Available in PDF, EPUB and Kindle. Book excerpt: The purpose of this descriptive study was to examine students' perceived confidence to provide culturally competent care. The literature showed a lack of empirical studies investigating the confidence of nurses and nursing students in providing culturally competent care. Therefore, the research question was, "What are Southwest Missouri senior baccalaureate student nurses' self-perceptions of their competence in providing cultural care for diverse ethnic clients?" A convenience sample of baccalaureate nursing students, enrolled during the fall 2009 semester, from two universities in Southwestern Missouri were recruited for this study. Students completed the Bernal and Froman's Cultural Self-efficacy Scale (CSES). This study found that the participants' self-perceived cultural knowledge, concepts, patterns, and transcultural nursing skills ranged between 3 (neutral) and 4 (moderate confidence) on the Likert scale 1 to 5. Based on the computed subscales of the CSES, the score of the total self-efficacy was 194.53, which means the participants rated themselves as neutral to moderately competent. The study found that there is a minimal increase in the self-perceived confidence levels of cultural competence compared to other studies in the Midwest.

As a result, these demographic trends pose particular challenges to the effective delivery of health care and the elimination of health care disparities. Thus, the need for improved foundational knowledge, skills and abilities in cultural competence is universal in nursing programs; however, the nursing students' experience of cultural competence in training is missing from research literature. The purpose for this case study is to better understand student experiences with cultural competence in training, give voice to these experiences, and develop recommendations regarding cultural competence in nursing education from themes that emerged a priori and after focus group and individual interviews. The research question is as follows: What are nursing students' experiences with cultural competence during their pre-professional training? In order to answer this research question, an instrumental case study is the culture of inquiry used for this study. Data was collected via focus group and individual interviews, transcribed, and thematically coded. Five themes emerged and are as follows: 1) Cultural Competence is difficult to teach, 2) Awareness is an integral part of Cultural Competence, 3) Respect is an integral part of Cultural Competence, 4) Cultural Competence is learned through Experience, 5)Cultural Competence is an on-going process. Throughout the study, participants resoundingly cited cultural encounters, exposure, and interaction as sources by which they developed clinical skills and increased understanding of cultures different from their own. Further, the overlap among the global themes and consistency with research literature connotes the idea that cultural competence is a dynamic process that is personal, interpersonal, and fueled by experiential learning and open dialogue. Thus, being a culturally competent nurse encompasses a cyclical pattern of education, awareness, respect, and experience that likely reemerges throughout one's career trajectory.

Book excerpt: Cultural competency is a crucial component of baccalaureate nursing education to support patient-centered practice (Calvillo, Clark, Ballantyne, Pacquiao, Purnell, & Villarruel 2009). Studying abroad is associated with short-term gains in students' cultural competency; however, little research has addressed whether cultural competence is affected long-term, particularly when the study abroad experience is relatively short (Phillips, Bloom, Gainey, & Chiocca, in press). Therefore, the question that drove this study was: what are the long-term effects of a short-term study abroad experience on baccalaureate nursing students? The participants in this study were practicing nurses (N=21) who had traveled to Ghana for two-week study abroad experiences as senior nursing students between 2011 and 2015. Qualitative data (collected immediately post-travel) with these participants revealed four key themes of transformation consistent with cultural transformation: adaptability, cultural competency, understanding of social determinants of health, and mutual partnerships with patients. Social media was used to enroll participants in a study conducted over a one-month period in Fall 2016. Of the 45 contacted, 28 responded and consented to participate and 21 completed data collection. A series of Likert-style questions (5=Strongly Agree, 1=Strongly Disagree) were used to measure these nurses' perceived immediate post-travel change, versus their perceptions of the sustained changes over time. Results supported that the four key themes of cultural transformation persisted over time. Creating mutual partnerships and understanding social determinants of health had the highest mean score and lowest standard deviation (mean= 4.76, standard deviation= 0.44).

Book excerpt: It has been decades since the concept of cultural competency was introduced in the literature, yet nursing faculty still show low levels of cultural competence. The purpose of this study was to assess the level of cultural competence among nursing faculty teaching in collegiate schools of nursing in the U.S. and to identify and examine demographic factors that are associated with the cultural competence level of nursing faculty. The study design was a descriptive, correlational, non-experimental survey design with a sample of nursing faculty (n=461) teaching in Commission on Collegiate Nursing Education (CCNE) accredited baccalaureate nursing programs in the U.S. Data were collected using "Cultural Diversity Questionnaire for Nurse Educators Revised" CDQNE-R. The guided theoretical framework of this study was Campinha-Bacote's (2010) Process of Cultural Competence in the Delivery of Healthcare Services Model. Statistical analysis was done with SAS 9.3© (SAS Institute Inc., Cary, NC). The overall cultural competence (OCC) mean score for the study sample =166.3 ± 19.5 indicating that nursing faculty who teach at BSN programs across the U.S. have a moderate level of cultural competence. The highest indexes were for the Cultural Knowledge Subscale M= 43.53 ± 6.2, and the Cultural Teaching Behaviors Subscale M= 42.10 ± 4.98. The low indexes were for the Cultural Skills Subscale M= 31.53 ± 4.41, and the Cultural Encounter Subscale M =22.50 ± 4.42. Regression analysis found that selected demographic variables were predictive of OCC mean score. These variables included residing in a different country, language other than English, nursing specialty, highest degree attained, cultural continuing education, including cultural content in current nursing program, level of incorporating cultural content in current program, and assessing student needs and were significantly predictive of OCC mean score after adjusting for race, age group, and gender. The study also found that the transcultural teaching behaviors subscale was significantly associated with the OCC scale, adjusting for all other variables. The explained variance for the models ranged from 39% to 86%. Future studies are needed to identify factors that contribute to cultural competence from faculty and student perspectives; to identify the best-standardized evaluation tool to assess students' cultural competence levels; and to identify the best educational strategies to teach cultural competency in nursing education programs.
