Despite Bangladesh’s impressive strides in improving its economic and social development outcomes, the government still confronts health financing and service delivery challenges. In its review of the health system, this study highlights the limited fiscal space for implementing UHC in Bangladesh, particularly given low public spending for health and high out-of-pocket expenditure. The crisis in the country’s human resources for health (HRH) compounds public health service delivery inefficiencies. As the government explores options to finance its UHC plan, it must recognize that reform of its service delivery system with particular focus on HRH has to be the centerpiece of any policy initiative.

Ensuring universal access to affordable, quality health services will be an important contribution to ending extreme poverty by 2030 and boosting shared prosperity in low-income and middle-income countries (LMICs), where most of the world's poor live. The book synthesizes the experiences from 11 countries – Bangladesh, Brazil, France, Ethiopia, Ghana, Indonesia, Japan, Peru, Thailand, Turkey and Vietnam – in implementing policies and strategies to achieve and sustain UHC. These countries represent diverse geographic and economic conditions, but all have committed to UHC as a key national aspiration and are approaching it in different ways. The study examined the UHC policies for each country around three common themes: (i) the political economy and policy process for adopting, achieving, and sustaining UHC; (ii) health financing policies to enhance health coverage; and (iii) human resources for health policies for achieving UHC. The findings from these country studies are intended to provide lessons that can be used by countries aspiring to adopt, achieve, and sustain UHC. Although the path to UHC is specific to each country, countries can benefit from the experiences of others in learning about different approaches and avoiding potential risks.

Book excerpt: A strong health financing system will be an important priority as Bangladesh seeks to meet theexpectations of its population and embrace its future as a middle-income country. The objective of this report is to present policymakers with a foundation for an informed discussionabout government health spending and future health financing options in Bangladesh. It analyzesalternative sources of additional health financing and their potential contribution to higher spending in the sector. The findings will be relevant to both the Ministry of Finance (MoF) and Ministry of Health and Family Welfare (MoHFW) and can inform a dialogue between the two Ministries as the government seeks a way forward. Fiscal space for health is a particularly important policy challenge for Bangladesh because it currently has one of the lowest levels of government health spending in the world. Fiscal space for health refers to the availability of budgetary room that would allow a government to increase health spending without jeopardizing the sustainability of its fiscal position. In general there are five potential sources of fiscal space: (1) economic growth which translates into more overall government revenues and, in turn, a larger health budget; (2) a reprioritization within the budget in favor of health, resulting in higher expenditures; (3) increased overseas development assistance (ODA) for health; (4) an increase in health-specific resources, such as earmarked taxation or insurance contributions; and (5) greater efficiency in the use of existing health budget resources such that additional resources become available for new spending. The report also presents illustrative scenarios to show the potential impact that these channels can have on fiscal space for health in Bangladesh going forward. In summary, financing health in Bangladesh over the short to medium term will require a combination of existing and additional domestic resources that result from economic growth, improved tax collection, and most significantly a moderate re-prioritization of the budget in favor of the health sector. A longer-term agenda will relate to the possibility of generating additional resources through individual contributions starting with the formal employed sector, if that is the path chosen.

The ideas presented here may look outrageous when viewed through the prism of what has been the norm, but the authors demonstrate the feasibility of the changes they propose by providing concrete examples of how the changes would work. UHC is within India's means if the political will to implement radical change of the kind described in this book can be mustered. Health for all citizens is not an option but an imperative for a just and thriving republic.
