The pilot study, under the leadership of the United States and Belgium, focused on the current status of disaster preparedness in partici pating nations. An objective look was taken at the availability or resources to cope with disasters from an international standpoint. The types of disaster settings that were reviewed in the study included the following: (1) Earthquakes, floods, hurricanes, avalanches (2) Nuclear power plant accidents or spills (3) Water and aircraft accidents (4) Building fires, building collapses (5) Contamination by hazardous chemicals or biological waste (6) Civil disturbances A disaster can strike anywhere at any time. Some nations and local communities have well-developed disaster plans with which to meet catastrophic situations. The fact is that most communities are without written and coordinated disaster plans which offer their citizens the most effective and comprehensive protection in these unexpected and often critical situations. Disaster has been defined as a sudden event which involves large numbers of people and results in loss of life, serious injury and property loss. together with a severe disruption of community organization and services. In the United States, thousands of people are killed and injured in disasters every year. Thus, disasters constitute a serious problem in both individual and public health terms in this country alone. Disasters can be classified as natural and man-made.

Book excerpt: Over 3 million U.S. military personnel were sent to Southeast Asia to fight in the Vietnam War. Since the end of the Vietnam War, veterans have reported numerous health effects. Herbicides used in Vietnam, in particular Agent Orange have been associated with a variety of cancers and other long term health problems from Parkinson's disease and type 2 diabetes to heart disease. Prior to 1997 laws safeguarded all service men and women deployed to Vietnam including members of the Blue Navy. Since then, the Department of Veteran Affairs (VA) has established that Vietnam veterans are automatically eligible for disability benefits should they develop any disease associated with Agent Orange exposure, however, veterans who served on deep sea vessels in Vietnam are not included. These "Blue Water Navy" veterans must prove they were exposed to Agent Orange before they can claim benefits. At the request of the VA, the Institute of Medicine (IOM) examined whether Blue Water Navy veterans had similar exposures to Agent Orange as other Vietnam veterans. Blue Water Navy Vietnam Veterans and Agent Orange Exposure comprehensively examines whether Vietnam veterans in the Blue Water Navy experienced exposures to herbicides and their contaminants by reviewing historical reports, relevant legislation, key personnel insights, and chemical analysis to resolve current debate on this issue.

As early as 1921, an international convention regarding this matter was presented to govern ments of maritime countries for ratification. The First World Health Assembly, in 1948, recommended that WHO should establish, with the International Labour Of fice, a Joint Committee on the hygiene of seafarers. The first session of this Com mittee, held in 1949, defined the problems affecting the health of seamen and made a number of recommendations. In the opinion of this Joint Committee, the health of seamen called for interna tional attention for a nu mb er of reasons. By the nature of his work, the seafarer is obliged to travel from country to country and is exposed to great variations of cli mate and also to any disease that may be prevalent in the port of call. He may there fore become a carrier of disease, so that the protection of his health is of importance not only to himself and the other members of the crew but also to the populations of other countries he visits. Yet, on account ofthe nature ofhis employment, it is diffi cult to provide the seafarer with the same standard of health care that is gene rally available to other sections of the population.
