in delivering services like injectable contraceptives should also be tapped.

Family planning use protects the health of women and their children by spacing births, preventing unwanted or high-risk pregnancies, reducing the need for abortions and helping to fight HIV/AIDS. Further, family planning use supports women's rights and opportunities for education, employment, and full participation in society, and protects the environment by stabilizing population growth. In high fertility societies, slowing rapid population growth through voluntary means greatly facilitates economic and social development. While significant progress has been made in the provision and uptake of family planning, an estimated 222 million women around the globe who want to avoid pregnancy are not using an effective, modern method of contraception. Community based family planning (CBFP) programs can successfully reach remote, resource-constrained populations with high unmet need for family planning. The most common approach to CBFP is the community based distribution of contraceptives, in which community health workers (CHWs) provide family planning education and counseling, contraceptive methods, and referrals for clinic-based services. The past decade has seen worldwide rapid growth in the delivery of health services via mobile communication devices, collectively known as mHealth. With the increased availability, adaptability and capability of mobile phones come new prospects for improving the quality of and access to health care. Increasingly, these new technologies are being developed, tested, and deployed with CHWs in developing countries. mHealth tools could enhance community based approaches to family planning, yet this potential is only beginning to be explored. Accordingly, FHI 360, the Tanzania National Institute for Medical Research, Pathfinder International, and D-Tree International collaborated on the development of a mobile phone job aid for family planning service delivery by CHWs in Tanzania. The introduction of this mobile job aid presents an exciting, innovative opportunity to increase knowledge about, access to and use of family planning information and services. This dissertation aims to provide data that can improve the design, implementation, monitoring and evaluation of mHealth projects targeted for CHWs, and can inform program and policy decisions to implement mHealth technologies into future family planning efforts, in Tanzania and beyond.

This study looks at the socioeconomic background of individual women and draws a correlation between them and the characteristics of their nearest source of family planning. The study assesses the importance of the socioeconomic background and the availability, price, and quality of services on contraceptive use and fertility. In 1969, Ghana was among the first Sub-Saharan African countries to adopt a population policy. Today, the average distance to a source of family planning is still about three miles. Population and fertility growth rates are high, and contraceptive use is low. The results suggest that raising levels of female schooling will also raise contraceptive use and lower female fertility, particularly in rural areas. However, the distances between facilities and related service centers remain a binding constraint on contraceptive use among those in the sample. The study also found no consistent effect on the demand for contraception or on fertility when measuring the quality of services.

The intrauterine device (IUD) is used by 150 million women around the world. It is the second most prevalent method of female fertility control in the global South and the third most prevalent in the global North. Over its five decades of use, the IUD has been viewed both as a means for women's reproductive autonomy and as coercive tool of state-imposed population control, as a convenient form of birth control on a par with the pill and as a threat to women's health. In this book, Chikako Takeshita investigates the development, marketing, and use of the IUD since the 1960s. She offers a biography of a multifaceted technological object through a feminist science studies lens, tracing the transformations of the scientific discourse around it over time and across different geographies. Takeshita describes how developers of the IUD adapted to different social interests in their research and how changing assumptions about race, class, and female sexuality often guided scientific inquiries. The IUD, she argues, became a “politically versatile technology,” adaptable to both feminist and nonfeminist reproductive politics because of researchers' attempts to maintain the device's suitability for women in both the developing and the developed world. Takeshita traces the evolution of scientists' concerns—from contraceptive efficacy and product safety to the politics of abortion—and describes the most recent, hormone-releasing, menstruation-suppressing iteration of the IUD.
