She is unable to give a clear account of her first sensations, but states that she was extremely nervous and that very early she began to have double vision. This was followed by disturbance of gait, so that when she went through a door she was likely to bump on either side. It was not the typical gait of cerebellar ataxia, but was a general ataxia. (100 pounds). The diet had never averaged higher than 70 gm. Protein, 10 gm. Carbohydrate, and 1500 calories. During his entire time at home the patient had carried on his regular business with only six days' total absence on account of colds. This same condition continues today. Glycosuria and aci dosis remain continuously absent. Slight hyperglycemia has been the rule rather than the exception, but it is permissible in a case of this type because the age involves less tendency to progressiveness than in younger persons and children. Inherent non-progressiveness of the condition cannot be assumed as an explanation of the good result or an excuse for the high mortality among adults with Similar or milder cases on excessive diets in this series. We have here a decisive contrast between two periods: first, eight years with active symptoms and extreme downward progress; second, eight years on reduced diet with out symptoms and without downward progress. The case is still severe and the tolerance low. It is known that diabetes is hardest to control in the severe stage. What has been aecom plished for this patient can be accomplished for all patients Of this type, granted fidelity to diet and the absence Of any unusual complications. With milder cases the results are correspondingly easier and better. For typical patients of this age or Older the question of spontaneous downward progress is definitely decided in the negative. The second case was No. 66 in the Rockefeller Institute series. This girl was received for treatment in March, 1916. Excerpt: ...to starvation it was decided to take the child on a forty-eight-hour auto trip, so that he would not see any food except his broth and orange-juice; 6 ounces of broth were given every three hours and a tablespoonful of orange-juice night and morning. The amusement afiorded by the trip, together with the fact that the child neither saw nor smelled food other than his own, served to lighten the task of diet restriction. At the end of the second twenty-four-hour period the urine amounted to 500 c.c. and upon examination showed no sugar, diatetic acid, or acetone; the child was apparently none the worse for the starvation. His weight was pound less than before the fast was begun. According to various observers a. child at the age of two years requires approximately 80 calories for each kilogram of weight during a twenty-four-hour period, and an attempt was made to provide a diet for this child that would keep the urine sugar, diacetic acid, and acetone free, and provide the largest number of calories. During the third twenty-four-hour period the determination of carbohydrate tolerance was begun by the feeding of 5 per cent. vegetables in the form of cabbage, spinach, and lettuce leaves. The child was less irritable, yet the task of restricting the diet was a most diflicult one. The vegetables were thrice boiled, thereby lessening the amount of carbohydrate taken and increasing the bulk. The danger of causing gastro-intestinal disturbances was lessened by passing the vegetables through a flour sieve, and no intestinal upset was encountered; 150 grams of 5 per cent. vegetables, when thrice boiled, were calculated as having but 3 per cent. of carbohydrate or 5 grams of carbohydate, and each day 5 grams of carbohydrate were added until sugar...
