Book excerpt: Prior to the 1995 election, the relationship between the OMA and the government was negotiated through "Framework Agreements".8 In 1995, the newly -elected government was concerned by evidence of a dramatic increase in the rate of medical service utilization. [...] A court should be cautious about characterizing structural changes to OHIP which do not shut out vulnerable persons as discriminatory, given the institutional impediments to design of a healthcare system by the judiciary." In this case, the changes to the Schedule of Benefits were found not to discriminate within the meaning of s. 15(1) of the Charter and the application was dismissed on that grou [...] For example, the terms of reference of the RBRVSC make no explicit provision for input into its decisions by actors besides the OMA or MOHLTC.29 The sum total seems to be that, despite the current PSC-centred bargaining framework's integration of evidence with rationing choices, amendments to the schedule are ultimately treated by this framework as a largely private matter best left to the interpl [...] An interesting recent episode of rationing in the de-listing process was brought about by the requirement in the 1997 Agreement that the PSC find $50 million in savings in the overall cost of the Schedule of Benefits. [...] Given the importance of changes to the OHIP schedule to the broader public interest, these are the main weaknesses of the current MOHLTC-OMA bargaining model.

Book Bending the Cost Curve in Health Care

Download or read book Bending the Cost Curve in Health Care written by Gregory P. Marchildon and published by University of Toronto Press. This book was released on 2014-12-03 with total page 518 pages. Available in PDF, EPUB and Kindle. Book excerpt: Through Canadian and international perspectives, Bending the Cost Curve in Health Care explores the management of growing health costs in an extraordinarily complex arena. The book moves beyond previous debates, agreeing that while efficiencies and better value for money may yet be found, more fundamental reforms to the management and delivery of health services are essential prerequisites to bending the cost curve in the long run. While there is considerable controversy over direction and details of change, there also remains the challenge of getting agreement on the values or principles that would guide the reshaping of the policies, the structures, and the regulatory environment of health care in Canada. Leading experts from around the world representing a range of disciplines and professional backgrounds come together to organize and define the problems faced by policy-makers. Case studies from the United States, the United Kingdom, Australia, the Nordic countries, and industrialized Asian countries such as Taiwan offer useful reform experiences for provincial governments in Canada. Finally, common Canadian cost factors, such as pharmaceuticals and technology, and paying the health workforce, are explored. This book is the first volume in The Johnson-Shoyama Series on Public Policy, published by the University of Toronto Press in association with the Johnson-Shoyama Graduate School of Public Policy, an interdisciplinary centre for research, teaching, and executive training with campuses at the Universities of Regina and Saskatchewan.

Download or read book Learning What Works written by Institute of Medicine and published by National Academies Press. This book was released on 2011-06-09 with total page 610 pages. Available in PDF, EPUB and Kindle. Book excerpt: It is essential for patients and clinicians to have the resources needed to make informed, collaborative care decisions. Despite this need, only a small fraction of health-related expenditures in the United States have been devoted to comparative effectiveness research (CER). To improve the effectiveness and value of the care delivered, the nation needs to build its capacity for ongoing study and monitoring of the relative effectiveness of clinical interventions and care processes through expanded trials and studies, systematic reviews, innovative research strategies, and clinical registries, as well as improving its ability to apply what is learned from such study through the translation and provision of information and decision support. As part of its Learning Health System series of workshops, the Institute of Medicine's (IOM's) Roundtable on Value & Science-Driven Health Care hosted a workshop to discuss capacity priorities to build the evidence base necessary for care that is more effective and delivers higher value for patients. Learning What Works summarizes the proceedings of the seventh workshop in the Learning Health System series. This workshop focused on the infrastructure needs-including methods, coordination capacities, data resources and linkages, and workforce-for developing an expanded and efficient national capacity for CER. Learning What Works also assesses the current and needed capacity to expand and improve this work, and identifies priority next steps. Learning What Works is a valuable resource for health care professionals, as well as health care policy makers.

Download or read book Shaping Ontario s Physician Workforce Building Ontario s Capacity to Plan Educate Recruit and Retain Physicians to Meet Health Needs written by and published by . This book was released on 2001 with total page 112 pages. Available in PDF, EPUB and Kindle. Book excerpt: This is the report of a panel formed to provide advice on the most effective ways to ensure an adequate supply of physician services and appropriate use of other health professionals over the medium to long term in Ontario. To analyze key issues, the panel established three working groups, conducted primary research on improving physician workforce data and secondary research on managing physician resources in Ontario & other jurisdictions. The panel also received submissions from interested parties. The report begins with background on the panel & its work, then reviews the challenges of human health resource planning, past efforts at physician resource planning, and current efforts to manage the physician supply. The main part of the report lays out the key steps that Ontario can take to plan services to meet needs, provide appropriate education, produce the right supply & mix of physician services, and attract & retain physicians where they are needed. Recommendations are made throughout on actions to achieve those objectives. The final part contains preliminary cost estimates and a proposed implementation schedule. Appendices include a section that develops a model of access to health care services and presents results of various scenarios of physician supply.
