The efficacy of the ankle taping for injury prevention has long been under scrutiny as numerous studies have shown that tape rapidly loses its ability to constrain ankle motion with exercise. Consequently, ankle braces (orthoses) are being used with increasing frequency for the prevention and functional management of ankle injuries. However, the motion restraining qualities of ankle orthoses have not been widely evaluated in closed kinetic chain environments under physiologic loads. The primary purpose of this study was to compare the abilities of four ankle orthoses (ankle taping, lace-up brace, semirigid orthosis and hybrid brace) against a control condition (no brace or tape) to control subtalar and talocrural motion during running on a laterally-tilted treadmill at 16.2 km/h before and after exercise. It has been hypothesized that ankle orthoses make a secondary contribution to injury prevention through enhanced proprioception. The secondary purpose of this study was to quantify the effects of the aforementioned ankle orthoses on postural stability during single-limb stance following a bout of exercise. Fifteen healthy university students (8 men and 7 women) with no history of significant ankle injuries (age, mean ± SD: 22.9 ± 3.9 years) volunteered to participate in this study. Three-dimensional kinematic data were captured with an active infrared digital camera system sampling at 120 Hz. To address the first question, data analyses were performed using 2way univariate (Ankle Orthoses x Pre/Post-Exercise x Subjects) (5 x 2 x 15) repeated measures analysis of variance (ANOVA) to determine the existence of differences among three closed and four open kinematic chain dependent measures before and after exercise. Maximum inversion angles (MAXINV) were similar for all ankle orthoses, with no orthosis limiting inversion during tilted treadmill running significantly more than another, or compared to the control condition, either before or after exercise (p>.05). Pre-exercise MAXINV group means and standard deviations during treadmill running ranged from 6.8 ± 3.4 deg with the Royce Medical Speed Brace to 9.5 ± 4.1 deg in the tape condition; post-exercise MAXINV mean values ranged from 7.6 ± 3.2 deg for the Aircast Sport Stirrup to 9.1 ± 4.6 deg with closed basketweave tape. While not statistically significant (p=0.10), ankle taping provided the least amount of inversion restraint, both before and after the exercise bout. The MAXINV angles measured during treadmill running (8.2 ± 4.0 deg) and open chain inversion AROM measured with a goniometer (34.5 ± 6.2 deg) were not related (r=-0.0003). The compressive forces present during closed kinetic chain activity are known to increase joint stability and thus may explain why MAXINV under dynamic varus loads was so much less in magnitude than inversion AROM measured under open kinetic chain conditions. The nonlinear relationship of these two variables supports our contention that reports of the motion controlling properties of ankle orthoses measured in open kinetic chain environments should not be used to infer the response characteristics of these same orthoses under dynamic, physiologic loads. To address the second question, data were analyzed using 3-way univariate (Ankle Orthoses x Pre/Post-Exercise x Eyes Open/Closed x Subjects) (5 x 2 x 2 x 15) repeated measures ANOVAs. Subjects' postural stability was assessed using a Biodex Balance System with eyes open and eyes closed conditions, before and after an exercise bout. The ankle orthoses evaluated did not influence postural stability as measured by mediolateral sway index, anteroposterior sway index, and overall sway index. Removal of visual perception via blindfolding resulted in significant decreases in all three measures of postural stability (p=.001). There was poor association among the closed chain postural stability parameters and the open chain AROM measures. These correlations ranged from r=.04 to .17, indicating minimal relationship between the amount of AROM permitted by the orthoses and postural stability as quantified by this method.

