Book excerpt: Excerpt from Ohio Medical and Surgical Journal, 1864, Vol. 16 The evidence in this case tends to show, that Joseph Saul, an infirm old man residing in Franklin county, Ohio, conveyed his property there, worth about $1000, to Peter Priest, in considera tion of which Priest agreed to support him during his life. That Priest married an adopted daughter of Saul, raised by him, and Saul resided with Priest and his wife; that Priest sold his Frank lin county property, and bought a small farm in Paulding county; that he with his wife and Joseph Saul started to remove from Franklin county to Paulding county, to reside there; that on the way Saul was taken violently and suddenly sick, at Big Island, Marion county, shortly after he went to bed; that the next day they went on to Kenton, and staid all night at the house of a rela tive of Priest the defendant, where Saul died on the night of the day after which they arrived, and was interred the next day, May 5, 1860, in the cemetery near Kenton. Amidst the snows of the extreme North, and on the sunny plains of the tropic, has it sent forth its noisome exhalations, polluting the air, and propagating its poison wherever victims could be found. Previous to the introduction, of vaccination, the deaths from small-pox in London alone averaged about two thousand annually, as appears from the bills of mortality. In a single year, 1776, more than three thousand and five hun dred died of this disease in that city. The Spanish historians of the conquest of Mexico relate that in a short time after the small-pox reached the capital more than three millions and a half of lives were lost by it in that nation alone. The conclusions to which they would lead are, that phlegm asia dolens or phlebitis is caused by the introduction of diseased matter, usually pus, into the blood - that inflammation of a vein is not an essential part ofthe primary affection, which precedes constitutional symptoms, even when morbid matter has found its way into the circulation through a vein, and that when the veins are inflamed, it is an effect, and not the cause of the recep tion of diseased or foreign matter into them. This is done for empyema, pyothorax, or suppurative pleuritis. These names denote a variety of pleuritis, characterized by the accumulation, in the pleural cavity, of purulent fluid. The term empyema only ex presses the existence Of pus, without indicating its situa tion. Pyothorax is a better name, but this does not express the existence of inflammation. Suppurative pleuritis is a simple title suggested as more fully expressive of the affec tion. It is not necessary for me, her to go into the clinical history of this disease. Suffice it to say, the pathological character is suppurative - acute, or sub-acute inflammation being the cause. A positive diagonsis, perhaps, may be made in cases in which spontaneous perforation of the chest does not occur, or, without waiting for the occurrence of this event, a small exploring trochar may be introduced into the chest, and a few drops of the liquid withdrawn; the gross and microscopical characters of the Specimen thus oh tained will at once settle the question. This procedure is warrantable as a means of diagnosis in view of the bearing on the prognosis and treatment. Indeed, the operation is so slight as to be in no wise objectionable. In both the cases on which I operated by incision, the relief was immediate - one made a quick and perfect recovery, the other suc cumbed from secondary cau es. The greatest objection to incision has been the fear of admiting air into the pleural cavity, and thereby increasing the inflammation, indepen dent of the effect on the suppuration. In my experience, this has been exaggerated, and, in future, I shall never delay the Operation on this account. I Opened the chest of my first patient about thirteen years ago. He fell under my care after an acute attack of pleuro-pneumonia, from which he very imperfectly recovered, having a cough remaining, with some dyspnoea, for nearly five months. He gradually grew worse, but his former attendan't Opposed surgical interference, until his great emaciation and short ness of breath demanded it. I made the Opening in the intercostal space on the left side, below the nipple, evacuat ing six or seven pints of fetid pus; it kept flowing during the next day. I then washed it out with carbolic acid freely, and for the patient ordered quinine and iron, with generous diet. He sank in about a week from inflammation of the right lung, followed by gangrene. And such is the intimate relation and dependence of one important function on all the others, that no sudden disturbance of one can take place without inducing more or less change in all. Thus disease, occupying any considerable part of the cutaneous surface, suspends the elimination from the skin, causing, on the one hand, rapid accumulation of perspirable matter in the blood; thereby exerting a universal irritant influence; and on the other, eqnally' rapid accumulation of heat from the diminished insensible evapo ration, by which, in health, the caloric on the surface of the body is constantly being rendered latent, and conveyed away. F rom these two alterations - the retention of irritating matter in the circulating fluids, and the accumulation of caloric, general febrile symptoms are very quickly developed. Again, inflammation of any portion of the respiratory apparatus suficient to materially impede respiration quickly alters the interchange of oxygen and carbonic acid through the lungs, thereby altering the quality of the blood, and, through it, the functions and properties of the system. If we bear in mind the fact, that every change in the capillary circulation is accompanied by a change of function, we shall see that the first step in the inflammatory process, namely, determina tion of blood to, and its accumulation in the inflamed part, almost necessarily involves more or less alteration in the capillary cir culation generally, and consequent] y must induce at once some general disturbance.
