Book excerpt: Suffering is an unavoidable reality in healthcare. Not only are patients and families suffering but also the clinicians who care for them. Commonly the suffering experienced by clinicians is moral in nature, in part a reflection of the increasing complexity of health care, their roles within it, and the expanding range of available interventions that challenge their moral foundations. Moral suffering is the anguish that arises occurs in response to moral adversity that challenges clinicians integrity: the inner harmony that arises when their essential values and commitments are aligned with their choices and actions. The sources and sequelae of moral distress, one type of moral suffering, have been documented among clinicians across specialties. Transforming their suffering will require solutions that expanded individual and system strategies. Moral resilience, the capacity of an individual to restore or sustain integrity in response to moral adversity, offers a path forward. It encompasses capacities aimed at developing self- regulation and self-awareness, buoyancy, moral efficacy, self-stewardship and ultimately personal and relational integrity. Whether it involves gradual or profound radical change clinicians have the potential to transform themselves and their clinical practice in ways that more authentically reflect their character, intentions and values. The burden of healing our healthcare system is not the sole responsibility of individuals. Clinicians and healthcare organizations must work together to transform moral suffering by cultivating the individual capacities for moral resilience and designing a new architecture to support ethical practice. Used worldwide for scalable and sustainable change, the Conscious Full Spectrum approach, offers a method to solve problems to support integrity, shift patterns that undermine moral resilience and ethical practice, and leverage the inner potential of clinicians and leaders to produce meaningful and sustainable results that benefit all.

Book excerpt: Findings from this study show that work setting and having left a previous job were predictors of moral distress but total years' experience in oncology was predictive of moral courage. Moral courage was displayed in major areas of supporting the patient, risk taking, advocacy, enlarging the circle for decision-making, putting aside personal beliefs, respecting patient autonomy, empowering the patient, fighting for the patient in the face of consequences in a complex system, sharing information, getting to the meaning, handling tricky situations, protecting the patient and truth-telling. Despite experiencing levels of moral distress, oncology nurses demonstrate support and respect for patients' decision-making and autonomy. Ethics education derived from clinical practice can provide an opportunity for open discussion for nurses to create and maintain morally acceptable work environments that enable them to be morally courageous. This research underscores the presence of moral distress and moral distress residue among oncology nurses and the importance of finding ways to lessen moral distress and strengthen moral courage in nurses.

This report explores the extent, consequences, and contributing factors of clinician burnout and provides a framework for a systems approach to clinician burnout and professional well-being, a research agenda to advance clinician well-being, and recommendations for the field.

Every nursing act is measured against requisite moral standards to do no harm, to promote justice, to be accountable, and to provide safe and competent care. However, as nurses attempt to act, there may be obstacles to pursuing the course of care as agreed to with the patient: inadequate staffing, cost-containment strategies, and policy constraints. In attempting to reconcile ideals of practice with what may be an opposing reality, nurses may experience moral distress. The purpose of this study was to examine how structural empowerment, psychological empowerment, interprofessional collaboration, compassion, and the perception of the quality of care affect nurses' perceptions of moral distress and the relationship between moral distress and intention to leave. It is organized in an integrated article format comprised of 4 papers. The first paper traces how the concept of nurse moral obligation and the possible sources of nurse moral distress. The second paper is a scoping review of the variables that have been correlated with moral distress. The literature suggests the constructs structural and psychological empowerment, interprofessional collaboration, quality of care, and intention to leave are associated with it. Compassion is also considered as it is a core nursing attribute. The third paper concerns itself with understanding compassion fatigue and compassion in a nursing context. The fourth article reports the findings of a study conducted to exam the relationships of the variables structural and psychological empowerment, interprofessional collaboration, compassion, quality of care, and intent to leave with the variable moral distress. The study confirmed the hypothesized relationships between structural empowerment, psychological empowerment, quality of care, and moral distress but not between moral distress and intent to leave. One mediating but no moderating relationships were found. The findings of these papers demonstrate that moral distress continues to be an important issue in nursing. With these findings and with those from other studies there is an opportunity to begin the work on studies to evaluate strategies to mitigate moral distress as it relates to the nurse-patient therapeutic relationship.

Moral distress has been anecdotally associated with professional burnout and leaving a nursing position or the profession itself. Ethical climate is the perception of practicesand conditions within the work environment that facilitate the discussion and resolution of difficult patient care issues. Intent to turnover is a variable which measures an individual's likelihood of leaving a job.

Book excerpt: The concept of moral distress was first developed in the mid 1980's by the American philosopher Dr. Andrew Jameton. Since then, moral distress has been studied in many health care practice settings and among various disciplines. However, moral distress among nurses in community health care settings has remained largely unexplored. Community health nurses (CHNs) work with clients, families, and communities with a variety of diverse care needs. Therefore, in this research I cast a wide net by asking a broad question, "What guides us here?" What guides nurses through the ethical landscape of community health nursing? The purpose of this research, then, is to explore CHNs experiences of everyday ethical issues and moral distress and identify educational opportunities for ongoing learning and support. To do this I used a narrative inquiry approach and interviewed 20 Canadian CHNs by phone. Based on my analysis of the data, categories of key ethical issues, themes, and educational needs were explored and identified. From the interview data, participants' stories were then re-storied by me into meta-narratives. Nine meta-narratives, inspired by multiple-participant interview responses, were created as an end product of this study. The meta-narratives represent stories that are true to life in the details regarding situations that the CHNs reported from their nursing practice. At the same time, however, the meta-narratives do not describe any one situation; rather they reflect aspects of several stories combined. Thus, the meta-narratives offer protection of confidentiality, provide an evidence-based, detailed account of these experiences, and are educational resources for CHNs. I also designed and had participants evaluate an educational intervention in the form of a self-directed e-learning module (e-module). Based on the research findings, I offer recommendations for action to promote ethical nursing practice and support CHNs, which include implications for education, theory, research, and nursing practice. This research provides foundational knowledge about the ethical landscape of community health nursing in Canada, the everyday ethical issues faced, experiences of moral distress, and educational opportunities to support CHNs.
