Book excerpt: Background: HPV vaccine uptake is low among East African-American (EAA) adolescents in the U.S. To improve HPV vaccine uptake in EAA communities, we developed a theory-based, community-informed comic book for adolescents as a part of an intervention targeting mothers and adolescents. We evaluated the impact on participating adolescents' HPV and HPV vaccine knowledge, attitudes and vaccination intentions. Methods: The culture-specific comic book intervention was delivered to unvaccinated EAA adolescents who participated in ethno-linguistic-specific dinners with their mothers in Washington (8 dinners in Somali, 2 in Amharic) between 2017-2018. English-speaking adolescents aged 14-17 were assigned to complete a self-administered survey either before or after reading the comic book. Surveys were identical and assessed knowledge, barriers, communication with parents, and intention. Responses between adolescents completing the pre- vs post-test were compared with chi-squared tests. We used generalized estimating equation (GEE) models to estimate relative risks (RRs) and 95% confidence intervals (CIs) to measure the magnitude of the impact of the intervention on each construct. We also stratified the GEE models by gender to enumerate gender-specific intervention effects and tested for the presence of interaction between the participants' gender and the assigned survey group (e.g., pre- or post-intervention). Results: Among 136 (Pre-test=64, Post-test=72) participants, 90% were Somali and 54% female. Adolescents assigned to the post-test were more likely to correctly answer questions on HPV/HPV-vaccine knowledge than those assigned to the pre-test (correct responses ranged from 27%-67% on the pre-test vs. 66%-90% post-test). We describe the development and production of a theory-based, audience centered HPV vaccine comic book for East African adolescents. We held three focus groups with East African immigrant mothers in King County, Washington (n=30) to identify themes and suggestions to inform comic book production. We conducted message mapping to bridge focus group themes, theoretical constructs, message objective, and message appeal. Finally, we embedded the theory-based messages into the story script and gathered input from community members on cultural appropriateness. The comic book incorporated emerging themes from the focus groups including mothers’ preference that comic book characters resemble members of their community; resonance with the social connections between the adolescent main character and her social network including adolescent-teacher, adolescent-peers, adolescent-mother, and adolescent-grandmother; integration of emotion through humor; and information needs. The story highlighted information about HPV infection and transmission, vaccine benefits, and risks. The story consisted of three scenes: 1) adolescent HPV vaccine education by a health professional at school; 2) discussion of HPV vaccine beliefs and experiences among a group of adolescent peers; and 3) communication with their mothers about HPV vaccine and intent for vaccination. Through this formative research process we identified suggestions linked to larger established themes regarding HPV vaccine and used them to produce a comic book for East African adolescents. Ongoing testing will assess acceptability and impact of the comic book on HPV vaccine knowledge, vaccination intention, and vaccine uptake.

In the United States (US), young adults, especially college students, are mostly affected. HPV vaccine was approved in the US since 2006. They are safe and effective, but coverage remains lower than other vaccines for young adults, implying missed vaccination chances, and a need to improve HPV vaccination promotion strategies. Comics are potentially effective for health education of diverse groups, because they are easily accessible, low-cost, engaging, and unobtrusive. They may help to improve knowledge and beliefs regarding HPV vaccine, increase its uptake, and decrease HPV sequelae.MethodsWe conducted a mixed methods study to examine predictors of intention to complete HPV vaccination among college students in northeast Ohio. We developed an educational HPV/HPV vaccine comic book for college students informed by evidence from current target population-based studies on HPV vaccination, pilot testing, and the Integrated Behavior Model (IBM). We hypothesized that the intervention would improve their HPV vaccine knowledge, beliefs, attitude, perceived norm, personal agency, and intention to complete HPV vaccination within 12 months. Using a quasi-experimental pretest/posttest survey design, we evaluated the effect of the intervention on a diverse population of 18 to 26-year-old male and female students recruited from a college in northeast Ohio in 2017 to review the comic book. ResultsA majority of participants, n=314, males (28%), females (72%) were satisfied with the comic and reported it is an acceptable way to get HPV vaccine information, easy to read, culturally relevant, with good quality information and graphics. Only 29% reported they have completed the recommended three doses of HPV vaccine. Multiple linear regression (n=157) showed age (ß ^=-0.11*); race (ß ^=0.81*); instrumental attitudes (ß ^=0.43*); injunctive norms (ß ^=0.20*); and descriptive norm (ß ^=0.55**) were significant predictors of intention to complete HPV vaccination in the next 12 months [R2=0.47, F (8) =16.12, p

