It encompasses more than one hundred bilateral agencies, global funds, and independent initiatives that interact with an equally complex and diverse set of institutions involved in financing and providing health care in developing countries. Numerous efforts have been made to better coordinate these activities in the interest of making them more effective. The Health Systems Funding Platform is one of the most recent of these initiatives. Established in 2009, it has advanced farthest in two countries, Ethiopia and Nepal, and is currently expanding to several others. This paper briefly assesses the Platform and argues that the way the initiative is proceeding differs little from prior initiatives, such as sector-wide approaches and budget support. However, the initiative does represent an opportunity to make global health aid more effective if it were to deepen its commitment to improving information for policy, link funding explicitly to well-chosen independently verified indicators, and establish an evaluation strategy to learn from its experience.

Book excerpt: Background: Over the last twenty years, despite unprecedented new resources to global health, there has been insufficient progress to achieve the health-related 2015 Millennium Development Goals. Health systems have been perceived as the binding constraint. Major global health agencies have expanded their funding priorities from disease specific programs to include support for health systems strengthening. Strengthening health systems broadly is beyond the mandate of agencies such as the Global Fund. Nonetheless, since its inception, the Global Fund has supported health systems; however, it has had a long-running organizational struggle with how to do it. Mechanisms for health systems support have varied, and proposals for health systems support consistently have been of a lower quality than disease proposals. The Technical Review Panel of the Fund has criticized it for lack of clarity about what it means by health systems strengthening and called into question the Fund's ability to support effective responses to health systems constraints. Purpose: The purpose of this dissertation is to present a case study of the Global Fund's policies and strategy on health systems strengthening using the lens of neo-institutional theory. This dissertation explores the role legitimacy and the cognitive beliefs about health systems strengthening held by stakeholders in the Global Fund's environment have played in shaping the Fund's policies on health systems strengthening. Methods: Qualitative research methods are used to examine the beliefs about health systems strengthening held by members of Board delegations and the role these beliefs played in shaping the Global Fund's strategy on health systems strengthening. Official Global Fund documents were reviewed and primary data was gathered through 31 semi-structured in-depth interviews with members of the Board delegations, Secretariat, Technical Partner organizations and other relevant stakeholders in the Global Fund's environment to complement the document review. Findings: (1) Health system strengthening is a vague concept. (2) Depending on the context, there are two meanings - a technical one and a political one. (3) There is dispute over what constitutes acceptable health systems strengthening work. (4) Health system strengthening is essential for the success of the Global Fund, but there is disagreement over whether the Global Fund should engage in health system strengthening; if it does, how, and to what extent. (5) Linking funding to measurable health outcomes is essential for legitimacy. Health systems strengthening is beyond the scope of the Fund's mandate, current technical capacity and organizational design. The Fund ought to focus on what it does well, which is financing scale up of essential inputs. The Health Systems Funding Platform provides an opportunity for the Fund to support public health system strengthening in a way that will appease key stakeholders but still allow it to stay within the bounds of its disease-specific mission.

Book excerpt: Shortages, geographic imbalances, and poor performance of health workers pose major challenges for improving health service delivery in developing countries. In response, development agencies have increasingly recognized the need to invest in Human Resources for Health (HRH) to assist countries in achieving their health system goals. In this paper we analyze the HRH-related activities of three multilateral agencies the Global Alliance for Vaccines and Immunization (GAVI); the global fund for AIDS, Tuberculosis, and Malaria (the global fund); and the World Bank. First, we reviewed the type of HRH-related activities that are eligible for financing within each agency. Second, we reviewed the HRH-related activities that each agency is actually financing. Third, we reviewed the literature to understand the impact that GAVI, the global fund, and the World Bank investments in HRH have had on HRH in developing countries. Our analysis found that by far the most common activity supported across all agencies is short-term, in-service training. There is relatively little investment in expanding pre-service training capacity, despite large health worker shortages in developing countries. We also found that the majority of GAVI and the global fund grants finance health worker remuneration, largely through supplemental allowances, with little information available on how payment rates are determined, how the potential negative consequences are mitigated, and how payments are to be sustained at the end of the grant period. Based on the analysis we argue that there is an opportunity for improved coordination between the three agencies at the country level in supporting HRH-related activities. Existing initiatives, such as the international health partnership and the health systems funding platform, may present viable and timely vehicles for the three agencies to implement this improved coordination.

