Book excerpt: Abstract: The female athlete triad has been shown to be of severe consequence for female athletes who do not consume enough dietary calories to meet the high energy demands of intense or prolonged physical activity. The three components of the triad (low energy availability, menstrual dysfunction, and low bone mass) can lead to severe and possibly permanent health complications such as amenorrhea, anovulation, stress fractures, and even osteoporosis. A review of the literature has found that most of the research has focused on exploring the depths of one component of the triad or on the interrelationship of two components of the triad. Only four research studies to date have examined the true prevalence of all three components of the triad. Other limitations of the research include a lack of 1) longitudinal data, 2)data on recreational runners (as opposed to elite runners), and 3)large scale studies. This thesis will serve to explain in detail the components of the present study, The Prevalence of the Female Athlete Triad in Recreational Endurance Runners, as well as a prospective follow-up study, Treatment Methods for Recreational Runners Presenting with the Female Athlete Triad: A Longitudinal Study. The goal for these two studies is to increase awareness and show support for the true prevalence of the female athlete triad in a large scale study using recreational runners as well as forming a foundation for prevention and treatment methods for future research concerning the triad.

Book excerpt: After decades of work to identify and remove barriers to sports participation, female athletes are now participating in recreational and competitive sports at record numbers. However, female athletes are still underrepresented in sports research, leaving dangerous gaps in knowledge that we need to fill to encourage the development of more complete female athletes with better performance, healthier bodies, and improved mental health. Epidemiology provides a helpful framework for confronting these knowledge gaps and preventing them from undercutting the next generation of female athletes. It's urgent that female athletes are not treated as "smaller" male athletes, or worse--ignored altogether. By studying female athletes and addressing the unique physiological and sociological challenges they face, then translating the research into practical guidance and education, we have an opportunity to support generations of athletes to come. Every female athlete should have the tools to find their personal definition of "strong, " understanding how that individual and evolving strength will bring out their best in sports and in life. In this dissertation, I address pressing problems facing female athletes to evaluate new strategies for improving health and well-being by applying epidemiological principles and studies on research translation. The chapters in this dissertation intersect to cover six themes: (1) mental health; (2) bone health; (3) energy availability; (4) menstrual cycle; (5) performance; and (6) research translation (Figure 1-1). Chapter 1 introduces the scope of the field and how the themes intersect. In Chapters 2 and 3, we evaluate the effect of a nutrition education intervention on bone stress injury rates among female distance runners at two NCAA Division I institutions across a seven-year prospective study. In Chapter 4, we examine how a five-part video education series compares to online educational pamphlets for improving high school female runners' knowledge and for capturing their interest. In Chapter 5, we assess the risk of the Female Athlete Triad in ultramarathoners and complete exploratory analyses examining hormonal biomarkers and bone mineral density. And finally, in Chapter 6, we examine how early COVID-19 restrictions impacted the mental health and training of endurance athletes, which provides a broader context for how mental and physical health can be impacted by external factors, and how we can support athletes who are navigating prolonged breaks from competition. Recognizing and uplifting female athletes requires understanding their unique mental and physical health contexts, addressing the specific challenges they face, and empowering them to find their strong, in sports and in life.

Low energy availability with or without disordered eating, amenorrhea, and osteoporosis are the three components that make up the Female Athlete Triad. The competitive nature and extreme exercise expenditure of female athletes at the Division 1 level, increases their risk of experiencing the components of the Female Athlete Triad. The purpose of this study was to determine the prevalence of the Female Athlete Triad in NCAA Division 1 female athletes. A total of 100 participants were chosen from various sports through Gardner-Webb University athletics. Each participant that met the inclusion criteria filled out an informed consent form before completing the surveys. The participants were asked questions about eating habits, menstruation and history of stress fracture. The results were evaluated with two separate analyses. The first was a chi-squared test was used to assess the different proportions risk factors associated with all of the participants. The second was used to evaluate each risk factor for each component of the Female Athlete Triad (the three questionnaires) as separate variables. It is hypothesized that NCAA Division 1 female athletes are at an increased risk of developing at least one, if not all components of the Female Athlete Triad.

Book excerpt: The female athlete triad and the recent introduction of the relative energy deficiency in sports (REDS) classifications define the negative consequences of the relationship between energy availability and overall physical health in athletes. The current triad paradigm considers the spectrum of energy availability of the athlete that can range between appropriate exercise with adequate fueling to more extreme weight loss methods such as restriction and over-exercising that may be associated with low energy availability (LEA) or a formal eating disorder. Eating disorders are typically characterized by restriction in dietary intake affecting total calories, macronutrients, and micronutrients consumed. In order to determine the diet composition, diet records and food frequency questionnaires are the two most common recording methods. However, their reliability and ability to be replicated in the female athlete population remains unclear. Furthermore, the macronutrient distribution of the female athlete's diet has been given little attention to establish if this distribution plays a role in the presence of LEA and skews total energy intake found in diet records.

Book excerpt: The purpose of this study was to evaluate the prevalence of the female athlete triad (disordered eating, amenorrhea, and low bone mineral density) in athletes and nonathletes and to determine if there was a difference between the two groups at the University of Wisconsin-Stout. The study consisted of 24 student athletes and 26 nonathlete students who completed a survey on disordered eating (Eating Disorder Inventory-3) and menstrual history. Also their height, weight and bone density (heel bone test) were recorded at the Nutrition Assessment Lab at UW-Stout. The study determined that 16.7% (n=4) of athletes and 11.5% (n=3) of nonathletes had both disordered eating and amenorrhea with no statistical significance between the groups. There were no participants in either group who had low bone mineral density when a t-score of -2.0 was used. In conclusion, the findings show that there were no cases of the complete female athlete triad among the athletes and nonathletes studied; however there are signs of the female athlete triad starting to form in both athletes and non-athletes.
