Book excerpt: This project was designed and executed to investigate the potential correlation between increased nursing knowledge on pain management and effective pain control in the acute care setting. The process included creating a convenience sample of ED nurses at one community based hospital to receive directed education on pain and pain management. These volunteer nurses were given a pretest in the form of the Knowledge and Attitudes Survey Regarding Pain (KASPR) to assess baseline knowledge. Concurrently, a retrospective chart review was conducted to record patient reported pain values from triage and discharge. Next, an educational program was conducted to provide increased awareness on various topics related to pain management. When all nurses within the sample had completed the education, a post test was conducted again utilizing the KASPR instrument. Lastly a second retrospective chart review was conducted to collect post education data on pain scores in triage and discharge. Analysis looked at the mean change in pain related to the mean change in nursing knowledge. Using Spearman's rho the results showed a complete lack of correlation between nursing education and changes in pain with an r(8) = .08 on a scale of -1 to 1 and a significance level of .826 with a set p > .05.

Book excerpt: Pain management in the emergency room continues to be problematic causing frustration for patients, nurses, and physicians. Emergency rooms are accustomed to patients presenting with pain yet 70% of those that present with pain feel that their pain is not treated adequately(Bergman, 2012). If nurses that are working in the emergency room take classes on pain management they will be better prepared to care for patients that present with pain. Once the nurses have received education in pain management patients pain scores should decrease by the time of discharge and their satisfaction with the treatment of their pain in the emergency room will increase. A study conducted by Cerns, Mueller, and Glatzel showed that nurses scores on a knowledge-based test were significantly improved after receiving education and an increase inpatients satisfaction was documented (Cerns, Mueller, and Glatzel, 2009). A trial will be conducted using ten nurses that currently work in the emergency room at Monroe Clinic, five of their patients will be asked their pain scores on admit and discharge from the emergency room before the nurse takes a class in pain management and five additional patients will be asked after the nurse has completed the class. Approval for this trial will be obtained from the director of the emergency room, emergency room coaches, emergency room council, and upper management. The results of this trial should prove that patients rate their pain score lower on discharge when they are cared for by a nurse that has received education in pain management. The results will be presented to the key stakeholders and fellow nurses, if patients pain scores are improved and patient surveys show an increased satisfaction with pain management all nurses will be asked to complete training in pain management.

Participants consisted of 29 registered nurses that currently work in pediatric emergency departments. Participants completed an online survey that assessed participants' perceptions of the child life specialist role, use of nonpharmacological pain management, and their knowledge and attitudes regarding pediatric pain and pain management. Results revealed that pediatric nurses' knowledge and attitudes toward pain management in children varied by level of education and amount of experience working in pediatrics. Findings also suggest that most nurses recognize the significant impact that child life specialists have on the psychosocial wellbeing of patients and families but might not recognize fully how child life interventions can improve the physical well-being of patients. Study implications for practice are discussed.

Book excerpt: According to the research reviewed pain is a predominant presenting diagnosis to the emergency department (ED), estimating that at least 75% of patients present to the ED with a chief complaint related to pain (Downey and Zun, 2010, pages 326). In a study about patients' satisfaction a significant number of patients (between 20-50%) were not satisfied with the care they received for pain in the ED (Downey and Zun, 2010, pages 326). Patient satisfaction is beginning to impact facilities two different ways. First it makes the facility look bad or undesirable when patients have a bad experience and then report that experience in a survey. Patients are now consumers that can shop for health care with the use of sites like Health Grades, where any doctor of facility is searchable and their performance is graded. Facilities need to be concerned about keeping patients happy or it can mean a loss of business. Secondly dissatisfied patients get patient survey's, like the HCAPS. These survey directly impact the reimbursement that facilities get from Medicare. To address the issue we must start with what we can do reasonable. We have to have a starting point and that starting point should be to get everyone on the same page. How do we do that? We start with education for staff and for the patients. Part of the issue is that the perceptions of how to manage pain and what is acceptable, differ. Most patients may not really know what to expect and staff may also have prejudices or preconceived ideas that present a barrier. "Patients in this study had high expectations for pain relief. In fact, almost half of the patients expected complete relief of their pain" (Yee, Puntillo, and Neighbor, 2006, pages 285). Education is the way to bridge these gaps and serves a starting point to fix this overwhelmingly large issue. Patients want great care and clinicians want to give great care. For any of that to happen facilities have to operate in the black. If facilities are not making money they combine and consolidate, they cut resources and the care suffers. The bottom line is that the bottom line matters, therefor so should pain management. A staff and patient directed education is the best way to achieve better pain management in the ED setting. Beginning with a mandatory pain management course for all ED staff with a pre and post education survey. Implementation of skills and teaching points would begin with patients when all staff education was complete. Patient would be given an accompanying flyer for their review and would asked exit questions regarding their experience. The simple interventions could have the potential to transform pain management and significantly improve patient satisfaction.

The incidence of pain has remains high with an estimated 75% of patients reporting moderate to severe pain in the acute setting, this has remained unchanged since the 1950's. Throughout the literature it is felt that this is largely due to a lack of knowledge and misconceptions regarding assessment and management of pain by all healthcare professionals. A modified version of McCafferty & Ferrell Knowledge and Attitudes survey was used to evaluate nursing staff current level of knowledge and attitudes towards pain assessment and management. A baseline audit was performed and following the results a bespoke education plan was delivered to registered nurses across the District General Hospital. The survey was then repeated to evaluate the impact of the educational interventions. 58 surveys were completed in the base line audit and a further 65 were completed following educational interventions. The mean knowledge score increased from 73% to 76% following educational interventions, individual question analysis also reviled a significant improvement across questions relating to pharmacology, pain assessment and belief in patient's report of pain improved. This audit has allowed for the identification of knowledge gaps and following educational intervention it was possible to change and challenge long held misconceptions regarding pain assessment and management. Even though there was a small improvement in mean knowledge score there was a significant improved seen in individual question analysis.

Book excerpt: ABSTRACT: The purpose of this mixed methods study was to describe the cognitive processes/knowledge sources used by Emergency Department (ED) nurses in decision-making activities regarding triage and pediatric pain assessment and management. Deficiencies persist in ED pediatric pain assessment, and management methods or approaches that might help resolve these deficiencies have not been identified previously. Methodology triangulation with sequential use of qualitative- quantitative methods provided a rich description of knowledge sources and cognitive processes used by ED nurses relative to pediatric pain assessment decisions. Based on qualitative results, a set of vignettes was developed to assess ED nurses. Data analysis using ordinal logistic regression with a cumulative logit model identified patient and nurse variables which influence triage acuity decisions. Five common themes emerged from the qualitative data; 1) Age of the child is important, 2) Behavior can tell a lot, 3) Really looking at the patient, 4) Things that help make decisions, and 5) Things that hinder decisions. Ordinal logistic regression analysis of the quantitative data identified predictor variables of infants compared to school-age children, Hispanic ethnicity, moderate number of years of ED experience (11 -20 years) and years of education that were associated with higher triage levels .The implications of this new knowledge include changes in ED triage nurse practice towards pain assessment, and increased awareness of the need for education in use of pain assessment tools. Additional implications include education related to pain management practices by ED physicians and pain medication protocols at triage. This information may enhance triage and care of the pediatric patient experiencing pain, expand the knowledge base of emergency nursing, identify areas in which to implement changes, assist in improving care provided to children experiencing pain, and provide direction for future education, training, and research.