Book excerpt: Medical crowdfunding is the practice of using websites to raise money from donors to pay for medical care or related expenses. While in terms of overall funding volume, medical crowdfunding should still be considered as a niche phenomenon, it is rapidly growing in many countries and is seen by many people as a way to cope with government cuts on public health financing. Examining the worldwide population of healthcare crowdfunding platforms, this study is the first to offer global and cross-platform evidence on healthcare crowdfunding by providing an assessment of how and where healthcare crowdfunding platforms emerge. We explore the relationship between healthcare crowdfunding and national health systems, finding evidence of a substitution effect when public health coverage is low. Moreover, our findings support the evidence that the number of successfully funded health projects is higher when the platform is not investment-based or dedicated only to healthcare projects.

Frequently, partnerships between donors and other stakeholders can coalesce on a specific topical area of expertise and interest. However, to sustain these successes and continue progress, there is a growing recognition of the need to strengthen health systems more broadly and build functional administrative and technical infrastructure that can support health services for all, improve the health of populations, increase the purchasing and earning power of consumers and workers, and advance global security. In June 2015, the National Academies of Sciences, Engineering, and Medicine held a workshop on the role of public-private partnerships (PPPs) in health systems strengthening. Participants examined a range of incentives, innovations, and opportunities for relevant sectors and stakeholders in strengthening health systems through partnerships; to explore lessons learned from pervious and ongoing efforts with the goal of illuminating how to improve performance and outcomes going forward; and to discuss measuring the value and outcomes of investments and documenting success in partnerships focused on health systems strengthening. This report summarizes the presentations and discussions from the workshop.

This review summarizes HSPA domains and indicators used by Member States in their HSPA or health system-related reports. Thirty Member States published in the English language and from their latest documents, 1485 distinct indicators were extracted. The number of indicators reported per Member State ranged from 9 to 146, with a mean of 50. Among the 14 domains of the WHO 2007 framework, service delivery and improved health were covered by virtually all Member States analysed (30 and 29, respectively), but coverage varied for the other 12 domains, with health workforce and financing having good coverage (25 and 26, respectively) but others, such as safety, efficiency, coverage or responsiveness, covered in only 20-30% of documents. Further refinement of frameworks, both in clarity on scope and function and in the conceptual robustness of domains, is warranted and further standardization of generic sets of indicators should be sought.

The answers given vary according to political ideology, economic expediency, and the moral standing of individuals and society. The sources of funding are essentially two: either the individual directly, or a larger group acting on his behalf. In the second case, we have two main categories. The individual is either covered by private for-profit insurance, or by a public insurance scheme financed by mandatory employment contributions and/or by taxes on income and/or wealth. The economic implications of each form of health insurance are immense-for individuals, employers, the government, and for the economy as a whole. The main differentiation is the position of health care in the value system of society. If health care is considered a right, its financing must be similar to that of other public goods or rights such as justice, national security, personal safety, basic education, etc. At the same time, the provision of all public goods is a public responsibility and government is judged by how well it measures up to this responsibility. If, on the other hand, health care is considered a good, bought and sold on the market, then it is up to individuals to provide for themselves. Obviously, this fundamental issue belongs to the sphere of politics and is up to society to judge, according to its code of ethics. The time to decide has come in America, somewhat belatedly, but in a way more acute than ever. The health of individuals, but also and mainly the economic health of the nation, depends on the decision.
