Book excerpt: Considerable controversy exists among demographers, economists, and sociologists over the causes of fertility change in developing and developed countries. The neoclassical economic approach to fertility is embraced by its supporters because it facilitates the application of sophisticated consumer and household production theory to one of the most private and intimate questions: a couple's reproductive behavior. Despite the theoretical appeal of the economic approach, it has been eschewed by many critics because of its lack of social and institutional context, its neglect of cultural factors, and its requirement of 'rationality'. The integration of social interaction with economic fertility models in this book emerges as a powerful tool to overcome many of these criticisms. First, the analysis provides a formal integration of economic, sociological, and other approaches to fertility, and shows that there is a useful and promising agenda at the intersection of these schools. The second and more important goal is to sharpen the analytic lens with which theorists from different schools investigate fertility. For economists the work shows the advantages of moving beyond individual decision-making and embedding fertility decisions in a 'local environment' with interpersonal information flows, 'atmospheric' or social externalities, norms, and customs. For sociologists the work shows that theorizing about interactions within social networks can be more sophisticated. The implications of social networks depend substantially on the specific contexts and stages of the demographic transition, and these differences can be used to empirically distinguish between social learning and social influence. Thirdly, the findings have important implications for population policy. The analyses in this book indicate when family planning is likely to diffuse and lead to rapid adoption of birth control, and they derive conditions where Pareto-improving policy measures are likely to exist.

Those disparities are caused not only by fundamental differences in health status across segments of the population, but also because of inequities in factors that impact health status, so-called determinants of health. Only part of an individual's health status depends on his or her behavior and choice; community-wide problems like poverty, unemployment, poor education, inadequate housing, poor public transportation, interpersonal violence, and decaying neighborhoods also contribute to health inequities, as well as the historic and ongoing interplay of structures, policies, and norms that shape lives. When these factors are not optimal in a community, it does not mean they are intractable: such inequities can be mitigated by social policies that can shape health in powerful ways. Communities in Action: Pathways to Health Equity seeks to delineate the causes of and the solutions to health inequities in the United States. This report focuses on what communities can do to promote health equity, what actions are needed by the many and varied stakeholders that are part of communities or support them, as well as the root causes and structural barriers that need to be overcome.

Book excerpt: In this paper, micro-level data from a survey of 4,849 women in Malawi are linked to information from a service availability questionnaire to assess the relative importance of socioeconomic background and various aspects of family planning provision on contraceptive use in one country in sub-Saharan Africa. Maximum-likelihood probit is used to assess the independent influence of four distinct dimensions of family planning effort on contraceptive use: mass media exposure, i.e., promotion of family planning through radio and print messages; contraceptive choice; accessibility of contraceptive services; and service quality. Results indicate that all four components of family planning effort contribute to the use of modern contraceptives in Malawi, although their relative importance varies significantly across different segments of the population.

In doing so, it focuses on fertility outcomes (the number and timing of births), women's health and socio-economic outcomes (mortality, human capital, and labor force participation), and children's health and socio-economic outcomes throughout the life cycle. Although effect sizes are heterogeneous, long-term studies imply that in practice, family planning programs may only explain a modest share of fertility decline in real-world settings (explaining 4-20% of fertility decline among studies finding significant effects). Family planning programs may also have quantitatively modest - but practically meaningful - effects on the socio-economic welfare of individuals and families.

As of 1998, 179 countries with 99 percent of the world's population had some form of family planning program. Despite their pervasiveness, family planning programs have caused controversy and drawn criticism from a variety of perspectives, primarily for two reasons. First, they deal with a sensitive subject: birth control. Public discussion of the issue was taboo for many years and continues to evoke strong reactions from some quarters. Adding to this sensitivity, the emotionally charged debate over abortion has at times spilled over into the discussion of family planning. Second, concern about the negative effects of rapid population growth and high fertility in the developing world spawned a political advocacy movement that promoted particular public policies, family planning among them. While this movement enjoyed considerable support in both developing and donor nations, it also generated political opposition and raised questions among some social scientists and others. This report examines the origins and evolution of family planning programs in the context of the major criticisms and controversies surrounding them. It also explores how programs have responded to these criticisms and assesses the validity of these criticisms as reflected in the research literature. Although some of the criticisms are based on ideological perspectives that scientific research does little to illuminate, simply describing these perspectives should be useful.

Book excerpt: This volume helps fill the gap left from insufficiently archived details of family planning programs carried out in many developing countries from the 1950s through the 1980s of their operations, their commonalities, and their differences, with much useful information and informed analysis. The programs were complex undertakings in difficult settings that had little prior experience to draw upon. Not surprisingly, as the case studies described here demonstrate, no single strategy was available that could be employed across these diverse situations, and procedures that were successful in one country did not necessarily function well in another. The case studies also indicate that developing a successful program was as much an art as a science. The key ingredient was being able to distinguish when a somewhat radical new approach was needed and when only some fine-tuning was necessary. While not a focus of this book, the family planning programs had several important, indirect effects on the field of population studies that merit attention as part of the record. First, uncertainty about the programs' worth and how to measure the extent of their success spurred a great deal of research on the measuring and modeling of fertility and contraceptive practice, on fecundity issues, on the effect of marriage patterns on fertility, and on a host of related topics. Second, the programs greatly advanced the science of evaluation. Third, the programs led demographers to work with specialists from many other disciplines, including public health, economics, sociology, political science, and psychology. Finally, the family planning efforts attracted many new and talented people to the field of population studies. The 23 case studies presented here were the earliest national efforts to establish organized family planning programs for entire populations. The resulting chapters naturally vary in terms of their balance of history, analysis, and personal reflections given the wide diversity of national contexts and program types. The study's overall conclusion is that, for the most part, the family planning program "experiment" worked: policy and program interventions contributed substantially to the revolutionary rise of contraceptive use and to the decline in fertility that has occurred in the developing world in the past three decades.
