Remember the sinus pocularis. Well, how will you avoid it? Oil the index finger of the right hand and introduce it into the rectum. After introducing your catheter hold it in the left hand and push it down until you meet the obstruction. Then follow the catheter with the index finger to its point - I mean the index finger in the rectum gently raise it up, apply a little more force with the left hand, and ninety-nine times out of a hundred you will be sur prised to find how easily the instrument enters the bladder. I can say without boasting that I have never failed in this simple operation in my life, and it is sel dom now that I ever draw blood or give the patient much pain. Never try to introduce a catheter into the bladder where the prostate gland is enlarged without having the finger in the rectum to spread the lateral lobes apart and lift the point of the instrument above the sinus pocularis.-ez. Diagnostic errors may cause harm to patients by preventing or delaying appropriate treatment, providing unnecessary or harmful treatment, or resulting in psychological or financial repercussions. The committee concluded that improving the diagnostic process is not only possible, but also represents a moral, professional, and public health imperative. Improving Diagnosis in Health Care, a continuation of the landmark Institute of Medicine reports To Err Is Human (2000) and Crossing the Quality Chasm (2001), finds that diagnosis-and, in particular, the occurrence of diagnostic errorsâ€"has been largely unappreciated in efforts to improve the quality and safety of health care. Without a dedicated focus on improving diagnosis, diagnostic errors will likely worsen as the delivery of health care and the diagnostic process continue to increase in complexity. Just as the diagnostic process is a collaborative activity, improving diagnosis will require collaboration and a widespread commitment to change among health care professionals, health care organizations, patients and their families, researchers, and policy makers.
