Book excerpt: Excerpt from Diseases of the Heart: Diagnosis and Treatment In writing the following pages, I have been scrupulously careful not to place my own unsustained assertions in opposition to the results of clinical demonstrations or extreme probabilities. I have also endeavored to give the anatomy, or topography, of the structures under consideration, with as much accuracy as possible. There are no such obstacles now, in the way of writing intelligibly on diagnosis of diseases of the heart, as there were even twenty-five years ago. If medicine has not yet become a science, it has, at least, so far advanced as to recognize its own defects, and to be conscious of its absolute knowledge. Physicians now know where their knowledge ends and their theories begin; and in this they are far in advance of their forefathers. My aim has been to limit myself, as closely as possible, to known facts, and to indulge in no idle speculations, for the purpose of introducing some favorite theory of my own, in order to make myself seem original. I need not tell my intelligent readers that this treatise is merely an abstract; but it is hoped that they will find it what the author designed it to be, a careful and conscientious epitome of the knowledge now possessed by the profession on the subject of which it treats. My only claim to their commendation will consist in having placed the knowledge already possessed in a more accessible form, of having made the facts we daily need to use more tangible, by stripping them of some of their garniture, and grouping them in closer and more obvious relations to each other. Unusual opportunities for seeing the clinical and pathological aspects of these diseases have furnished him with the material illustrating the relation of clinical signs to post mortem appearances. Over a hundred cases from his individual records are given at some length. But the book is not an encyclopaedia. Little space is given to the Anatomy or Surgery of the Heart or Aorta, to Congenital Anomalies or Tumors, or to Parasites of the Heart. And there are obvious omissions of subjects that are sometimes treated at length in books on Heart Disease. For example, only brief mention is made of the Sphygmograph, because it is thought to mislead rather than help at the bed-side or in the consulting room. The practical value of the Sphygmanometer, however, is recognized. Nor do theories or methods of treatment that have little, or merely a historical value, receive: much notice. At the same time it is believed that the exceptional quantity and character of the data made use of, will give proper emphasis to the newer views expressed. From this point of view, the comparative values of diagnostic signs, and the relative frequency and gravity of valvular diseases are subjects that may enlist special interest. A good deal of space is given to modern methods of treatment. The volume is based on a series of articles that originally appeared in our Medical periodicals, but have been revised, while new chapters have been written. The majority of the cuts are original. With all its failings, for which the author must be responsible, it is hoped that the general practitioner will find it a useful addition to his store of handy-books, helping him to manage his heart cases more intelligently, and therefore with more satisfaction to himself and his patients. Now, if he had been a layman, or if he had not had the advantage of knowing something more than is taught in ordinary text books, he might have done as many a one has done under like circumstances - gone home, given himself up for lost, and verified the insurance doctor's prognosis by dying in a short time. He did nothing of the kind. He went to work as usual, made up his mind to insure himself, and has lived long enough to save the sum he would have paid in premiums three or four times over. Not very long ago an old patient of mine came to me in a terrible state of agitation. A note he brought me, announcing the fact that an insurance office to which he had applied for a policy could not see its way to accept him, explained his trouble. He had a valvular defect, it is true, but it was fully compensated. I assured him that there was not the least necessity for him to regard the refusal as a sentence of death, and advised him to follow the example of my friend and insure himself. Life took a new aspect to him he recovered his spirits and has not required any assistance from me since that time. As my object is to deal more especially with the therapeutic: of heart disease, I shall only devote to the consideration of the pathology of diseases of the heart and its vessels just so much Space as is needed for my purpose. My work on Iodide of Arsenic in Organic Disease of the Heart being now out of print, I have made use of the material it contained in the present volume, and also of the cases narrated in a paper which I contributed to the World's Homoeopathic Congress of r893, entitled The Curative Action of Homoeopathic Remedies in Cases of Organic Disease of the Heart.
