Book excerpt: Excerpt from The Ophthalmic Record, Vol. 12: A Monthly Review of the Progress of Ophthalmology; June, 1903 Burnett was the first to call attention to the peculiar and remark able appearance of the conjunctivae of the negro in this disease and so complete is his description that I cannot do better than to quote it verbatim: This consists in a brownish discoloration of the con junctiyal tissue, most intense at the scleral base of the mass, and gradually fading away toward the equator of the ball. This discolor ation. Which is undoubtedly situated in the conjunctival tissue itself, as well as in the epithelial layer, is not regular in its distribution but appears to consist of a collection of small deposits of brown pigment. These deposits are sometimes collected in rather large masses, particu larly near the base of the elevation, but towards the equator they appear as small, brownish Specks in the dull and succulent conjunctiva. In some instances this brownish discoloration extends but 3 mm. From the circum - corneal elevation, while in others it reaches almost to the canthus. It does not usually extend so far towards the equator of the globe in either superior or inferior direction. In one case I have seen the whole of the conjunctiva visible through the palpebral opening present this dirty, light-brown appearance in striking contrast to the brilliant, white aspect which the sclera usually presents in this race. His philosophic caution is to be admired. Not so the repetition of objections which have long been answered. For instance, he doubts the importance of a closed filtration angle because it has been found in eyes which are not glaucomatous. The facts of the case need hardly be repeated, but we would suggest to Dr. Berry the following simile. Let the steam pipe of a boiler be closed while the boiler is in full work the steam pressure will rise. But one does not expect a rise of pressure if steam is not being generated, or if it is escaping freely elsewhere. It need not be said that the subjects of refraction and accommodation are dealt with in a thorough and masterly manner. The student of ophthalmology who has the necessary knowledge of mathematics may here dive into some of the profundities of physiological optics, and those who cannot follow the author through all his formulae may safely pass the small print by and trust to the large for all essential facts. A yellow mass fills the lower two-thirds of the anterior chamber. The iris and lens are in situ. The retina is thickened, and the optic disc swollen. Microscopical Appearances. - There is Slight diffuse round cell infiltration throughout the whole cornea, but there are two areas in the sections where it is much more intense than elsewhere. These correspond to the two Sides of the ring described in the clinical notes. One of these areas Of infiltration commences on the inner Side of the wound. The layers of fibrous tissue Of the cornea immediately beneath the epithelium are not the most involved, but those about the middle of its thickness. There is some accumulation of cells between Descemet's membrane and the fibrous tissue. Between the lips of the wound is a thin layer of cells; the fibrous tissue external to it is but little infiltrated. In the anterior chamber there is a delicate network of fibrinous material supporting numerous round cells with dividing nuclei. Material presenting a Similar appearance is seen in the circumlental Space and in the anterior portions of the vitreous. There is Slight round cell infiltration of the iris and ciliary body. Case II. - Wound a little inside centre of cornea. Excision less than twenty-four hours later. Nearly complete ring of infiltration.
