Book excerpt: Excerpt from Some Points on the Physical Examination of the Chest Spirometry, once much in vogue, seems now to have fallen almost into disuse. Yet the indications afforded by it are valuable, and the more so as they are capable of exact record. Perhaps one reason that it has lost favor is that, as generally practiced, it is inherently uncleanly An instrument that is blown into by numbers of people of all sorts and conditions, and in all stages of pulmonary disease, and that cannot conveniently be cleansed after each use, must soon become repulsively and dangerously contaminated. This may be avoided by having the patient breathe from the receiver, instead of into it. The receiver, properly balanced, is filled with air by drawing it up to its fullest height, and the patient, having emptied his lungs as completely as possible, inhales from the instrument until his chest is expanded to the utmost. The number of cubic inches of air withdrawn from the receiver is then read off from the scale. If the instrument is properly arranged, the results should not vary from those procured in the usual manner. Of course the scale should read from below upward, instead of from above downward. The chief usefulness of the spirometer, it appears to me, is in the indications it affords for pulmonary gymnastics. In no other way is the need for such exercises, in suitable cases, made so apparent, both to the practitioner and the patient, and in no other way can the good results be so readily demonstrated, and perseverance on the part of the patient secured. Moreover, the use of the instrument is in itself of no small value in correcting the very defects it brings to light. In fact, it is very common to find that a decided increase in the vital capacity, to use Hutchinson's term, follows upon the first few efforts to inflate the lungs to their fullest limit. Book excerpt: Excerpt from Auscultation and Percussion The primary object of the physical examination of the chest is the attainment of 'a knowledge of the physical condi tion of the important organs contained within it. Not until this knowledge has been obtained are we in a position to de termine with all possible accuracy the cause or causes which have led to, or which underlie, those deviations from the normal physical condition revealed to us by the methods about to be described. Essentially the same physical con dition may be encountered in widely difierent diseases - z'. E., as the result of quite difierent causes - to discriminate be tween which the family and previous history of the patient; the influences to which he has been exposed; the symptoms which he presents, with their mode of onset, progress, and sequence; a careful examination of the patient as a whole, of his other organs or systems of organs; and, finally, a thorough knowledge of the natural history of general and local morbid processes; must all likewise be duly noted and weighed. In a word, percussion and auscultation - the two chief modes of thoracic physical exploration - lead directly to the detection of diseased conditions, only indirectly to that of diseases. Health precedes disease; it is, therefore, incumbent on us to master healthy conditions first. Perfect familiarity with the anatomy of the healthy chest and its contents, with the structure of each separately and the mutual relations of all is of vital importance. Furthermore, the variations of size and relation within the limits of health which may occur in the same person at difierent periods of life or under difier ent conditions - as in activity and repose - as well as the limits of normal variation in difierent persons. Must beknown. Finally, the physiology of respiration and circula tion must be thoroughly understood. The possession of most of this knowledge is 'here presupposed, inasmuch as this series of Manuals is intended rather for physicians than for younger students. We may, consequently, now go on to consider the special methods of physical exploration and the results which they may be made to yield in health and dis ease. At the same time the accompanying plates, after Weil, may serve to refresh the memory as to the space occupied by the thoracic viscera and their mutual relations. Book excerpt: Excerpt from A Handbook of Physical Diagnosis: Comprising the Throat, Thorax, and Abdomen It is, further, an essential part of a systematic pathological inquiry that a certain uniform order be Observed in the application of the different methods; the natural order is that first the general physical signs, and then the special, be taken up, as, without a knowledge of the former, the latter are apt to be misunderstood, and to lead to erroneous conclusions. The examination should begin, therefore, with the general inspection of the body; the practised eye often discovers in this way a multitude of signs, which not merely declare which of the organs is at fault, but fre quently reveal the nature and stage of the disease with absolute accuracy. Then only should the special examination be proceeded with. This ought never to be limited to the particular organs or parts concerning which the patient makes special complaint, as his sensations are often excited by purely adventitious circumstances. In the graver class of cases the condition of all the organs must be ascertained; the physician thus fre quently lights upon disorders of whose existence the patient, from the absence of subjective symptoms, had no conception, or complications are discovered without the knowledge of which a true understanding of the nature of the disease, or the formation of a sound diagnosis, is impossi ble. The pathology of the thoracic and abdominal organs presents nu merous examples of the intimate relation which the various diseases of these parts bear to each other, and of the importance of keeping this pathological connection before the mind in forming an opinion as to the origin and nature of the morbid process under investigation. The correct books are too large; the small books are out of date and repeat such well-worn myths as that the aortic second sound is normally louder than the pulmonic second, that aortic regurgitant murmurs are usually best heard in the sec ond right interspace, that a hypertrophied left auricle can produce dulness and pulsation near the left sternal border, that systolic re traction at the cardiac apex means adherent pericardium, that epi gastric pulsation denotes hypertrophy of the right ventricle, etc. Further, none of the smaller text-books contains any adequate ao count of muscle sounds, of pulrnonary atelectasis, or of adherent pericardium. To record the well-known but often forgotten truth on such matters as these has seemed to me of importance in small books as well as in encyclopaedic treatises. My original drawings were but roughly made in the hurry of a student's life - they have therefore been cast aside, and their plan, only, retained in this work, the plates of which are taken-from fresh specimens of disease, and have been executed by a skilful and practised artist. In the pages which treat of acoustics, I have attempted to collect, in a simple and intelligent form, the principles upon which the physical diagnosis of disease within the chest is based, freely illustrating them with examples, in order more closely to connect the theories of the science with the practice of the art. The succeeding chapters are principally occupied with dissertations on the plates, and directions for educating the organs of sense to appreciate the physical signs of disease. My endeavour has been throughout to produce a short, simple, and strictly practical work, which should, if pos sible, omit nothing absolutely essential to a fundamental knowledge of the subject, and yet contain nothing that could safely be omitted. I must confess, however, that the difficulties I have encountered on my way leave me no hope of having kept this happy middle course without digressions on one side or the, Other. When I have ventured to make definite statements on subjects fairly open to discussion, I trust to be acquitted of presumption, on the grounds that these subjects could not be left unnoticed, while the Objects and limits of my book precluded the introduction of lengthy arguments.
