Book excerpt: The National Adolescent Student Health Survey (NASHS), initiated in 1985, is conducted to examine the health-related knowledge, practices, and attitudes of the nation's youth in the following health areas: AIDS; Nutrition; Consumer Health; Sexually Transmitted Disease; Drug and Alcohol Use; Suicide; Injury Prevention; and Violence. Findings identify areas in which students are well informed, as well as areas in which students have insufficient or inaccurate knowledge. Following a statement of purpose, the document is organized into five parts: (1) an overview presents the project's background, a history of NASHS questionnaire development, and the original study design; (2) implementing a survey, discusses school and student selection for participation, parental consent, and sample selection; (3) preadministration arrangements; (4) the day of survey administration; and (5) detailed instructions for survey administration. The bulk of the document consists of five appendices: (1) responding to students' questions about specific questionnaire items; (2) annotated questionnaires forms 1-3; (3) sample parental consent letter and form; (4) school information form with directions; and (5) NASHS survey booklets/questions.
