Book excerpt: Excerpt from The Orthopedic Treatment of Gunshot Injuries It is a satisfaction to give a welcome to a book that comes to us at a time when it is definitely needed. We are having to decide problems in surgery, which are either new, or which present such new phases, that we must re-adjust ourselves in the methods of their reception and treatment. Dr. Mayer, in his presentation of the subject in this book, has led to these important questions, and has given the results of a practical experience with many of the new problems which have come to us in the last three years of military life. It has been definitely demonstrated that early radical methods are frequently necessary for ultimate conservative results, and for the final completion of full function, and that continued treatment given as early as possible, and in many instances that the early application of correct mechanical principles, are necessary, if we are to have in mind the complete rehabilitation of the injured man. The importance of the restoration of the disabled individual back into his working life is now having its proper recognition, under the stimulus of war conditions, although we must remember that this problem is not new, for it has had, for some time, the attention of a few men, who had recognized its importance and have done much toward establishing this feature in industrial surgery. The need of surgeons who have a knowledge of these correct principles and of the mechanical supplements to surgery, is emphasized in this work of Dr. Mayer. Dr. Mayer has called attention to the need of planning the treatment of long surgical cases so that the patient can be restored finally to his full function, and has also emphasized that this includes not only the restoration of his physical and mental condition, but also the restoration of the individual himself back to the position which places him in a wage-earning status. Book excerpt: Excerpt from The After Treatment of Wounds and Injuries This book does not pretend to be a complete account of the principles and practice of orthopaedic surgery. In it I have tried to give the results of the experience acquired during two and a half years' work in a Military Orthopaedic Hospital, to state the principles which I have been led to adopt, and to give sufficient examples of methods and results to guide others in their practical work. The principles of the work are the same as the principles of the older civil orthopaedic surgery. They depend upon three things, a knowledge of pathology, a clear appreciation of mechanics, and the realisation that the surgeon's aim is to restore function. Many new methods have appeared in military orthopaedics, but in almost every case they depend upon the application of an old principle. In Chapter II I have thought it well to describe the rational treatment of chronic sinuses of bone, perhaps the most frequent sequel of a wound, and the most important cause of delay in reparative work. I adopted the radical operation described from Prof. Broca's book and have found it most successful. It is now, I believe, generally adopted all over the country. Chapters III to X are concerned with the principles of treatment. Chapters XII to XVI deal with injuries to specific regions and contain many examples of actual cases. Chapters XVII to XIX give a very brief account of the methods of splinting, plaster-of-Paris work, and physical methods of treatment. A good practical, knowledge of splinting and of the use of plaster of Paris is an essential part of the training of an orthopaedic surgeon; without resource in these methods he is lost. Physical methods, massage, baths, electric treatment and exercises are usually left in the hands of specialists, but the surgeon should have a general knowledge of their methods and objects. Chapter XIX only pretends to indicate this in outline, to give a minimum that the surgeon must accept. A fuller knowledge is desirable, and for this books on the special branches of therapeutics must be read. Rest in the recumbent position, with the chest slightly elevated, is essential in aiding spontaneous arrest of hemorrhage and in the prevention of complications. A rise in the temperature in the first forty-eight hours is no indication of sepsis, as, with few exceptions, it indicates a febrile dis turbance caused by the absorption of fibrin ferment, the so-called fermentation fever. Should later symptoms set in suggestive of septic infection, aspiration should be promptly resorted to, and if not followed by speedy improvement, no time should be lost in subjecting the pa tient to the same medical treatment as advised and practised for empyema, that is, rib - resection, free incision and drainage. The production of an artificial pneumo thorax or hydrothorax by the introduc tion into the pleural cavity on the injured side of a nontoxic gas or filtered atmos pheric air or sterilized water or nontoxic antiseptic solutions has not proved satis factory in the treatment of infra-tho.racic traumatic hemorrhage. From what has been said it is clear that the best treatment in penetrating gunshot wounds of the chest consists in hermetically seal ing the wound of entrance and exit, if such exists, under strict aseptic precau tions, and in watching for and treating subsequent complications as they present themselves.
