Book excerpt: ABSTRACT: Background: Published research addressing thoracic spine pain is less prevalent in comparison to cervical and lumbar pain. Because there are similar anatomic features between the lower cervical spine and upper thoracic spine, thoracic spine manipulation (TSM) is applicable when treating both regions. The purpose of this review was to determine the effectiveness of TSM in reducing pain and improving function in both the thoracic spine and the cervical spine due to the biomechanical link between the 2 regions. Our patient is a 26 year old female with a 7 year history of thoracic spine pain, cervical stiffness, and a history of herniated discs in the thoracic region, with the specific location and extent of the injury unknown. Methods: A comprehensive search was performed using the PubMed, MEDLINE, CINAHL, and PEDro databases. The search yielded 14 articles that met predefined inclusion and exclusion criteria. These articles were then appraised and annotated for quality and applicability to the clinical question. Articles were synthesized in order to make a final clinical decision. Results: A total of 7 high quality articles were selected for review, including 5 RCTs and 2 systematic reviews. Discussion/Conclusion: According to the literature, TSM is a beneficial intervention for patients with thoracic pain and cervical pain with immediate and short-term reductions in pain and disability. Clinicians should consider using TSM for patients with cervical spine pain and acute or subacute thoracic spine pain. TSM is not recommended as a first line of treatment for our patient due to the lack of literature investigating the use of TSM for patients with chronic thoracic spine pain.
