Book excerpt: Introduction: There is a growing demand for esthetic appliances during orthodontic treatment, with a specific interest in WildSmiles shaped brackets amongst children and adolescents. Bond failure of brackets over the course of orthodontic treatment can result in increased treatment time, increased overhead costs, and feelings of frustration for the patient and practitioner. Objective: The purpose of this study was to the evaluate the effect of orthodontic bracket base shape on shear bond strength (SBS) to human enamel, in vitro, at two time points, as well as the nature of debonding fractures using the Adhesive Remnant Index (ARI). Methods: Orthodontic brackets with different shaped bases (flower, heart, rectangular) were bonded to 120 extracted human third molars. SBS was measured using a Servohydraulic Test System at 24 hours and 2 months after initial bonding. ARI scores were evaluated under 10x magnification to assess the amount of resin left on the tooth. Results: The control bracket with a rectangular base shape had the highest mean SBS (26.78 ± 8.19 MPa), and significantly differed from flower (17.19 ± 4.45 MPa) and heart (18.86 ± 3.50 MPa) base shapes (p 0.001). The mean SBS of flower and heart shaped brackets did not differ considerably from each other (p 0.05). The mean SBS between debond times at 24 hours (21.50 ± 7.46 MPa) and 2 months (20.38 ± 6.69 MPa) were not statistically significant (p > 0.05). Analysis of ARI scores showed a significant difference between Flower-24 hours vs Heart- 2 months (p=0.039), Flower-24 hours vs Heart- 24 hours (p=0.004), and Control-2 months vs Heart-24 hours (p=0.015). The heart shaped bracket had a greater proportion of adhesive bond failures at the enamel-resin interface. Conclusion: Bracket base shape has an effect on SBS, with the rectangular base shape having a higher mean SBS compared to flower and heart base shapes. All bracket shapes had bond strengths above the clinically acceptable range of 6-8 MPa, and thus may provide adequate SBS in a clinical situation. Variations in ARI scores occurred based on bracket shape and were of a mixed adhesive-cohesive nature.

Current literature on the effects of thermocycling on bond strength is equivocal. This is largely due to a lack of a universally accepted standardized technique to examine the effects of thermocycling on orthodontic bond strength. As a result, differences in testing methodology between laboratories and investigators within the same laboratory make it difficult to directly compare data from thermocycling studies. The aims of this study were (1) to show that the testing method developed at the University of Toronto is independent of the investigator; (2) to determine the effect of thermocycling on the shear bond strength of orthodontic brackets bonded to bovine enamel; (3) to determine the effect of long term storage on the mean shear bond strength of orthodontic brackets bonded to bovine enamel; (4) to examine the bond failure interface in order to determine if a relationship exists between thermocycling and the location of the cement after debonding; (5) to examine the bond failure interface in order to determine if a relationship exists between long term storage and the location of the cement after debonding. Book excerpt: Objective: This study evaluated the effect of primer protocol for metal orthodontic brackets bonded to zirconia ceramic and resulting shear bond strength (SBS), adhesive remnant index (ARI), and debonded and polished zirconia surface roughness. Methods: Zirconia specimens were milled, glazed, embedded in acrylic resin and the target area microetched with aluminum oxide. Glazed and microetched representative specimens were evaluated with energy dispersive spectroscopy (EDS). Specimens were randomly assigned to primer protocol, enamel (TransbondTM Plus Self-Etching Primer) or zirconia (Assure® Plus), and primed according to manufacturer instructions. Metal orthodontic brackets were bonded to the primed surface with resin adhesive (Transbond XTTM) and light cured. To simulate intraoral conditions, all primer and bonding protocols took place in an environmental chamber set at 33±2°C and 75±5% humidity followed by a 24-hour dark cure at 37 C. Following SBS testing, an ARI value was assigned to debonded brackets. Residual adhesive was removed from the debonded zirconia specimens, the zirconia surface polished, and surface roughness measured using a profilometer. Their main advantage is that they reduce the number of steps required in the bonding of orthodontic brackets. However, their low ph was a cause of concern. This book describes a study undertaken to see the effects of ph of self-etchant on shear bond strength of orthodontic brackets. These shear bond strengths were also compared to those obtained with a conventional 3-step bonding system. It was found that self-etching primers not only reduce the chair time in an orthodontist's clinic but also reduce the amount of enamel lost during etching. The bond strength achieved was less than that of the conventional bonding system but was acceptable for orthodontic purposes. Thus self-etching primers are an important and useful invention for simplifying the most critical step in orthodontic clinic- bonding.

Book excerpt: The purpose of this study was to compare the shear bond strength of orthodontic brackets to enamel in four bonding protocols: SEP without prior pumicing (None), Pre-etch and SEP without prior pumicing (Pre-etch), control: SEP with prior pumicing (Pumice), and Pre-etch and SEP with prior pumicing (Both). 80 extracted bovine incisors were randomly divided into 4 groups of 20, and brackets were bonded under the different experimental conditions. Debonding force was measured with an Instron universal testing machine. A two-way ANOVA comparing the four groups indicated that there was a significant difference in debonding force (P = 0.001). The SEP without prior pumicing group (17.69 " 7.18 MPa) was statistically different from the SEP group with prior pumicing (25.82 " 6.84 MPa). There was no statistical difference found among the other groups. Differences in the Adhesive Remnant Index (ARI) were analyzed by chi-square. ARI scores differed significantly (P =0.0048).

Book excerpt: The purpose of this investigation was to evaluate the effects of air-powder polishing on shear bond strength in vitro for two metal bracket adhesive systems currently in use for direct orthodontic bonding procedures. Ninety-six third molar teeth with flattened enamel surfaces were randomly assigned to be bonded with metal brackets using either a two paste or a no-mix orthodontic adhesive resin. Thwelve samples in each treatment group were air-powder polished for 0, 15, 30, or 60 seconds. Each bracket adhesive bond was then tested for shear strength using the Instron Universal Testing Device. Scanning electron microscopy determined the fracture pattern of representative samples from each treatment group. Mean shear bond strength values for each treatment group were calculated and expressed in magapascals (MPa). Values from baseline to 60 seconds varied from 22.9 ± 1.9 MPa to 18.2 ± 4.1 MPa for the two paste system and from 15.5 ± 2.1 MPa to 14.6 ± 1.9 MPa for the no-mix adhesive. Differences between the adhesive systems at all air-powder polishing times were statistically significant (p d".05). Statistical analysis by a two-way ANOVA in combination with the Newman-Keuls Multiple Range Test for pairwise mean comparisons determined a differential effect of air-powder polishing on the bond strength of the two adhesive systems. After 60 seconds of air-powder polishing, results showed a significant decrease (p d".05) in the mean bond strength of the two paste adhesive resin when compared to baseline, 15, and 30 second treatments. No significant time effect of air-powder polishing was found for the no-mix adhesive system. SEM analysis of the bond failure sites showed tooth-adhesive, within adhesive and bracket-adhesive fracture patterns for the viewed samples of each adhesive type and air-powder polishing treatment time. Fractures within tooth structure were also noted for the two paste system, indicating strong enamel-adhesive-bracket bonds. Differences in the fracture pattern of the two paste 60 second air-powder polishing group may account for the differential effect of treatment. Although in vitro results showed decreased bond strength for the two paste system, use of air-powder polishing for the routine maintenance of the orthodontic patient should not affect clinical performance of this adhesive.
