Book excerpt: Excerpt from Principles of Surgery When I published the first edition of this little work, in the year 1788, it was my intention to add one or two volumes more, so as to comprehend a large proportion of the subjects usually treated upon in a course of Chirurgical Lectures. The state of my health did not allow me to accomplish my pur pose at that time; and increasing engagements in business, afterwards, obliged me to defer, from one season to another, the further prosecution of my design. Various reasons, of little moment to the public, have at length determined me to conclude the work, by relinquishing altogether the labour of adding another volume to the principles of surgery. In revising the work, for the edition now presented to the surgical student, I made some alterations, and interspersed sev eral additions, especially on the subject of Scirrhus and Can cer. The chapter on the (edema appears now for the first time. Although many important additions have been made, in the course of a few years, to the Principles of Surgery, and several valuable improvements have been introduced into the operative part - yet a number of diseases still exists, for which we have no adequate remedies, and many Operations continue to disappoint the hopes of the surgeon and the patient. If the prospect of that which remains to be done, after the labours of so many ages, tends to abate hope and to discourage endeavour - yet it should be remembered, that no man can determine the measure of success which may be connected with industrious research and zealous exertion, and that whatever may be the degree of advancement of which surgery is capable, he who endeavours to perfect it, has the satisfaction, at least, of knowing, that he is fulfilling his duty, when his efforts are directed to alleviate the sorrows and diminish the sufferings of mankind. It would be absurd to confound the principle of heat, the expansion of steam, and the machinery by which the power thus generated acts, as causes of the same kind. The remote and the proximate cause in such a case have no immediate relation, but are connected together by a number of intermediate circumstances which, like the steps of an argument, unite the conclusion to the premises. It is very necessary to bear this in mind in physiological reasoning, in consequence of our almost total ignorance of these intermediate links in the chain of causation. In the instance above adduced by the author, we have no knowledge of the mode in which the poison of a rabid animal, or the poison of syphilis, produce their efl'ects on the animal body nor how mercury and other medicines should tend to eradicate the latter from the constitution. We are unable in these cases to advance a step further than to perceive the necessity of absorption of the poison, after which all is conjecture. A full perception of the whole sequence of effects would doubtless at once discover the precise nature of the remedy, as well as the proper cases in which it should be administered. It is not without mature consideration that I have determined upon publishing the present vol. Ume; but I have come to the conclusion, that as its contents are of a practical character, embodying the experience of twenty-five years, dur ing which time I have occupied the position of surgeon to Guy's Hos pita], it would be found useful, not only to the student, but also to those who have entered upon the practice of their profession. It must be borne in mind, that it has not been my intention to write a systematic work on the elements of the science of surgery. These lectures were originally delivered before my hospital pupils; and in thus presenting them to the profession in a collected form, my object has been to furnish a useful compendium of surgery, in which the student may meet with a clear account of the practice of that science, established, not only on my own experience, but likewise upon the best acknowledged authorities. That the practice may not bear in some cases the character of empiricism, I have endeavoured, so far as I have been able to lay down the general principles of science upon which the practice is based at the same time I have avoided as much as possible any discussion of the hypotheses of the time, deeming my practical book an unfit medium for such matter.
