Book excerpt: Cerebral palsy is one of the most common causes of childhood disability, and a major cause of severe disability in children. Mothers of adolescents and young adults with severe CP often assume a role exceeding the usual normative activities of parenting in relation to the intensity, complexity, and temporal nature of the family caregiving experience. The aim of this phenomenologic inquiry was to explore the lived experience of maternal caregiving as experienced by mothers caring for adolescents and young adults with severe CP. Furthermore, the study sought to examine the essential aspects of this human experience using the phenomenological contextual processing and analysis method proposed by van Manen (1990). Eleven mothers were interviewed using a semi-structured interview format. One overarching theme, A Different Life, and four inter-related essential themes emerged from the narratives. The overarching theme, A Different Life, was revealed in the mothers' stories of their unexpected roles as the parent of a child with severe disabilities. The first essential theme, Managing an Unexpected Life, was uncovered in the experiential descriptions of a maternal caregiver role that included elements that differed from mothers of typically developing children. The second essential theme, Finding My Own Way, was associated with mothers seeking and finding their own approaches to balancing the demands of caregiving and family life. The third essential theme, Serving as My Child's Voice, was revealed in the advocacy activities on behalf of their child and other community members with disabilities; and the fourth essential theme, Facing an Uncertain Future, was uncovered in experiential data suggesting uncertainty associated with their child's age-triggered transitions in the health care and educational settings, and with the prospect of maternal caregiving in the future. Future research is needed to gain a better understanding of the support needs of mothers caring for adolescents and young adults with severe physical disabilities. This study suggests the need for improvements in the delivery of family-centered health care services for adolescents and young adults with severe CP and their families, and the development of interventions to support and sustain family caregivers over the prolonged course of maternal caregiving.

The phenomenon was elicited utilizing Roy (Roy & Andrews, 1999) adaptation model of nursing as the theoretical framework and phenomenology implementing Colaizzi's (1978) method of data analysis. The themes identified capture the collective experiences of these women. The study examines an altered mothering role that begins with the diagnosis of the child with severe cerebral palsy and the adaptation necessary to care for the child. It describes how the mothers bonded and communicated with their child. Mothers also described how they dealt with multiple burdens, adapted with and without support and the altered family relationships that resulted. There is a description of the intense emotional responses of these mothers to the experience, and the positive aspects of this parenting experience.

Book excerpt: Depression is a widespread condition affecting approximately 7.5 million parents in the U.S. each year and may be putting at least 15 million children at risk for adverse health outcomes. Based on evidentiary studies, major depression in either parent can interfere with parenting quality and increase the risk of children developing mental, behavioral and social problems. Depression in Parents, Parenting, and Children highlights disparities in the prevalence, identification, treatment, and prevention of parental depression among different sociodemographic populations. It also outlines strategies for effective intervention and identifies the need for a more interdisciplinary approach that takes biological, psychological, behavioral, interpersonal, and social contexts into consideration. A major challenge to the effective management of parental depression is developing a treatment and prevention strategy that can be introduced within a two-generation framework, conducive for parents and their children. Thus far, both the federal and state response to the problem has been fragmented, poorly funded, and lacking proper oversight. This study examines options for widespread implementation of best practices as well as strategies that can be effective in diverse service settings for diverse populations of children and their families. The delivery of adequate screening and successful detection and treatment of a depressive illness and prevention of its effects on parenting and the health of children is a formidable challenge to modern health care systems. This study offers seven solid recommendations designed to increase awareness about and remove barriers to care for both the depressed adult and prevention of effects in the child. The report will be of particular interest to federal health officers, mental and behavioral health providers in diverse parts of health care delivery systems, health policy staff, state legislators, and the general public.

Book excerpt: Oftentimes, parents describe their children as a gift or a blessing, but when a child is born with a disability, the gift or the blessing becomes a challenge physically, emotionally, psychologically, spiritually, and financially. Disability is a condition of being unable to perform due to physical and mental unfitness. One of the most common disabilities that affect a child is Cerebral Palsy. Cerebral Palsy is a non-specific term applied to disorders characterized by early onset of impaired movement and posture. It is non-progressive and maybe accompanied by perceptual problems, language deficits and intellectual impairement (Hockenberry and Eaton, 2007). This qualitative-phenomenological study described the lived experiences of Filipino mothers taking care of their child with Cerebral Palsy. Using a purposive sampling, seven (7) mothers were interviewed in-depth in a natural setting. The interviews were audiotaped, transcribed, and analyzed in accordance with the Collaizi's (1978) procedural steps. Results of the study emerged four (4) major themes from the data. These are (1) Learning About the Diagnosis: The Start of the Journey, (2) Complexities of Child Care: There's Always Something to Deal and Overcome, (3) Intertwining Lives: Learning Through Living and (4) Pillars of Acceptance: The Role of the Support System. Filipino mothers reported successive hardships and challenges in caring for their special child. Despite these experiences, these mothers were able to manage as they poured their unconditional love to their special children, strong faith in God and receiving assistance from varied support system. Findings in this study will contribute to a better understanding on the Filipino mothers' experiences in taking care of their child cerebral palsy based on their rich descriptions. The result of this study will provide additional and significant inputs to the mothers of children with cerebral palsy, children with cerebral palsy, health care workers in the institutions, nursing education, nursing administration and research.

Preterm birth is a cluster of problems with a set of overlapping factors of influence. Its causes may include individual-level behavioral and psychosocial factors, sociodemographic and neighborhood characteristics, environmental exposure, medical conditions, infertility treatments, and biological factors. Many of these factors co-occur, particularly in those who are socioeconomically disadvantaged or who are members of racial and ethnic minority groups. While advances in perinatal and neonatal care have improved survival for preterm infants, those infants who do survive have a greater risk than infants born at term for developmental disabilities, health problems, and poor growth. The birth of a preterm infant can also bring considerable emotional and economic costs to families and have implications for public-sector services, such as health insurance, educational, and other social support systems. Preterm Birth assesses the problem with respect to both its causes and outcomes. This book addresses the need for research involving clinical, basic, behavioral, and social science disciplines. By defining and addressing the health and economic consequences of premature birth, this book will be of particular interest to health care professionals, public health officials, policy makers, professional associations and clinical, basic, behavioral, and social science researchers.
