Book excerpt: Originally published in 1923 Chance and Error examines the vagaries of chance, and how this is the result of the interference of yes and no. The book basis its examination of chance on the idea of a two-sided coin. The book stipulates that contradictories are head and tail, or yes and no. When the coin is flipped in the air yes normally wins half of the trials, but this includes half of the half that normally go to no. Thus, normally in one quarter of the trials there is an interference of yes and no. From this the chance of any number of heads or tails can be easily calculated, and all results that are attained by more difficult mathematics are secured. The book uses this idea to examine interference of yes and no in everyday life and argues that this causes the variations in everything that goes on around us in nature and in our daily life. This book will be of interest to philosophers of logic, as well as mathematicians.

Book excerpt: In Unbelievable Errors, Bart Streumer defends an error theory about all normative judgements: not just moral judgements, but also judgements about reasons for action, judgements about reasons for belief, and instrumental normative judgements. This theory says that these judgements are beliefs that ascribe normative properties, but that these properties do not exist. It therefore entails that all normative judgements are false. Streumer also argues, however, that we cannot believe this error theory. This may seem to be a problem for the theory, but he argues that it is not. Instead, he argues, our inability to believe this error theory makes the theory more likely to be true, since it undermines objections to the theory, it makes it harder to reject the arguments for the theory, and it undermines revisionary alternatives to the theory. Streumer then sketches how certain other philosophical views can be defended in a similar way, and how philosophers should modify their method if there can be true theories that we cannot believe. He concludes that to make philosophical progress, we should sharply distinguish the truth of a theory from our ability to believe it

The diagnostic process is a complex, collaborative activity that involves clinical reasoning and information gathering to determine a patient's health problem. According to Improving Diagnosis in Health Care, diagnostic errors-inaccurate or delayed diagnoses-persist throughout all settings of care and continue to harm an unacceptable number of patients. It is likely that most people will experience at least one diagnostic error in their lifetime, sometimes with devastating consequences. Diagnostic errors may cause harm to patients by preventing or delaying appropriate treatment, providing unnecessary or harmful treatment, or resulting in psychological or financial repercussions. The committee concluded that improving the diagnostic process is not only possible, but also represents a moral, professional, and public health imperative. Improving Diagnosis in Health Care, a continuation of the landmark Institute of Medicine reports To Err Is Human (2000) and Crossing the Quality Chasm (2001), finds that diagnosis-and, in particular, the occurrence of diagnostic errorsâ€"has been largely unappreciated in efforts to improve the quality and safety of health care. In Part I (History), he explores the historical context of the debate, and discusses the moral error theories of David Hume and of some more or less influential twentieth century philosophers, including Axel Hägerström, Bertrand Russell, Ludwig Wittgenstein, and Richard Robinson. He argues that the early cases for moral error theory are suggestive but that they would have been stronger had they included something like J. L. Mackie's arguments that moral properties and facts are metaphysically queer. Part II (Critique) focuses on these arguments. Olson identifies four queerness arguments, concerning supervenience, knowledge, motivation, and irreducible normativity, and goes on to establish that while the first three are not compelling, the fourth has considerable force, especially when combined with debunking explanations of why we tend to believe that there are moral properties and facts when in fact there are none. One conclusion of Part II is that a plausible error theory takes the form of an error theory about irreducible normativity. In Part III (Defence), Olson considers challenges according to which that kind of error theory has problematic ramifications regarding hypothetical reasons, epistemic reasons, and deliberation. He ends his discussion with a consideration of the implications of moral error theory for ordinary moral thought and talk, and for normative theorizing.
