American vaudeville is here regarded as the carefully elaborated ritual serving the different and paradoxical myth of the new urban folk. It demonstrates that the compulsive myth-making faculty in man is not limited to primitive ethnic groups or to serious art, that vaudeville cannot be dismissed as meaningless and irrelevant simply because it fits neither the criteria of formal criticsm or the familiar patterns of anthropological study. Using the methods for criticism developed by Susanne K. Langer and others, the author evaluates American vaudeville as a symbolic manifestation of basic values shared by the American people during the period 1885-1930. By examining vaudeville as folk ritual, the book reveals the unconscious symbolism basic to vaudeville-in its humor, magic, animal acts, music, and playlets, and also in the performers and the managers—which gave form to the dominant American myth of success. This striking view of the new mass man as a folk and of his mythology rooted in the very empirical science devoted to dispelling myth has implications for the serious study of all forms of mass entertainment in America. On the other hand, there are fears and misconceptions. I have heard many doctors say "I have never been sued for a section I did, but I have been sued for the section I did not do. " The fear of not having performed a section in my opinion is real, although difficult to prove, and until the public can be educated that cesarean section delivery cannot eradicate fetal death and damage, this fear will remain and will be responsible for some unnecessary cesarean sections. Bruce Flamm and I hope this book will correct misconceptions that have been responsible for many unnecessary cesarean sections. I am still frequently asked the same old question: What is an ideal cesarean section rate? I still give an answer similar to the 1983 answer, perhaps somewhat modified.

Crucial to astute clinical diagnosis is broad clinical experience. In short, you have to have seen one to recognize one! Patients with movement disorders nearly always present as outpatients but, as aptly recognized by Drs. Frucht and Fahn, this may include acute manifestations leading to emergency presentations, often in an emergency room setting, where they are very likely to be unrecognized and therefore poorly managed. The authors define an “emergency” movement disorder as one in which failure to promptly diagnose and treat may result in significant morbidity or mortality. However, they also stress the importance of certain “can’t miss” diagnoses such as Wilson’s disease, dopa-responsive dystonia, and Whipple’s disease in which delayed diagnosis in less emergent situations can lead to slowly evolving and often irreversible neurological damage with tragic consequences.
