CHAPTER II FIXED IDEAS THE study of suggestion has shown us that the thoughts of hystericals are not equilibrated; that under diverse influences one of them may develop to an extreme extent'and live, so to say, isolated, its own life, to the great detriment of the mental organism. This tendency is not only manifested in artificial experiments; it continually gives place to natural phenomena, which are quite analogous to suggestions. Fixed ideas are for us phenomena of this kind; that is to say, psychological phenomena which are developed in the mind in an automatic manner, outside the will and the personal perception of the patient, but which, instead of being, like suggestions, experimentally called forth, are formed naturally under the influence of accidental causes. This difference in the artificial or natural provocation of automatic phenomena has, from a clinical and especially therapeutic point of view, quite grave consequences to justify this distinction. Ideas of this kind have been described at length in the case of patients considered as lunatics. They went under the name of obsessions, impulsions, phobias; they characterise the delirium which develops with some neurasthenics or, as they are often called in France, certain degenerates. Putnam, professor of the diseases of the nervous system, asked me to deliver before the students some lectures about pathological psychology. I greatly appreciated this honour, and tried to sum up before the American students some elementary psychological researches about a well-known disease, Hysteria, in order to show them how the study of the mental state of the patient can sometimes be useful to explain many disturbances and to give some unity to apparently discordant symptoms. So the following fifteen lectures were given in the Harvard Medical School between the fifteenth of October and the end of November, 1906. The kind reception these lectures on hysteria have met with encourages us to publish a second edition of this work. It does not seem to us very useful to modify it profoundly, for the interest of a scientific work resides almost always in the date at which it was drawn up, and one should not confusedly mix the ideas of one period with those of another. I only wish to show in a short preface that certain notions set forth in these lectures of 1906 have spread very much since that date and have played a great part in the interpretation of hysteria. Looking back to nineteenth-century psychiatry, Katrien Libbrecht attempts to answer the question: Is there such a thing as a hysterical psychosis or are we dealing with hysteria exhibiting psychotic features? Hysterical Psychosis is divided into three sections. The first part of the book carries the reader back to the second half of the nineteenth century, the heyday of the study of hysteria on the eve of the discovery of psychonanalysis.
