The GRP continues to build on its past work and seeks to learn, think about, and plan for how and where it can have the greatest cumulative and lasting impacts.

Legal settlements with the companies held responsible led the federal government to ask the National Academy of Sciences to form and administer a 30-year program to enhance oil system safety, human health, and environmental resources in the Gulf of Mexico and other U.S. continental shelf areas where offshore oil and gas exploration and production occur or are under consideration. The new Gulf Research Program will receive $500 million to support activities using three broad approaches: research and development, education and training, and environmental monitoring. The Gulf Research Program: A Strategic Vision establishes the Program\'s foundation and introduces its mission, goals, and objectives. Understanding the interrelationships among health, ecological, and economic impacts of disasters and other environmental stressors will be crucial to addressing these challenges. Opportunities for the Gulf Research Program: Community Resilience and Health summarizes a Gulf Research Program workshop held on September 22-23, 2014, in New Orleans, Louisiana. The workshop examined opportunities to improve the health, well-being, and resilience of communities in the Gulf region through discussions with about 50 participants with diverse expertise and experience. These discussions identified perceived needs, challenges, and opportunities that align with the Gulf Research Program's mission and goals - particularly its goal to improve understanding of the connections between human health and the environment to support the development of health and resilient Gulf communities. This workshop is expected to lead to the development of additional Program activities and opportunities for the research community.

In some cases, billions of dollars from public, private and charitable sources are invested to help communities recover. National rhetoric often characterizes these efforts as a "return to normal." But for many American communities, pre-disaster conditions are far from optimal. Large segments of the U.S. population suffer from preventable health problems, experience inequitable access to services, and rely on overburdened health systems. A return to pre-event conditions in such cases may be short-sighted given the high costs - both economic and social - of poor health. Instead, it is important to understand that the disaster recovery process offers a series of unique and valuable opportunities to improve on the status quo. Capitalizing on these opportunities can advance the long-term health, resilience, and sustainability of communities - thereby better preparing them for future challenges. Healthy, Resilient, and Sustainable Communities After Disasters identifies and recommends recovery practices and novel programs most likely to impact overall community public health and contribute to resiliency for future incidents. This book makes the case that disaster recovery should be guided by a healthy community vision, where health considerations are integrated into all aspects of recovery planning before and after a disaster, and funding streams are leveraged in a coordinated manner and applied to health improvement priorities in order to meet human recovery needs and create healthy built and natural environments. The diagnostic process is a complex, collaborative activity that involves clinical reasoning and information gathering to determine a patient's health problem. According to Improving Diagnosis in Health Care, diagnostic errors-inaccurate or delayed diagnoses-persist throughout all settings of care and continue to harm an unacceptable number of patients. It is likely that most people will experience at least one diagnostic error in their lifetime, sometimes with devastating consequences. Diagnostic errors may cause harm to patients by preventing or delaying appropriate treatment, providing unnecessary or harmful treatment, or resulting in psychological or financial repercussions. The committee concluded that improving the diagnostic process is not only possible, but also represents a moral, professional, and public health imperative. Improving Diagnosis in Health Care, a continuation of the landmark Institute of Medicine reports To Err Is Human (2000) and Crossing the Quality Chasm (2001), finds that diagnosis-and, in particular, the occurrence of diagnostic errorsâ€"has been largely unappreciated in efforts to improve the quality and safety of health care. Without a dedicated focus on improving diagnosis, diagnostic errors will likely worsen as the delivery of health care and the diagnostic process continue to increase in complexity. Just as the diagnostic process is a collaborative activity, improving diagnosis will require collaboration and a widespread commitment to change among health care professionals, health care organizations, patients and their families, researchers, and policy makers. The recommendations of Improving Diagnosis in Health Care contribute to the growing momentum for change in this crucial area of health care quality and safety.

Book excerpt: Valued for its ecological richness and economic value, the U.S. Gulf of Mexico is under substantial pressure from human activities. The Deepwater Horizon platform explosion and oil spill significantly damaged Gulf ecosystems and led to the largest ecological restoration investment in history. The unprecedented number and diversity of restoration activities provide valuable information for future restoration efforts, but assessment efforts are hampered by many factors, including the need to evaluate the interaction of multiple stressors and consider long-term environmental trends such as sea level rise, increasing hurricane intensity, and rising water temperatures. This report offers a comprehensive approach to assess restoration activities beyond the project scale in the face of a changing environment. A main component of this approach is using different types of scientific evidence to develop "multiple lines of evidence" to evaluate restoration efforts at regional scales and beyond, especially for projects that may be mutually reinforcing (synergistic) or in conflict (antagonistic). Because Gulf of Mexico ecosystems cross political boundaries, increased coordination and collaboration is needed, especially to develop standardized data collection, analysis, synthesis, and reporting. With these improvements, program-level adaptive management approaches can be used more effectively to assess restoration strategies against the backdrop of long-term environmental trends.

