Book excerpt: "Supporting a Movement for Health and Health Equity" is the summary of a workshop convened in December 2013 by the Institute of Medicine Roundtable on the Promotion of Health Equity and the Elimination of Health Disparities and the Roundtable on Population Health Improvement to explore the lessons that may be gleaned from social movements, both those that are health-related and those that are not primarily focused on health. Participants and presenters focused on elements identified from the history and sociology of social change movements and how such elements can be applied to present-day efforts nationally and across communities to improve the chances for long, healthy lives for all. The idea of movements and movement building is inextricably linked with the history of public health. Historically, most movements - including, for example, those for safer working conditions, for clean water, and for safe food - have emerged from the sustained efforts of many different groups of individuals, which were often organized in order to protest and advocate for changes in the name of such values as fairness and human rights. The purpose of the workshop was to have a conversation about how to support the fragments of health movements that roundtable members believed they could see occurring in society and in the health field. Recent reports from the National Academies have highlighted evidence that the United States gets poor value on its extraordinary investments in health - in particular, on its investments in health care - as American life expectancy lags behind that of other wealthy nations. As a result, many individuals and organizations, including the Healthy People 2020 initiative, have called for better health and longer lives.

Different individuals mobilize in different ways—some flee, some pick up arms, and some support armed actors as civil war begins. Drawing on nearly two hundred in-depth interviews with participants and nonparticipants in the Georgian-Abkhaz war of 1992–1993, Anastasia Shesterinina explores Abkhaz mobilization decisions during that conflict. Her fresh approach underscores the uncertain nature of the first days of the war when Georgian forces had a preponderance of manpower and arms. Mobilizing in Uncertainty demonstrates, in contrast to explanations that assume individuals know the risk involved in mobilization and make decisions based on that knowledge, that the Abkhaz anticipated risk in ways that were affected by their earlier experiences and by social networks at the time of mobilization. What Shesterinina uncovers is that to make sense of the violence, Abkhaz leaders, local authority figures, and others relied on shared understandings of the conflict and their roles in it—collective conflict identities—that they had developed before the war. As appeals traveled across society, people consolidated mobilization decisions within small groups of family and friends and based their actions on whom they understood to be threatened. Their decisions shaped how the Georgian-Abkhaz conflict unfolded and how people continued to mobilize during and after the war. Through this detailed analysis of Abkhaz mobilization from prewar to postwar, Mobilizing in Uncertainty sheds light on broader processes of violence, which have lasting effects on societies marked by intergroup conflict.

Such a comparative analysis helps understand how contextual factors shape distinctive struggling middle-class citizen responses to external shocks. Book excerpt: The political transformations initiated by the so-called Arab Spring in Egypt, Libya, Tunisia and Yemen have been marked by strong political contention, continued social mobilization and, albeit to different degrees, weak central state institutions. This book proposes that, rather than agreed roadmaps of institutional change (e.g. elections, drawing up new constitutions) and centrally crafted transition processes, it has been the competition of key political actors for resources of political power and control that has set the pace and influenced the direction and depth of the transformation processes. Hence, the contributions in this volume use an actor-centred approach. Two perspectives are assumed: first key political actors – referring to the "Politically Relevant Elite (PRE)"– are identified and their motivations as well as their strategies and capacities to steer the transformation process. Secondly , the authors investigate the capacity of politically "Mobilized Publics" to exert influence on agenda setting and decision making, ask to what extent popular and social movements have emerged as political actors in their own right, and to what extent such forms of bottom-up participation have constituted a fundamental change to the political culture of these countries. Both avenues of inquiry analyze how the elites are constrained by continued social mobilization, how they engage with mobilized publics to promote their own agendas, and whether the extended scope of popular participation contributes to the legitimacy and stability of the emerging political orders, or causes disruption, fragmentation and conflict.
