Book Patient Better A Comprehensive Guide to Self Health Management

Download or read book Patient Better A Comprehensive Guide to Self Health Management written by Jennifer Woodruff and published by Patient Better LLC. This book was released on 2019-06-19 with total page 158 pages. Available in PDF, EPUB and Kindle. Book excerpt: This book is the first book of an educational series for people to learn how to self-manage care. Patient Better specializes in helping patients and their informal caregivers collaborate an at-home care team, enhance communication with clinicians, and organize their own medical managing system. This program teaches everyday people through meaningful learning, which is a concept where previously learned information is applied and connected to new information. Because our students come from such diverse backgrounds, we begin our training at the grassroots of healthcare and focus on developing the administration skills required to properly self-manage care. KEY ADMINISTRATION PRINCIPLES: (1) Enhance Participation in Treatment – Have fewer complications, lessen emergency room visits, construct an information-driven medical appointment, and reduce unforeseen costs. (2) Responsible Medical Utilization – Prepare for medical appointments, reduce unnecessary phone calls and office visits, and effectively take advantage of offered services, treatments, and resources. (3) Improve Risk Management – Have a realistic calculation of services needed for proper treatment and recovery to make more informed decisions. (4) Properly Self-Manage Care Records – Become an efficient liaison in the transfer of information from one doctor’s office to the next. Difficult topics of educational “soft spots” answered: · Healthcare Finance Management · How to reverse an approved and then later denied claim · Read and understand the doctor’s medical notes · Properly Treatment Plan · Understand the Outcome’s (and measures) purpose and importance · Conduct health care research online and discuss findings with practitioners · Create a personal medical record keeping system · Enhance informal caregiver’s capabilities and participation

Book Registries for Evaluating Patient Outcomes

Download or read book Registries for Evaluating Patient Outcomes written by Agency for Healthcare Research and Quality/AHRQ and published by Government Printing Office. This book was released on 2014-04-01 with total page 396 pages. Available in PDF, EPUB and Kindle. Book excerpt: This User’s Guide is intended to support the design, implementation, analysis, interpretation, and quality evaluation of registries created to increase understanding of patient outcomes. For the purposes of this guide, a patient registry is an organized system that uses observational study methods to collect uniform data (clinical and other) to evaluate specified outcomes for a population defined by a particular disease, condition, or exposure, and that serves one or more predetermined scientific, clinical, or policy purposes. A registry database is a file (or files) derived from the registry. Although registries can serve many purposes, this guide focuses on registries created for one or more of the following purposes: to describe the natural history of disease, to determine clinical effectiveness or cost-effectiveness of health care products and services, to measure or monitor safety and harm, and/or to measure quality of care. Registries are classified according to how their populations are defined. For example, product registries include patients who have been exposed to biopharmaceutical products or medical devices. Health services registries consist of patients who have had a common procedure, clinical encounter, or hospitalization. Disease or condition registries are defined by patients having the same diagnosis, such as cystic fibrosis or heart failure. The User’s Guide was created by researchers affiliated with AHRQ’s Effective Health Care Program, particularly those who participated in AHRQ’s DEcIDE (Developing Evidence to Inform Decisions About Effectiveness) program. Chapters were subject to multiple internal and external independent reviews.