Opportunities for the Gulf Research Program: Monitoring Ecosystem Restoration and Deep Water Environments summarizes a Gulf Research Program workshop held on September 3-4, 2014 in New Orleans, Louisiana. The workshop gathered about 40 participants from the energy industry, state and federal government, academia, and nongovernmental organizations to examine two broad issues that were seen as time-sensitive opportunities in light of significant investments in the Gulf for restoration and accelerating development of energy resources in the deep Gulf: monitoring ecosystem restoration and deep water environments. As participants explored potential opportunities for the Program to consider, they noted the essential role that communication and outreach play in successful monitoring, and the importance of applying an ecosystem service approach to monitoring, forging partnerships among stakeholders, and supporting efforts to organize and manage monitoring data.

Book excerpt: Myalgic encephalomyelitis (ME) and chronic fatigue syndrome (CFS) are serious, debilitating conditions that affect millions of people in the United States and around the world. ME/CFS can cause significant impairment and disability. Despite substantial efforts by researchers to better understand ME/CFS, there is no known cause or effective treatment. Diagnosing the disease remains a challenge, and patients often struggle with their illness for years before an identification is made. Some health care providers have been skeptical about the serious physiological - rather than psychological - nature of the illness. Once diagnosed, patients often complain of receiving hostility from their health care provider as well as being subjected to treatment strategies that exacerbate their symptoms. Beyond Myalgic Encephalomyelitis/Chronic Fatigue Syndrome proposes new diagnostic clinical criteria for ME/CFS and a new term for the illness - systemic exertion intolerance disease(SEID). According to this report, the term myalgic encephalomyelitis does not accurately describe this illness, and the term chronic fatigue syndrome can result in trivialization and stigmatization for patients afflicted with this illness. Beyond Myalgic Encephalomyelitis/Chronic Fatigue Syndrome stresses that SEID is a medical - not a psychiatric or psychological - illness. This report lists the major symptoms of SEID and recommends a diagnostic process.One of the report's most important conclusions is that a thorough history, physical examination, and targeted work-up are necessary and often sufficient for diagnosis. The new criteria will allow a large percentage of undiagnosed patients to receive an accurate diagnosis and appropriate care. Beyond Myalgic Encephalomyelitis/Chronic Fatigue Syndrome will be a valuable resource to promote the prompt diagnosis of patients with this complex, multisystem, and often devastating disorder; enhance public understanding; and provide a firm foundation for future improvements in diagnosis and treatment.

Because they lack traditional university departmental boundaries, researchers at field stations have the opportunity to converge their science disciplines in ways that can change careers and entire fields of inquiry. Field stations provide physical space for immersive research, hands-on learning, and new collaborations that are otherwise hard to achieve in the everyday bustle of research and teaching lives on campus. But the separation from university campuses that allows creativity to flourish also creates challenges. Sometimes, field stations are viewed as remote outposts and are overlooked because they tend to be away from population centers and their home institutions. This view is exacerbated by the lack of empirical evidence that can be used to demonstrate their value to science and society. Enhancing the Value and Sustainability of Field Stations and Marine Laboratories in the 21st Century summarizes field stations' value to science, education, and outreach and evaluates their contributions to research, innovation, and education. This report suggests strategies to meet future research, education, outreach, infrastructure, funding, and logistical needs of field stations. Today's technologies - such as streaming data, remote sensing, robot-driven monitoring, automated DNA sequencing, and nanoparticle environmental sensors - provide means for field stations to retain their special connection to nature and still interact with the rest of the world in ways that can fuel breakthroughs in the environmental, physical, natural, and social sciences. The intellectual and natural capital of today's field stations present a solid platform, but many need enhancements of infrastructure and dynamic leadership if they are to meet the challenges of the complex problems facing the world. The agency's missions abroad and their host country partners work in quality improvement, but a lack of evidence about the best ways to facilitate such improvements has constrained their informed selection of interventions. Six different methods - accreditation, COPE, improvement collaborative, standards-based management and recognitions (SBM-R), supervision, and clinical in-service training - currently make up the majority of this investment for USAID missions. As their already substantial investment in quality grows, there is demand for more scientific evidence on how to reliably improve quality of care in poor countries. USAID missions, and many other organizations spending on quality improvement, would welcome more information about how different strategies work to improve quality, when and where certain tools are most effective, and the best ways to measure success and shortcomings. To gain a better understanding of the evidence supporting different quality improvement tools and clarity on how they would help advance the global quality improvement agenda, the Institute of Medicine convened a 2-day workshop in January 2015. The workshop's goal was to illuminate these different methods, discussing their pros and cons. This workshop summary is a description of the presentations and discussions.