Book excerpt: Objective: To determine how psychosocial factors, including knowledge, self-efficacy, and communication, predict intention to receive the HPV vaccination and actual vaccination uptake. Methods: A secondary data analysis of African American adolescent females (n=200) aged 12 to 18 years in Philadelphia, PA who participated in an educational intervention program called 'HPV Vaccination of Underserved Adolescents and Young Women in Pennsylvania.' Baseline survey data was used to assess psychosocial factors and intention to receive vaccine. Uptake of HPV vaccination was assessed through linkage to the Philadelphia KIDS immunization registry. Results: In this population of urban adolescents, 44 % had high intention to vaccinate, 36.9% received the first HPV vaccination and 20.7% received both the first and second vaccination out of a three dose series. Self-efficacy and communication with parents and health care providers were associated with intention to vaccinate. Compared to those with low self-efficacy, participants with moderate or high efficacy scores had increased odds of intention to vaccinate (OR=5.01, 95% CI 1.84-13.64 and OR=28.20, 95% 9.48 - 83.86, respectively) and those with moderate to high communication scores have an increased odds for intention to vaccinate compared to those with low communication scores (OR= 3.48, 95% 1.52-7.97 and OR=9.62, 95% 4.04-22.93). Knowledge of the HPV virus and self-efficacy was significantly associated with receiving the first HPV vaccination, whereas only knowledge of the HPV virus was associated with receipt the first and second vaccination. Intention to receive HPV vaccine was not associated with receipt of vaccine. Conclusions: Future health educational interventions directed towards urban adolescents should focus on increasing participant's self-efficacy to receive the HPV vaccination along with increasing their knowledge of the vaccine itself.

Book excerpt: "The human papillomavirus (HPV) vaccine is a research breakthrough to prevent HPV-related cancers and morbidity. Despite assured vaccine efficacy and safety, adolescent vaccine receipt remains suboptimal. At a Convenient Care clinic in Central New Jersey, several missed clinical opportunities to offer HPV vaccination to adolescent patients were observed due to poor provider vaccine recommendations. An evidence-based practice change project was implemented to improve vaccine recommendations practices among providers. The intervention involved an individualized educational program focusing on HPV vaccine as a method of cancer prevention. A validated tool, the HPV knowledge and attitudes survey (HKAS), was utilized to assess provider knowledge of HPV disease, HPV vaccination and vaccine recommendation intent. Nurse Practitioner providers (n=29) were recruited, and all participants completed a pretest, posttest and knowledge retention survey. Knowledge retention was measured four weeks after intervention with the same tool. Using comparative means analysis, intended project outcomes were determined to be met based on selected benchmarks. Findings revealed an 18% increase in provider HPV disease knowledge, a 19% increase in HPV vaccine knowledge and a 33% higher likelihood to recommend HPV vaccination post intervention. Retention scores at 4 weeks post-intervention demonstrated a 1.12% knowledge increase. Therefore, providers retained knowledge gained. The educational programs directed at increasing providers HPV knowledge and promoting vaccine recommendation practices was an effective approach to closing the missed clinical opportunities to recommend HPV vaccination. Findings from this project support that widespread provider HPV education is warranted as a step towards improving the low adolescent vaccination rates. Book excerpt: Human Papillomavirus (HPV) causes nearly all cervical cancers, 90% of anal cancers, and 60-70% of penile, vaginal, valvular, and oropharyngeal cancers (CDC, 2020). Hispanics have the highest cervical cancer incidence rate of any minority group and a mortality rate 26% higher than Whites (CDC, 2021; American Cancer Society, 2018). HPV vaccination completion rates in the Hispanic population (62.7% of adolescents aged 13 to 17) are much below Healthy People 2030's goal of 80% of 13- to 15-year-olds up to date with HPV vaccinations (Healthy People 2030, n.d.). There is a lack of culturally appropriate interventions to increase HPV vaccine uptake. Prior studies suggest that promotora led education is an effective strategy to increase health behavior activities. The purpose of this study is to determine the effect of a culturally appropriate educational intervention, promotora delivered education, on HPV and HPV vaccine knowledge and intention to vaccinate with the HPV vaccine in Hispanics. The dependent variables are HPV and HPV vaccine knowledge level and intention to vaccinate. This study is significant to advanced practice nursing because providing culturally appropriate education and interventions will help combat ethnic and racial health disparities to ultimately decrease the incidence and mortality rates of HPV related cancers in Hispanics.

Evidence indicates that about 79 million people are affected in the United States. Every year, about 14 million Americans are infected with HPV (CDC, 2016c). It is associated with multiple diseases such as cervical cancer, vaginal and vulvar cancers, anal cancers, throat cancer and penile cancers. A safe and efficient vaccine has been approved for both boys and girls. The Center for Disease Control and Prevention announced that teens and young adult under age 26 are at a particular risk for HPV. It is essential to start the vaccination series between 9 and 17 years of age. Parental and caregivers acceptance of HPV vaccine for their children has been a problem. Despite, the HPV vaccine promotions and availability, the vaccination rates of HPV continue to low. The focus should be on parents and caregivers because parental attitudes, beliefs, knowledge, intention to vaccinate and behaviors appear to contribute to low parental acceptance of the vaccine (Gamble et al., 2010). Parental and caregiver education improved parental knowledge, intention to vaccinate and attitude towards HPV vaccination. The purpose of this project was to develop an evidence-based parental and caregiver educational intervention to improve knowledge, intention to vaccinate and attitude towards HPV vaccine, hoping to influence parents and caregivers of children ages 9 to 17 to consent to the HPV vaccine. Parental education as an intervention, focusing on HPV and HPV vaccine was found to have positive impact on parental knowledge, intention to vaccinate and parental attitude toward HPV vaccine. There was a 40%, 73%, and 33% improvement respectively on parental knowledge, attitude, and intention to vaccinate which was higher than the benchmark.
