Book excerpt: The purpose of this study was to analyze colorectal cancer survival data for patients in Mississippi diagnosed between 2003 and 2011. The data was obtained from the Mississippi Cancer Registry. Several variables were included in the file for analysis including age, race, diagnosis date, date of last contact, vital status, census tract level poverty, county of residence at diagnosis, and stage of disease at diagnosis. Survival is of interest to the Centers for Disease Control and Prevention National Program of Cancer Registries and the National Cancer Institute's Surveillance, Epidemiology and End Results programs, the Mississippi State Department of Health, and the University of Mississippi Medical Center because survival indicates impact of treatment on cancer. This research analyzed the impact of three factors on the racial disparity in relative survival from colorectal cancer: residing in the Delta/ non-Delta areas of Mississippi, age, and socioeconomic status as defined by census tract level poverty. Colorectal cancer is the second most common cancer in Mississippi affecting both males and females accounting for 1,649 new invasive cases and 615 deaths in 2013. Mississippi ranks second in the country for incidence and mortality from colorectal cancer based on 2013 data. In Mississippi, mortality rates in whites have declined since 1975, but the mortality rates for blacks have been stable since 1991. Prior studies addressing colorectal cancer survival covered only limited sections of the United States that did not include Mississippi. Understanding factors associated with colorectal cancer survival such as place, age and socioeconomic status will inform treatment, education and screening programs in Mississippi. A retrospective cohort research design was used to study data collected by the Mississippi Cancer Registry (MCR). The study population included black or white adults ages 18-99 diagnosed with colorectal cancer between 2003 and 2011 in Mississippi with passive follow-up through 2013. Cases were recoded to reflect their vital status on December 31, 2011 due to the life tables which ended in 2011. Only the first primary colorectal cancer for each individual person was used in the analysis. This research project used the following categorical data: race, age at diagnosis, year of diagnosis, census tract poverty as a measure of SES, place (Delta/non-Delta), and stage of disease at diagnosis (SEER summary stage 2000). Variables for which categories were established include age at diagnosis, SEER Summary Stage 2000, and diagnosis year. Age was combined into three age groups: less than 50, 50-64 and 65 and older. SEER Summary Stage was divided into two categories of stage, local and advanced. Poverty was divided into two categories, low and high. Descriptive statistics were generated for each variable of race, year at diagnosis, age at diagnosis, Delta and non-Delta regions, census tract poverty, and the combinations of age and stage of disease, Delta and non-Delta regions with stage of disease and census tract poverty with stage of disease. Also, descriptive statistics were generated on the combinations of race and age, race and region (Delta/non-Delta) and race and census tract poverty. Counts and percentages were calculated for the categorical variables while mean and standard deviations were calculated for the continuous variables. Chi square tests of significance were used to examine differences in categorical variables. The t-test was used to test differences in the continuous variable. Relative survival was calculated by age groups, census tract level poverty, and Delta/non-Delta regions for both whites and blacks the strel2 command in Stata 14 using a complete approach. Life tables for Mississippi by race, sex, age, and year (2003-2011) were obtained from the CONCORD-2 study of cancer survival in five continents. These tables are publicly available. Calculation of excess hazards and net survival in the proposed research for analysis of Delta/non-Delta regions, census tract level poverty, and age groups. Additionally, a separate analysis was conducted including stage of disease. The same analysis was also conducted with the addition of race to the model. Finally, analysis was done using both race and stage in the model. Excess hazards were compared using the 95% confidence intervals. Overlapping confidence intervals indicated no significant difference in hazards. In the analysis of region, the non-Delta region had significantly higher survival than the Delta region. When controlling for stage, the difference did not change for advanced disease. For local disease, the significant difference persisted but was delayed to the second year after diagnosis. For poverty groups, the low poverty census tracts had significantly higher survival than the high poverty census tracts. However, controlling for stage of disease at diagnosis reduced the difference in survival between the poverty groups to a statistically non-significant level. For the age groups, the 50 to 64 age group had significantly higher survival than both the less than 50 age group and the 65 and older age group. Controlling for stage of disease at diagnosis removed the difference between the 50 to 64 age group and the less than 50 age group. However, the 65 and older age group continued to have a significantly lower survival for both the local and advanced stage disease compared to the 50 to 64 age group and also showed a significant difference in survival for both the local and advanced age groups compared to the less than 50 age group. Significant differences in survival existed for region, poverty, and age groups overall. Controlling for stage alone only attenuated the survival difference for the poverty analysis. Controlling for race alone did not eliminate the survival differences between races in any of the three analyses. However, the within race survival differences did disappear for both poverty categories. For both regions, both poverty groups, and all three age groups, whites had significantly higher five-year relative survival rates than blacks. Controlling for stage of disease extended the period of similar survival between the races with local disease beyond the survival seen overall for region, poverty, and age group by race. For advanced disease, the significant differences between the races were still apparent within the first year after diagnosis for region, poverty, and age group. This study found that there is a significant difference in five-year relative survival between the Delta and non-Delta regions, between low and high poverty groups and between age groups. Controlling for stage of disease at diagnosis eliminated the difference in survival by poverty groups. However, the differences in survival remained for region and for age groups regardless of stage of disease. Comparing whites and blacks, whites had significantly higher survival compared to blacks for both regions, both poverty groups, and all three age groups. Controlling for stage of disease at diagnosis did not eliminate the racial differences in region, poverty groups or age groups. The within race difference did disappear for both poverty groups and both stages and for the Delta and non-Delta regions for local stage. The results of this study demonstrate the need for increased screening, ensuring standard of care for treating colorectal cancer for all patients, advancement of clinical trials in Mississippi, and management of chronic illnesses that can prevent a patient from getting necessary care. The Mississippi State Department of Health, University of Mississippi Medical Center, and other healthcare and community partners play a prominent role in addressing the issues identified through this study. Further research through multi-state analysis and patterns of care studies would further enlighten the health community on the colorectal cancer disparity.
