On the other hand, there are fears and misconceptions. I have heard many doctors say "I have never been sued for a section I did, but I have been sued for the section I did not do. " The fear of not having performed a section in my opinion is real, although difficult to prove, and until the public can be educated that cesarean section delivery cannot eradicate fetal death and damage, this fear will remain and will be responsible for some unnecessary cesarean sections. Bruce Flamm and I hope this book will correct misconceptions that have been responsible for many unnecessary cesarean sections. I am still frequently asked the same old question: What is an ideal cesarean section rate? I still give an answer similar to the 1983 answer, perhaps somewhat modified.

Crucial to astute clinical diagnosis is broad clinical experience. In short, you have to have seen one to recognize one! Patients with movement disorders nearly always present as outpatients but, as aptly recognized by Drs. Frucht and Fahn, this may include acute manifestations leading to emergency presentations, often in an emergency room setting, where they are very likely to be unrecognized and therefore poorly managed. The authors define an “emergency” movement disorder as one in which failure to promptly diagnose and treat may result in significant morbidity or mortality. However, they also stress the importance of certain “can’t miss” diagnoses such as Wilson’s disease, dopa-responsive dystonia, and Whipple’s disease in which delayed diagnosis in less emergent situations can lead to slowly evolving and often irreversible neurological damage with tragic consequences.

This book contains selected contributions to the 7th International Conference on Cerebral Vasospasm held in Interlaken, Switzerland, in June 2000. Part I of the book concentrates on basic science and experimental vasospasm. The molecular biology of vasospasm, the role of endothelin and nitric oxide as well as the potential of gene therapy are presented. Part II concentrates on the diagnosis and therapy of clinical vasospasm. New diagnostic tools are presented, including diffusion and perfusion-weighted MRI, MR spectroscopy and microdialysis with metabolic monitoring. Leaders in the field discuss the current indications and results of endovascular treatment of cerebral vasospasm.
