Care Without Coverage examines the real consequences for adults who lack health insurance. The study presents findings in the areas of prevention and screening, cancer, chronic illness, hospital-based care, and general health status. The committee looked at the consequences of being uninsured for people suffering from cancer, diabetes, HIV infection and AIDS, heart and kidney disease, mental illness, traumatic injuries, and heart attacks. It focused on the roughly 30 million-one in seven-working-age Americans without health insurance. This group does not include the population over 65 that is covered by Medicare or the nearly 10 million children who are uninsured in this country. The main findings of the report are that working-age Americans without health insurance are more likely to receive too little medical care and receive it too late; be sicker and die sooner; and receive poorer care when they are in the hospital, even for acute situations like a motor vehicle crash.

Book excerpt: Abstract: Little is known about what predicts longer intervals between diagnosis and treatment (initial treatment period) in breast cancer care, and what effects longer intervals of this kind have on the human and economic costs of cancer, especially in low-income populations. This study examined predictors of longer initial treatment periods and assessed the effects of longer initial treatment periods on overall and breast cancer-specific survival, and direct medical costs from the perspective of the North Carolina (NC) Medicaid system. A multilevel framework was used to characterize the entire project within the context of the breast cancer care continuum. Patients were NC Medicaid enrollees diagnosed with breast cancer in the linked NC Central Cancer Registry-Medicaid Claims database. The study population consisted of 1,786 women with mean age of 61.6 years. Median time from biopsy-confirmed diagnosis to treatment initiation was 22.0 days (d). Ordinal logistic regression analysis revealed that, compared to white women, non-white women were 1.30 (95%CI: 1.07, 1.58) times as likely to have longer initial treatment periods (>=30d vs. 0-29d and>=60d vs. 0-59d; hereafter referred to as odds of waiting at least 30d). Moreover, women with hormone receptor (HR) positive (vs. HR- ) breast cancer had a 38% increase in odds of having initial treatment periods at least 30d (p=0.024). Diagnosis at later cancer stage was inversely proportional to odds of longer initial treatment periods with women diagnosed at local, regional, and distant stage having 41%, 46%, and 71% reduction in odds of having longer initial treatment periods (p0.001). Women who had a disability or received home health care were also significantly less likely to have times from diagnosis to treatment that were at least 30d. Adjusted Cox proportional hazards regression showed that although initial treatment period length did not affect survival among those diagnosed at early stage, among late-stage patients, intervals between diagnosis and first treatment=60d were associated with worse overall (HR=1.66, 95%CI: 0.99, 2.77) and breast cancer-specific survival (HR=1.90, 95%CI: 1.06, 3.41). Log-transformed ordinary least squares regression showed that initial treatment period length did not affect direct total, inpatient, outpatient, or prescription drug Medicaid costs in the year following diagnosis. Factors that did impact direct costs typically reflected more severe disease, more comorbidites, any disability, and the use of more expensive treatments. Most Medicaid patients received timely breast cancer treatment after diagnosis, however, one-in-ten women waited at least 60d. Intervals of this length, though unrelated to direct medical costs, significantly worsened survival among late-stage patients. Moreover, results suggest that physicians and/or patients may be prioritizing quick treatment for aggressive cancer cases, but that non-white patients have disproportionately longer initial treatment periods even after adjustment for stage and other patient and tumor-specific characteristics. Patients who received home health care had shorter initial treatment periods and lower costs, thus the potential for navigators who work to reduce barriers and facilitate timely breast cancer treatment should be considered as a viable avenue to improve outcomes and lower costs in subpopulations at risk for long delays in obtaining treatment.

It has been proven to detect breast cancer at an earlier stage and to reduce the number of women dying from the disease. However, it has a number of limitations. These current limitations in early breast cancer detection technology are driving a surge of new technological developments, from modifications of x-ray mammography such as computer programs that can indicate suspicious areas, to newer methods of detection such as magnetic resonance imaging (MRI) or biochemical tests on breast fluids. To explore the merits and drawbacks of these new breast cancer detection techniques, the Institute of Medicine of the National Academy of Sciences convened a committee of experts. During its year of operation, the committee examined the peer-reviewed literature, consulted with other experts in the field, and held two public workshops. In addition to identifying promising new technologies for early detection, the committee explored potential barriers that might prevent the development of new detection methods and their common usage. Such barriers could include lack of funding from agencies that support research and lack of investment in the commercial sector; complicated, inconsistent, or unpredictable federal regulations; inadequate insurance reimbursement; and limited access to or unacceptability of breast cancer detection technology for women and their doctors. Based on the findings of their study, the committee prepared a report entitled Mammography and Beyond: Developing Technology for Early Detection of Breast Cancer, which was published in the spring of 2001. This is a non-technical summary of that report.