Book excerpt: BACKGROUND: Ankle sprains are the most common injuries among physically active people, with 40-70% developing functional ankle instability (FAI), characterized by frequent episodes of "giving way" and sensations of joint instability. Kinesio Taping, ® an intervention that has recently gained popularity may help decrease postural control deficits related to FAI. This study examined the immediate and prolonged effects of Kinesio Taping® on postural control in subjects with FAI and investigated differences between healthy, coper, and unstable ankles as measured through single-limb balance on a forceplate. METHODS: Sixty physically active, college-aged participants (height: 172.5 ± 9.7 cm, mass: 74.2 ± 16.2 kg, age: 21.5 ± 2.6 years) were sorted into three separate groups: healthy control, copers, and FAI. Subjects were stratified using the Cumberland Ankle Instability Tool (CAIT) combined with their history of ankle injury. Dependent variables were time to boundary (TTB) measures and traditional center of pressure (COP) measures in both the mediolateral (frontal) and anteroposterior (sagittal) planes. Testing was performed at baseline, immediately after application of the tape, 24 hours post-tape application, and immediately after removal of the tape. RESULTS: Significant group main effects for COP standard deviation (F2,57 = 4.309, p = 0.018) and range (F2,57 = 4.918, p = 0.011) in the sagittal plane were noted. Significant condition main effects for TTB absolute minima (frontal plane, F 3,159 = 1.607, p = 0.015) and standard deviation (frontal, F3,138 = 5.710, p = 0.002 and sagittal plane, F3,141 = 0.889, p = 0.029) were also noted. Post hoc testing revealed decreased COP standard deviation and range for the ankle instability group compared to controls and significant improvements at 24 hours post-tape in all groups for TTB absolute minima (p = 0.025) and standard deviation (frontal plane, p = 0.002). CONCLUSIONS: Deficits in COP variables were seen in unstable ankles as compared to both healthy ankles and copers. Minor improvements in TTB measures after 24 hours of continuous wear of Kinesio® tape were observed in healthy, coper, and unstable ankles. Since between group differences were seen in COP measures, but improvements post-tape application were seen only in TTB variables, we can conclude that Kinesio® tape did not improve the specific postural control deficits experienced by unstable ankles. Therefore, while Kinesio® tape may provide a small benefit to postural control at the ankle, it does not appear to be effective at improving ankle instability deficits. Since prophylactic ankle braces are widely used and accepted, it is critical to understand the effects prophylactic ankle braces are having on ankle functionality and postural control both in a braced and non-braced state. However, the effects of prophylactic ankle braces over the period of a season has limited research. Thirteen collegiate women's basketball players, who are required to wear prophylactic ankle braces through a 20-week season, participated in this study. The purpose of this study was to determine the effect of prophylactic ankle bracing on passive range of motion, static balance (COG displacement, COG pattern tracking over a period of time), and dynamic balance (COG displacement in a dynamic setting, COG pattern tracking over a period, reach measures through SEBT) in collegiate women's basketball players across a 20-week season span. We rejected the null hypothesis and found differences in ROM, increases in Postural Stability scores, and improvements with Limits of Stability (LOS) scores. We also found a significant decrease in the anterior/lateral quadrant of the ASL test which suggests a change in COG placement over the 20-week span of the season. Although wearing prophylactic ankle bracing over the period of season showed shifts in ROM, improvements in COG location and postural stability scores, further research is needed on wearing ankle braces over a long period of time to determine when these changes are happening.

Book excerpt: The purpose of this study was to determine if prophylactic ankle tape and/or ankle braces improve dynamic stability in TTS measure. All subjects were healthy and had no prior history of ankle injuries. Data collection consisted of each subject performing a single leg jump-landing with ankle tape, ankle brace, combination of the two, and control (no tape or brace) conditions. Dynamic stability was assessed with time to stabilization force plate measure. Significant plane by ankle tape interaction (p=0.045) was found. No significant plane by ankle tape by ankle brace interaction (p=0.637), no significant ankle tape by ankle brace interaction (p=0.483), or plane by ankle brace interaction (p=0.697) were found. A notable finding was that subjects took longer to stabilize in the anterior/posterior direction than medial/lateral direction. In conclusion ankle tape, ankle brace, and the combination of ankle tape and ankle brace did not statistically improve dynamic stability in healthy ankles.
